【摘要】：目的:調(diào)查研究精神疾病患者出院2年內(nèi)服藥依從性現(xiàn)狀及患者預(yù)后情況,探討影響精神疾病患者服藥依從性的可能因素,為提高精神疾病患者出院后服藥依從性提供理論依據(jù)。方法:選取衡陽市第二人民醫(yī)院2012年1月1日后出院滿2年的精神疾病患者430人為研究對(duì)象,采用自行設(shè)計(jì)的調(diào)查問卷,以現(xiàn)場(chǎng)問卷和電話隨訪的方式對(duì)其服藥依從性的現(xiàn)狀及影響因素進(jìn)行調(diào)查,發(fā)放問卷430份,有效問卷410份,問卷有效率為95.3%,應(yīng)用SPSS 18.0統(tǒng)計(jì)軟件建立數(shù)據(jù)文件并進(jìn)行統(tǒng)計(jì)分析,率或構(gòu)成比比較采用卡方檢驗(yàn),服藥依從性為等級(jí)結(jié)果的采用非參數(shù)檢驗(yàn)。檢驗(yàn)水準(zhǔn)=0.05。結(jié)果:1、精神疾病患者服藥依從率為83.9%,其中完全依從率為40.5%。完全依從患者的復(fù)發(fā)率遠(yuǎn)遠(yuǎn)低于部分依從和完全不依從的復(fù)發(fā)率,不同服藥依從性患者復(fù)發(fā)率差異具有統(tǒng)計(jì)學(xué)意義;2、精神疾病患者的服藥依從性與患者的年齡、文化程度、地區(qū)、婚姻狀況、家庭人均月收入有關(guān),年齡越大、文化程度高、家庭經(jīng)濟(jì)收入好的患者依從性越好;城市患者好于農(nóng)村患者,已婚有配偶的患者的服藥依從性優(yōu)于未婚單身;3、不同類型精神疾病患者服藥依從性差別有統(tǒng)計(jì)學(xué)意義,以器質(zhì)性精神障礙患者依從性最好,精神分裂癥患者依從性最差;4、藥物的副作用及藥物種類影響患者的服藥依從性,患者服藥出現(xiàn)副作用及藥物種類越多,依從性越差;獲得家庭及社會(huì)成員支持的精神疾病患者其服藥依從性較高。結(jié)論:精神疾病患者2年內(nèi)的服藥依從性欠佳,患者服藥依從性受個(gè)體因素、社會(huì)家庭因素、醫(yī)護(hù)因素的影響。
[Abstract]:Objective: to investigate the status of drug compliance and the prognosis of patients with mental illness within 2 years of discharge, and to explore the possible factors affecting the compliance of patients with mental illness.To provide a theoretical basis for improving the compliance of patients with mental illness after discharge from hospital.Methods: 430 patients with mental illness who were discharged from Hengyang second people's Hospital after 1 January 2012 were selected as the research objects, and a self-designed questionnaire was used.The status quo and influencing factors of drug compliance were investigated by field questionnaire and telephone follow-up. 430 questionnaires and 410 valid questionnaires were given out. The effective rate was 95.3. SPSS 18.0 statistical software was used to establish the data file and carry on statistical analysis.The rate or composition ratio was compared by chi-square test, and nonparametric test was used when compliance was the grade result.The inspection level is 0.05.Results the compliance rate of the patients with mental illness was 83.9%, and the complete compliance rate was 40.5%.The relapse rate of patients with complete compliance was far lower than that of partial compliance and complete non-compliance. The difference of relapse rate of patients with different drug compliance was statistically significant.Area, marital status, family per capita monthly income, the older the age, the higher the education level, the better the compliance of the patients with good family income; the better the urban patients are than the rural patients.The drug compliance of married patients with spouses was better than that of unmarried patients. There were statistically significant differences in drug compliance among patients with different types of mental disorders, and the patients with organic mental disorders had the best compliance.The compliance of schizophrenia patients is the worst. The side effects of drugs and the kinds of drugs affect the compliance of patients. The more side effects and the more kinds of drugs, the worse compliance.Patients with mental disorders supported by family and social members had higher compliance with medication.Conclusion: the compliance of patients with mental illness within 2 years is poor, and the compliance of patients is affected by individual factors, social and family factors and medical care factors.
