新醫(yī)改下公立醫(yī)院醫(yī)師職業(yè)壓力與影響因素實證研究

【摘要】:在當前經(jīng)濟高速發(fā)展、社會結構快速轉型的背景下,醫(yī)療體制改革不斷深入,醫(yī)師已經(jīng)成為三高(高技術、高風險、高壓力)人群。隨著生活節(jié)奏的加快,醫(yī)師群體不僅在工作中承擔著較重的工作負荷,在生活和學習中也越來越擔負著過重的壓力。深入探究醫(yī)師職業(yè)壓力及產(chǎn)生壓力的影響因素制定緩解醫(yī)師職業(yè)壓力的措施并實施干預,不僅對緩解醫(yī)師職業(yè)壓力、提高醫(yī)療質(zhì)量、緩解醫(yī)患矛盾具有重要意義,同時為國家制定更加完善的醫(yī)療衛(wèi)生政策進一步提供科學依據(jù)。本文在查閱國內(nèi)外文獻及相關文件的基礎上,在參考國外已有的職業(yè)壓力量表基礎基礎上,結合我省目前醫(yī)師群體職業(yè)壓力現(xiàn)狀,在此基礎上采用自編式問卷,重點對安徽省幾所公立三甲醫(yī)院的226名醫(yī)師工作者進行個案研究,利用SPSS和EpiData軟件進行統(tǒng)計與分析并得出結論。歸納總結醫(yī)師群體所面對的職業(yè)壓力狀況,具體情況主要表現(xiàn)為:職業(yè)壓力在年齡、職稱、工作年限性等方面存在明顯差異;并對職業(yè)壓力產(chǎn)生的影響因素加以分析:工作任務負荷過重、科研壓力重、醫(yī)療風險突出、醫(yī)患矛盾嚴峻、收入不滿意、新醫(yī)改政策不到位等是主要影響醫(yī)師職業(yè)壓力產(chǎn)生的因素。本文最后從政府與社會、組織、個人三個方面提出緩解醫(yī)院醫(yī)師職業(yè)壓力的對策建議。包括:切實履行政府職能,深化醫(yī)療衛(wèi)生體制改革;改革醫(yī)療保障制度,推進大病保險;營造寬松的社會輿論環(huán)境;醫(yī)院內(nèi)部組織管理方面要加強科學共同體內(nèi)部約束;解決醫(yī)療服務與科研工作的沖突,創(chuàng)設寬松的醫(yī)療與科研內(nèi)部環(huán)境;個人要正視職業(yè)壓力,積極主動地去調(diào)節(jié)。正文共分為四個部分,第一部分主要介紹了相關背景及研究的目的與意義。第二部分主要是對醫(yī)師職業(yè)壓力的一個概述。第三部分以安徽省幾家公立三甲醫(yī)院醫(yī)師的職業(yè)壓力狀況為個案,通過實證研究得出新醫(yī)改下醫(yī)師的職業(yè)壓力狀況并分析其產(chǎn)生職業(yè)壓力的因素。第四部分是在第三章結論的基礎之上,提出緩解醫(yī)師職業(yè)壓力的對策建議。
[Abstract]:With the rapid development of economy and the rapid transformation of social structure, the reform of the medical system is deepening, and doctors have become the "three high" (high technology, high risk, high pressure) people. With the acceleration of life rhythm, doctors not only bear a heavy workload in their work, but also bear more and more heavy pressure in their life and study. It is of great significance to explore the influencing factors of doctors' occupational stress and to make measures to relieve doctors' occupational pressure and to carry out intervention, which is of great significance not only to relieve doctors' occupational pressure, improve the quality of medical treatment and alleviate the contradiction between doctors and patients, but also to improve the quality of medical treatment. At the same time for the country to formulate more perfect medical and health policy to provide further scientific basis. On the basis of consulting domestic and foreign literature and related documents, and referring to the existing occupational stress scale in foreign countries, combined with the current situation of occupational stress among doctors in our province, this paper adopts a self-made questionnaire. This paper focuses on the case study of 226 physician workers in several public third Class A hospitals in Anhui Province, and makes statistics and analysis by using SPSS and EpiData software and draws a conclusion. Summing up the occupational stress status faced by the group of doctors, the specific situation is as follows: there are obvious differences in age, professional title, working life and so on; The influencing factors of occupational stress are analyzed as follows: heavy workload, heavy scientific research pressure, prominent medical risk, severe contradiction between doctors and patients, unsatisfactory income. The new medical reform policy is not in place and so on are the main factors that affect the professional pressure of doctors. Finally, this paper puts forward countermeasures and suggestions to relieve the occupational pressure of hospital doctors from three aspects: government, society, organization and individual. It includes: carrying out the functions of government, deepening the reform of medical and health system, reforming the medical security system, promoting the serious illness insurance, creating a loose social public opinion environment, strengthening the internal restriction of the scientific community in the internal organization and management of the hospital; To solve the conflict between medical service and scientific research, to create a loose internal environment of medical treatment and scientific research, individuals should face up to the occupational pressure and actively regulate it. The text is divided into four parts. The first part mainly introduces the background and the purpose and significance of the research. The second part is an overview of physician occupational stress. The third part takes the occupational stress status of doctors in several public third Class A hospitals in Anhui Province as a case study through empirical research to get the occupational stress status of doctors under the new medical reform and analyze the factors that lead to occupational stress. The fourth part is based on the conclusion of the third chapter, put forward countermeasures and suggestions to relieve the occupational stress.
