【摘要】：本論文研究由“十二五”國(guó)家科技支撐計(jì)劃課題(2013BAJ15B03)資助。隨著我國(guó)經(jīng)濟(jì)水平的不斷提升,近年來(lái)我國(guó)醫(yī)療衛(wèi)生事業(yè)發(fā)展迅速,同時(shí)人民就醫(yī)需求也不斷增大。在此背景下,為滿(mǎn)足市場(chǎng)日益增長(zhǎng)的醫(yī)療需求,提升自身服務(wù)水平與競(jìng)爭(zhēng)力,許多醫(yī)院紛紛開(kāi)始對(duì)院內(nèi)建筑設(shè)施進(jìn)行改建、擴(kuò)建,在醫(yī)療界掀起了一股建設(shè)高潮。住院樓作為醫(yī)院建筑設(shè)施的重要組成部分,近年來(lái)也得到了較快發(fā)展。住院病房是住院樓中數(shù)量最多、最為重要的組成單元,也是住院病人在院內(nèi)所待時(shí)間最長(zhǎng)的場(chǎng)所,因此其設(shè)計(jì)的好壞一方面影響著使用者的舒適度與滿(mǎn)意度,另一方面也決定了整個(gè)住院樓建設(shè)是否經(jīng)濟(jì)合理。雖然近年來(lái)建成的住院病房較過(guò)去已有了一定發(fā)展,但與國(guó)外先進(jìn)病房相比,差距還很大,這是由我國(guó)目前的國(guó)情所決定的。本文正是基于這樣的背景,希望探討適應(yīng)我國(guó)國(guó)情的住院病房設(shè)計(jì)策略。本文選取具有代表性、典型性的大型綜合醫(yī)院普通住院病房作為研究對(duì)象。首先對(duì)國(guó)內(nèi)住院病房建設(shè)發(fā)展相關(guān)背景進(jìn)行了分析,梳理總結(jié)了國(guó)內(nèi)外研究中關(guān)于病房設(shè)計(jì)的相關(guān)理論研究,并對(duì)其進(jìn)行了對(duì)比分析。在此基礎(chǔ)上,筆者對(duì)大型綜合醫(yī)院住院病房設(shè)計(jì)影響因素進(jìn)行了深入分析,主要包括了醫(yī)療衛(wèi)生服務(wù)現(xiàn)狀、住院病房使用者行為與需求以及室內(nèi)空間尺度限定三方面。在理論研究的基礎(chǔ)上,筆者進(jìn)行了實(shí)地調(diào)研。調(diào)研內(nèi)容主要分為兩大部分:一部分為對(duì)國(guó)內(nèi)多家大型綜合醫(yī)院住院病房現(xiàn)狀的調(diào)研,并從床位設(shè)置與使用、平面布局、相關(guān)尺度、室內(nèi)環(huán)境、配套設(shè)施五方面對(duì)調(diào)研成果進(jìn)行了梳理分析;另一部分內(nèi)容則是通過(guò)問(wèn)卷調(diào)查的方式,對(duì)住院病人、護(hù)理人員、陪護(hù)人員三類(lèi)住院病房使用人群進(jìn)行調(diào)查了解,收集他們關(guān)于住院病房的相關(guān)使用意見(jiàn)與需求。通過(guò)實(shí)地調(diào)研以及對(duì)調(diào)研結(jié)果的整理與分析,筆者總結(jié)了國(guó)內(nèi)大型綜合醫(yī)院住院病房現(xiàn)存的問(wèn)題與矛盾,主要包括以下方面:①床位使用緊張,使用者需求與實(shí)際情況存在矛盾;②平面布局多采用簡(jiǎn)單規(guī)整的形式,缺乏多樣性、適應(yīng)性;③相關(guān)尺度在實(shí)際使用過(guò)程中存在不足;④室內(nèi)環(huán)境單調(diào),在豐富性、舒適性、人性化等方面有所欠缺;⑤配套設(shè)施在滿(mǎn)足使用者需求方面存在矛盾與不足�；谶@些問(wèn)題與矛盾,筆者結(jié)合對(duì)住院病房設(shè)計(jì)影響因素的分析、對(duì)使用者需求的調(diào)研以及對(duì)國(guó)內(nèi)外優(yōu)秀設(shè)計(jì)案例的收集整理,從可行性、普適性層面提出了五點(diǎn)適應(yīng)我國(guó)國(guó)情的大型綜合醫(yī)院住院病房設(shè)計(jì)策略,包括“床位規(guī)劃與使用的合理性、平面布局的適應(yīng)性、空間尺度的適宜性、病房環(huán)境的人性化、室內(nèi)設(shè)計(jì)的家居化”,并針對(duì)這五點(diǎn)設(shè)計(jì)策略進(jìn)行了較為深入的分析與闡述。

With the development of economy in our country, the medical and health service is developing rapidly in recent years, and the demand for medical treatment is also increasing. In this context, in order to meet the increasing medical needs of the market, improve their service level and competitiveness, many hospitals have begun to rebuild and expand the hospital building facilities, which set off a construction climax in the medical field. As an important part of hospital construction facilities, hospital buildings have developed rapidly in recent years. The inpatient ward is the most important component unit in the hospital building, and it is also the place where the inpatients spend the longest time in the hospital, so the quality of its design affects the comfort and satisfaction of the users on the one hand. On the other hand, it also determines whether the construction of the entire hospital building is economical and reasonable. Although the inpatient wards built in recent years have developed to some extent compared with the advanced wards abroad there is still a big gap which is decided by the current situation of our country. Based on this background, this paper hopes to discuss the strategies of inpatient ward design adapted to China's national conditions. This article selects the representative, the typical large general hospital general inpatient ward as the research object. First of all, this paper analyzes the related background of the development of inpatient ward construction in China, summarizes the relevant theoretical research on ward design at home and abroad, and makes a comparative analysis on it. On this basis, the author has carried on the thorough analysis to the large-scale general hospital inpatient ward design influence factor, has mainly included the medical and health service present situation, the inpatient ward user behavior and the demand as well as the indoor spatial scale limit three aspects. On the basis of theoretical research, the author carried out field investigation. The contents of the investigation are divided into two parts: one is the investigation of the present situation of the inpatient wards in many large general hospitals in China, and the research includes bed setup and use, plane layout, relevant scale, indoor environment, Supporting facilities in five aspects of the survey results were combed and analyzed; The other part is through the way of questionnaire, the inpatients, nursing staff, attendants of three types of hospital ward users to investigate and understand, collect their views and needs on the use of hospital wards. Through the field investigation and the analysis of the investigation results, the author summarizes the existing problems and contradictions in the inpatient wards of large general hospitals in China, mainly including the following aspects: 1. There is a contradiction between the needs of users and the actual situation; (2) the plane layout is mostly in the form of simple and regular, lacking diversity and adaptability; (3) the relative scale is insufficient in the actual use process; (4) the indoor environment is monotonous, lacking in richness, comfort, humanization and so on. There are contradictions and deficiencies in supporting facilities to meet the needs of users. Based on these problems and contradictions, the author combines the analysis of the factors affecting the design of inpatient wards, the investigation of user needs and the collection and arrangement of excellent design cases at home and abroad. In the aspect of universality, the author puts forward five strategies for the design of hospital ward in large general hospitals, including the rationality of bed planning and use, the adaptability of plane layout, the suitability of spatial scale, and the humanization of ward environment.
