【摘要】：目的了解膳食纖維對(duì)術(shù)后結(jié)腸癌患者的臨床治療效果,探究其對(duì)結(jié)腸癌患者術(shù)后免疫功能及炎性反應(yīng)的影響。方法采用前瞻性隨機(jī)對(duì)照研究,選取病理診斷為結(jié)腸癌并且行結(jié)腸癌根治術(shù)的患者60例,其中膳食纖維生態(tài)腸內(nèi)營(yíng)養(yǎng)組(試驗(yàn)組)30例,生態(tài)腸內(nèi)營(yíng)養(yǎng)組(對(duì)照組)30例。分別于術(shù)前第1天、術(shù)后第2天、術(shù)后第7天清晨空腹抽取外周靜脈血,檢測(cè)血T淋巴細(xì)胞亞群(CD3+,CD4+,CD8+)和血清免疫球蛋白(IgA,IgG,IgM)等免疫指標(biāo),腫瘤壞死因子-α(TNF-α)、白細(xì)胞介素-6(IL-6)、降鈣素原(PCT)等炎癥反應(yīng)指標(biāo)。結(jié)果各時(shí)間點(diǎn)兩組間的CD3+、CD8+、CD4+/CD8+、IgM水平差異無統(tǒng)計(jì)學(xué)意義(P0.05),而在術(shù)后第7天,試驗(yàn)組CD4+、IgA、IgG水平高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);試驗(yàn)組TNF-α、IL-6水平低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論在結(jié)腸癌患者中,術(shù)后早期給予膳食纖維可提高患者機(jī)體免疫功能、減輕炎性反應(yīng)。
[Abstract]:Objective to investigate the effect of dietary fiber on postoperative immune function and inflammatory response in patients with colon cancer. Methods A prospective randomized controlled study was conducted in 60 patients with colon cancer diagnosed by pathology and underwent radical resection of colon cancer, including 30 cases in the experimental group and 30 cases in the ecological enteral nutrition group and 30 cases in the control group. Peripheral venous blood was collected on the first day before operation, the second day after operation and the morning of 7 days after operation. The T lymphocyte subsets (CD3 T lymphocyte subsets) and serum immunoglobulin (IgA) immunoglobulin (IgG CD3) were detected in the first day of operation, the second day after operation and the morning of 7 days after operation. Tumor necrosis factor-偽 (TNF- 偽), interleukin-6 (IL-6), procalcitonin (PCT), and other inflammatory markers. Results there was no significant difference between the two groups at each time point (P0.05), but on the 7th day after operation, the level of CD4 IgA CD3 in the experimental group was higher than that in the control group (P0.05), and the level of TNF- 偽 IL-6 in the experimental group was lower than that in the control group (P0.05). Conclusion in colon cancer patients, early postoperative dietary fiber can improve the immune function of patients and alleviate inflammatory reaction.
