補(bǔ)糖對(duì)EAMD者生殖激素與相關(guān)能量代謝激素的影響研究

【摘要】:研究目的:本研究立足于可利用能量假說,采用補(bǔ)糖(不干預(yù)受試者運(yùn)動(dòng)訓(xùn)練與生活習(xí)慣)的方式對(duì)運(yùn)動(dòng)性月經(jīng)周期紊亂(EAMD)進(jìn)行干預(yù),分析補(bǔ)糖對(duì)機(jī)體能量狀況的影響以及機(jī)體能量狀況的改變對(duì)生殖激素、相關(guān)應(yīng)激激素和能量代謝激素的作用,并嘗試探討其相關(guān)性和影響機(jī)制,以觀察補(bǔ)糖對(duì)EAMD的防治效果。研究方法:招募14名EAMD受試者隨機(jī)分為補(bǔ)糖組與對(duì)照組,每組各7名,補(bǔ)糖組進(jìn)行連續(xù)三個(gè)月經(jīng)周期的補(bǔ)糖干預(yù)。在試驗(yàn)前后均對(duì)所有受試者進(jìn)行身體成份與能量平衡狀況的評(píng)估。在試驗(yàn)開始前一個(gè)月經(jīng)周期和試驗(yàn)第三個(gè)月經(jīng)周期采集所有受試者卵泡晚期和黃體中期的靜脈血液,以檢測促性腺激素釋放激素(Gn RH)、促卵泡激素(FSH)、黃體生成素(LH)、雌二醇(E_2)、孕酮(P)、睪酮(T)、胃饑餓素(Ghrelin)、瘦素(Leptin)、皮質(zhì)醇(C)、總膽固醇(TC)、甘油三酯(TG)、高密度脂蛋白膽固醇(HDL-C)、低密度脂蛋白膽固醇(LDL-C)。通過比較試驗(yàn)前后受試者能量平衡狀況和相關(guān)血液指標(biāo)的變化,擬探討補(bǔ)糖干預(yù)對(duì)EAMD的作用效果。研究結(jié)果:1.試驗(yàn)前所有受試者均為能量負(fù)平衡(補(bǔ)糖組:-468.9±367.1 kcal;對(duì)照組:-445.6±304.8 kcal),試驗(yàn)后補(bǔ)糖組能量平衡由負(fù)轉(zhuǎn)正(132.5±107.8 kcal),組間、組內(nèi)比較有統(tǒng)計(jì)學(xué)意義(p0.05),對(duì)照組能量狀況無明顯變化。2.試驗(yàn)后補(bǔ)糖組卵泡期E_2、黃體期P水平上升(E_2:52.4±6.5~88.5±41.1;P:6.7±7.2~7.7±5.1)與對(duì)照組比較有統(tǒng)計(jì)學(xué)意義(p0.05);除E_2、P外,Gn RH、FSH、LH、T、Ghrelin、Leptin、C、TC、TG、HDL-C、LDL-C未發(fā)現(xiàn)明顯變化。3.相關(guān)性分析顯示GnRH和TC、LDL-C具有正相關(guān)性,LH、T均與TC、HDL-C具有正相關(guān)性,FSH、P均與HDL-C具有正相關(guān)性。研究結(jié)論:1.補(bǔ)糖使EAMD者的能量負(fù)平衡轉(zhuǎn)變?yōu)檎胶?說明補(bǔ)糖對(duì)機(jī)體能量缺乏有良好的干預(yù)效果,能夠逆轉(zhuǎn)能量負(fù)平衡情況。2.補(bǔ)糖對(duì)相關(guān)生殖激素E_2、P的分泌有一定的促進(jìn)作用,使卵泡期E_2和黃體期P分泌上升,對(duì)EAMD者恢復(fù)正常的月經(jīng)周期有一定幫助。3.本研究相關(guān)生殖激素與膽固醇之間具有一定的相關(guān)關(guān)系:其中,GnRH和TC、LDL-C具有中度正相關(guān)性,LH、T均與TC、HDL-C具有正相關(guān)性,FSH、P均與HDL-C具有正相關(guān)性,進(jìn)一步證實(shí)膽固醇與HPO軸各級(jí)組織所分泌的生殖激素的關(guān)系,及其對(duì)正常月經(jīng)周期的影響作用。
[Abstract]:Objective: this study was based on the hypothesis of available energy and intervened in exercise menstrual cycle disorder (EAMDD) by using sugar supplement (not interfering with exercise training and living habits of subjects). The effects of glucose supplementation on energy status and the effects of energy status on reproductive hormones, related stress hormones and energy metabolic hormones were analyzed, and the correlation and mechanism were discussed in order to observe the effect of glucose supplementation on the prevention and treatment of EAMD. Methods: fourteen subjects of EAMD were randomly divided into two groups: the glucose supplement group (n = 7) and the control group (n = 7). All subjects were evaluated for body composition and energy balance before and after the trial. Venous blood was collected from all subjects during the late follicular and middle luteal stages of the first menstrual cycle and the third menstrual cycle of the trial. To detect gonadotropin releasing hormone (Gn RHH), follicle stimulating hormone (FSH), luteinizing hormone (LHN), estradiol (E), progesterone (P), testosterone (T), ghrelin, leptin (Leptinn), cortisol (C), total cholesterol (TC), triglyceride (TGN), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (HDL-C). Degree lipoprotein cholesterol (LDL-C). The effects of glucose supplementation on EAMD were studied by comparing the changes of energy balance and related blood indexes before and after the experiment. The result of the study was: 1. Before the experiment, all the subjects were in negative energy balance (w -468.9 鹵367.1 kcala in the glucose supplement group and in the control group: -445.6 鹵304.8 kcal.After the experiment, the energy balance in the glucose supplement group turned from negative to positive (132.5 鹵107.8 kcal-1). There was significant difference between the two groups (p 0.05), but there was no significant change in the energy status of the control group. Compared with the control group, the levels of E _ 2: 52.4 鹵6.5 ~ 88.5 鹵41.1% P: 6.7 鹵7.2 鹵7.7 鹵5.1) in the glucose supplement group were significantly higher than those in the control group (P < 0.05). Correlation analysis showed that there was a positive correlation between GnRH and TCU LDL-C. Both of them were positively correlated with TCU HDL-C. Both FSHP and HDL-C were positively correlated. Conclusion: 1. The negative energy balance of EAMD patients was changed into positive equilibrium by sugar supplementation, which indicated that glucose supplementation had a good intervention effect on energy deficiency, and could reverse the negative energy balance. 2. Glucose supplementation can promote the secretion of E2P, and increase the secretion of E _ 2 and P in follicular phase, which is helpful to the recovery of normal menstrual cycle in patients with EAMD. There was a positive correlation between GnRH and TCU LDL-C in this study. There was a positive correlation between GnRH and TCU LDL-C and HDL-C. FSHP was positively correlated with HDL-C. To further confirm the relationship between cholesterol and reproductive hormones secreted by HPO axis and its effect on normal menstrual cycle.
