基于DEA的河南省新農(nóng)合基金運行效率研究

【摘要】:新型農(nóng)村合作醫(yī)療(簡稱新農(nóng)合)是我國現(xiàn)行的農(nóng)村基本醫(yī)療保障制度,與城鎮(zhèn)居民基本醫(yī)療保險、城鎮(zhèn)職工基本醫(yī)療保險共同構(gòu)成社會基本醫(yī)療保險體系。自2009年"新醫(yī)改"方案出臺到2015年年末,河南省新農(nóng)合的發(fā)展呈現(xiàn)良好的上升態(tài)勢,參合人數(shù)達(dá)8285.06萬人,參合率達(dá)99.12%。新農(nóng)合基金是為實現(xiàn)新農(nóng)合制度目標(biāo)服務(wù)的、根據(jù)新農(nóng)合制度的相關(guān)政策籌集起來的資金。新農(nóng)合基金的籌集、統(tǒng)籌管理和補償是新農(nóng)合制度執(zhí)行中的重要內(nèi)容,是新農(nóng)合順利實施、平穩(wěn)運行的根本保證。分析評價河南省不同地區(qū)新農(nóng)合基金運行效率的差異,找出產(chǎn)生低效率的原因,能夠進(jìn)一步完善新農(nóng)合制度。本文對河南省新農(nóng)合的實施現(xiàn)狀進(jìn)行了籌資、統(tǒng)籌管理和補償三個方面的介紹,分析了河南省新農(nóng)合基金運行中存在的問題。然后通過基金和效率評價的相關(guān)理論,確定運用前沿分析方法的非參數(shù)法,即數(shù)據(jù)包絡(luò)分析方法中的規(guī)模報酬可變模型和全要素生產(chǎn)率指數(shù)模型,以輸出為導(dǎo)向,選取河南省24個樣本縣2013年到2015年的新農(nóng)合基金數(shù)據(jù)進(jìn)行效率分析,并針對分析結(jié)果和存在的問題提出了提高新農(nóng)合基金運行效率的建設(shè)性意見。本文的研究結(jié)論有:首先,河南省的新農(nóng)合基金運行效率良好。每年的技術(shù)效率平均值均在75%以上并逐年遞增。北部的新農(nóng)合基金運行效率總體上要優(yōu)于中部和南部,中部地區(qū)的技術(shù)效率增長最快,南部地區(qū)的效率雖然在三年間有所改善,但是仍不及北部地區(qū)。其次,河南省南部地區(qū)的低技術(shù)效率主要歸因于偏低的純技術(shù)效率,即在等量投入的情況下產(chǎn)出相對不理想,使籌集的資金不能充分發(fā)揮作用。而且規(guī)模報酬整體處于遞減的狀態(tài),說明規(guī)模過大。反觀中部地區(qū)的規(guī)模報酬整體是遞增的狀態(tài),暗示了這些樣本縣應(yīng)增大新型農(nóng)村合作醫(yī)療基金的規(guī)模。再次,河南省2013年到2015年無效地區(qū)的規(guī)模報酬變化遞增數(shù)量逐年減少,規(guī)模報酬變化遞減數(shù)量逐年增加,由整體遞增變?yōu)檎w遞減,表明河南省新農(nóng)合基金規(guī)模一直在增加,但基金運行缺乏一個相對有效的模式,需要加以改善,達(dá)到規(guī)模最優(yōu)。最后,投入金額的多少與技術(shù)效率的高低并不成正比關(guān)系,如中部地區(qū)新密市(DMU11)基金投入金額最多,但是它在2013年的技術(shù)效率0.581,2014年為0.766,2015年為0.856,雖逐年上升,但得分不是最高。而滑縣(DMU5)的投入金額在樣本縣中處于中低層次,卻能夠?qū)崿F(xiàn)基金的高效運行。
[Abstract]:The New Rural Cooperative Medical system (NCMS) is the current rural basic medical insurance system in China, which, together with the basic medical insurance for urban residents and urban workers, constitutes the basic social medical insurance system. Since the "new medical reform" program was introduced in 2009 to the end of 2015, the development of the new rural cooperation in Henan Province showed a good rising trend, the number of participants reached 82.8506 million, the participation rate reached 99.12. The new rural cooperative fund serves to realize the goal of the new rural cooperation system and raises funds according to the relevant policies of the new rural cooperation system. The raising, the overall management and the compensation of the new rural cooperative fund are the important contents in the implementation of the new rural cooperation system, and the fundamental guarantee for the smooth implementation and smooth operation of the new rural cooperative fund. This paper analyzes and evaluates the differences in the operating efficiency of NCMS funds in different regions of Henan Province, finds out the causes of low efficiency, and can further improve the NCMS system. This paper introduces the current situation of the implementation of NCMS in Henan Province from the aspects of financing, overall management and compensation, and analyzes the problems existing in the operation of NRCF in Henan Province. Then, through the relevant theories of fund and efficiency evaluation, the non-parametric method using frontier analysis method, namely the variable scale reward model and the total factor productivity index model in the data envelopment analysis method, is determined to be output oriented. This paper selects 24 sample counties of Henan Province from 2013 to 2015 to analyze the efficiency of the new rural cooperative fund, and puts forward some constructive suggestions to improve the efficiency of the new rural cooperative fund in view of the analysis results and the existing problems. The conclusions of this paper are as follows: first of all, the new agricultural cooperative fund in Henan Province has a good operating efficiency. The average technical efficiency of each year is above 75% and increasing year by year. The efficiency of the NCMS fund in the north is better than that in the central and southern regions in general. The technical efficiency in the central region has the fastest growth, and the efficiency in the southern region has improved in three years, but it is still not as good as the northern region. Secondly, the low technical efficiency in the southern region of Henan Province is mainly attributed to the low pure technical efficiency, that is, the output is relatively unsatisfactory under the condition of equal input, which makes the funds raised can not play a full role. And scale reward is in the state of decline as a whole, indicating that the scale is too large. On the contrary, the scale reward of the central region is increasing, suggesting that these sample counties should increase the scale of the new rural cooperative medical fund. Thirdly, from 2013 to 2015, the number of scale returns in invalid areas of Henan Province decreased year by year, the number of scale returns decreased year by year, from the overall increase into the overall decline, indicating that the scale of Henan New Rural Cooperation Fund has been increasing. However, fund operation lacks a relatively effective model, which needs to be improved to achieve optimal scale. Finally, the amount of investment is not proportional to the level of technical efficiency, such as in the central region of Xinmi City, DMU 11) the largest amount of investment, but its technical efficiency in 2013, 2014 is 0.766, 2015 is 0.856, although rising year by year, but not the highest score. The investment amount of MU 5 is in the middle and low level in the sample county, but it can realize the efficient operation of the fund.
