疲勞的現(xiàn)代研究

【摘要】:作為21世紀(jì)危害人類健康的重要因素,疲勞正逐漸引起人們的關(guān)注.通過(guò)對(duì)Pub Med數(shù)據(jù)庫(kù)進(jìn)行檢索,共檢索文獻(xiàn)4652篇,系統(tǒng)地圍繞疲勞的定義、中醫(yī)藥學(xué)對(duì)疲勞的認(rèn)識(shí)、疲勞的臨床診療、疲勞的生物學(xué)機(jī)制和動(dòng)物模型的研制及疲勞研究的展望方面展開述評(píng),以對(duì)幾十年來(lái)疲勞的相關(guān)研究進(jìn)行回顧和展望.目前,有關(guān)疲勞的機(jī)制研究更多的關(guān)注中樞神經(jīng)系統(tǒng)和內(nèi)分泌系統(tǒng).近10年來(lái),隨著神經(jīng)影像學(xué)技術(shù)的發(fā)展,依靠腦功能成像技術(shù)等新技術(shù)的臨床研究數(shù)量增多.然而疲勞的發(fā)病機(jī)制不僅是中樞內(nèi)分泌系統(tǒng)失調(diào)的單純作用結(jié)果,其涉及神經(jīng)-內(nèi)分泌-免疫網(wǎng)絡(luò)的功能協(xié)調(diào),并且其形成是從初始感受器經(jīng)過(guò)脊髓、中樞神經(jīng)、特定功能腦區(qū)、呼吸以及循環(huán)系統(tǒng),最終到達(dá)終端線粒體的一個(gè)周而復(fù)始的循環(huán)鏈?zhǔn)椒磻?yīng),每一個(gè)環(huán)節(jié)都可以影響到疲勞的發(fā)生和嚴(yán)重程度,臨床也可針對(duì)不同的反應(yīng)位點(diǎn)和機(jī)制開展干預(yù)研究.總而言之,疲勞的研究已經(jīng)從整體、籠統(tǒng)、單純的軀體功能等方面,發(fā)展到了局部、具體、涉及中樞-神經(jīng)-內(nèi)分泌系統(tǒng)以及免疫系統(tǒng)等多方面的實(shí)驗(yàn)和臨床探索,基因?qū)W和細(xì)胞器是今后研究的熱點(diǎn)和方向.
[Abstract]:As an important factor that endangers human health in the 21 st century, fatigue is gradually attracting people's attention. By searching the Pub Med database, 4652 papers were searched, including the definition of fatigue, the understanding of fatigue in TCM, and the clinical diagnosis and treatment of fatigue. The biological mechanism of fatigue, the development of animal models and the prospect of fatigue research are reviewed in this paper. At present, more attention has been paid to the central nervous system and endocrine system. In recent 10 years, with the development of neuroimaging techniques, the number of clinical studies relying on new techniques, such as brain functional imaging, has increased. However, the pathogenesis of fatigue is not only the result of the disorder of central endocrine system, but also involves the coordination of the function of neuroendocrine immune network, and its formation is from the initial receptor through the spinal cord, central nervous system, Specific functional brain regions, breathing and circulatory systems, and eventually reaching the terminal mitochondria in a cycle of circulatory chain reactions, each of which can affect the occurrence and severity of fatigue. Clinical intervention can also be carried out for different response sites and mechanisms. In a word, the study of fatigue has developed from the whole, the general, the simple body function and so on, to the local, the concrete, involves the center-neuroendocrine system and the immune system and so on many aspects experiment and the clinical exploration, Genetics and organelles are the focus and direction of future research.
