防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡結構研究

【摘要】:本文屬國家自然科學基金面上項目“防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡體系構建”(51078104)的重要組成部分。所謂突發(fā)性傳染病,指的是在短期內發(fā)生、有較大波及范圍,出現(xiàn)大量的病人或死亡病例,發(fā)病率遠超過常年發(fā)病率水平的一種傳染病疫情;因其意外性、高危害性、廣泛影響力等特點,已經成為了目前人類所面臨的醫(yī)學與社會的雙重難題;考驗著一個國家、地區(qū)、城市的公共衛(wèi)生服務體系,檢驗著醫(yī)療建筑、醫(yī)療設施的發(fā)展均衡性。本研究從醫(yī)療建筑網(wǎng)絡化的角度出發(fā),對防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡體系下的醫(yī)療建筑規(guī)劃和建筑設計進行研究,目的在于提出理論性的指導對策,并針對性的建構出防控突發(fā)性傳染病的醫(yī)療建筑體系的具體操作模式。同時促進國家“堅持公益性、調動積極性”的新醫(yī)改政策在疾病防控領域的具體應用,在新醫(yī)改政策的框架下完善當前的醫(yī)療建筑體系,完善醫(yī)療建筑設計方法。其最終的社會意義是探討“以人為本”的從建筑領域出發(fā)解決社會問題的新途徑。本文的具體研究指向防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡體系,以體系中最為關鍵的網(wǎng)絡結構為關注點,輔以系統(tǒng)論、協(xié)同論、區(qū)域科學理論等方法,創(chuàng)新性的提出了真實空間網(wǎng)絡、虛擬空間網(wǎng)絡、中介空間網(wǎng)絡三位一體的防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡架構,并用網(wǎng)絡配置計劃對組成網(wǎng)絡的醫(yī)療建筑要素進行具體的規(guī)劃與設計指引,用網(wǎng)絡運行機制對網(wǎng)絡架構中錯綜復雜的內部關系進行總結。研究循著從點到面的思路,從“中觀”層面構成網(wǎng)絡的醫(yī)療建筑要素的研究介入,向上拓展到城市規(guī)劃層面的醫(yī)療建筑網(wǎng)絡體系整合,向下則具象到保證醫(yī)療建筑網(wǎng)絡體系順暢運行的相關技術支撐,全方位的對醫(yī)療建筑網(wǎng)絡體系進行了梳理。在防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡要素的研究中,選取了整個網(wǎng)絡體系中最為關鍵的四類醫(yī)療建筑“結點”:疾病預防控制中心、傳染病?漆t(yī)院、綜合醫(yī)院傳染病防控空間以及基層傳染病防控醫(yī)療機構,通過對近年來大量相關案例的分析,對這四類醫(yī)療建筑在網(wǎng)絡中的職能、發(fā)展趨勢以及設計要點進行梳理與闡述。在防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡整合模式的探索中,從政策及策略層面提出以防控突發(fā)性傳染病的城市醫(yī)療建筑規(guī)劃專篇為目標的垂直整合模式,主要體現(xiàn)為規(guī)劃功能、規(guī)劃技術與規(guī)劃程序的整合。進而在具體方法及手段層面描述了以“聯(lián)動”為特質的醫(yī)療建筑水平整合模式,并主要以哈爾濱市防控突發(fā)性傳染病的城市醫(yī)療建筑規(guī)劃專篇為例說明了這一特質在真實空間網(wǎng)絡、虛擬空間網(wǎng)絡、中介空間網(wǎng)絡三個不同網(wǎng)絡架構上的體現(xiàn)。在防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡技術支撐的闡釋中,以突發(fā)性傳染病疫情的全過程為線索,首先從廣泛分析、精確控制和優(yōu)化救治三個角度探索了技術支撐平臺的構建方式;其次又從一體化預防、協(xié)同化控制和應急化救治三方面來明晰具體的技術支撐策略;最后還從相應的醫(yī)療技術方面,探索了其對于網(wǎng)絡體系的輔助支持。
[Abstract]:This is the National Natural Science Foundation Project "medical building network system for the prevention and control of emerging infectious diseases in the construction of" (51078104). This study from the medical building network perspective, research on medical construction planning and construction design of medical building network system for the prevention and control of emerging infectious diseases under the purpose of providing theoretical guidance Countermeasures and specific operation mode of prevention and control of emerging infectious diseases in the medical building system of construction. In the study of medical building network elements of prevention of sudden infectious diseases in the selected four kinds of medical building "key nodes" for the entire network in the system of center for Disease Control and prevention, infectious disease hospital, space disease prevention and control of infectious disease prevention and control and grassroots medical institutions of infectious hospital, through the Analysis in recent years a large number of cases, for the four types of medical building functions in the network, combing with the described development trend and design points. To explore the prevention and control of emerging infectious diseases in the medical building network integration model, from the policy and strategy of city health planning level put forward to prevention and control of emerging infectious diseases in the college for vertical integration pattern, mainly embodies the planning function, integration planning and planning program.
