燒傷患者分離金黃色葡萄球菌耐藥性與耐藥基因研究

【摘要】:目的了解燒傷病房患者分離金黃色葡萄球菌的耐藥性及耐藥基因分布,探討燒傷患者易感染金黃色葡萄球菌的原因。方法收集2013年7-12月醫(yī)院燒傷病房患者的金黃色葡萄球菌,菌種鑒定采用法國生物梅里埃公司VITEK-2Compact細(xì)菌鑒定儀,并進(jìn)一步作spa基因PCR檢測(cè),藥敏試驗(yàn)采用法國生物梅里埃公司VITEK-2Compact系統(tǒng)進(jìn)行,通過PCR方法分析其耐藥基因mecA、aac6′/aph2″、aph3′-Ⅲ、ant4′、ermA、ermB、ermC、mphC、msrA、tetM、tetK、qacA、bsa的表達(dá)。結(jié)果共收集金黃色葡萄球菌20株,其中19株來源于創(chuàng)面、1株來源于留置導(dǎo)管;金黃色葡萄球菌對(duì)青霉素G的耐藥率為100.0%,對(duì)萬古霉素、替考拉寧、利福平等耐藥率均為0;mecA和bsa基因檢測(cè)陽性率均為55.0%,aac6′/aph2″、aph 3′-Ⅲ基因檢測(cè)陽性率分別為50.0%和45.0%。結(jié)論燒傷患者分離的20株金黃色葡萄球菌中mecA及bsa基因檢出率較高,是燒傷患者易于感染金黃色葡萄球菌的原因,耐藥基因檢測(cè)提示其對(duì)季胺類消毒劑消毒皮膚效果可能欠佳。
