【摘要】：目的:通過對(duì)照磁共振PDWI抑脂序列與增強(qiáng)T1WI抑脂序列對(duì)肛瘺內(nèi)口診斷價(jià)值的差異,探索其對(duì)手術(shù)方案選擇的影響,以提高肛瘺的治療效果。方法:68例經(jīng)手術(shù)病理證實(shí)為肛瘺的患者,術(shù)前行MRI掃描。以手術(shù)結(jié)果為標(biāo)準(zhǔn),比較分析橫斷位PDWI抑脂序列與增強(qiáng)T1WI抑脂序列對(duì)肛瘺內(nèi)口的顯示。結(jié)果:以手術(shù)結(jié)果為標(biāo)準(zhǔn),PDWI抑脂序列與增強(qiáng)T1WI抑脂序列肛瘺內(nèi)口定位診斷正確率分別為94.11%和95.58%,差別無統(tǒng)計(jì)學(xué)意義(P0.05)。但增強(qiáng)T1WI抑脂序列對(duì)肛瘺內(nèi)口顯示清晰度的評(píng)分明顯高于PDWI抑脂序列,差別有統(tǒng)計(jì)學(xué)意義(P0.001)。結(jié)論:PDWI抑脂序列可以準(zhǔn)確定位肛瘺內(nèi)口,對(duì)肛瘺的術(shù)式的選擇及提高治療效果具有重要價(jià)值,但其對(duì)肛瘺內(nèi)口顯示的清晰度不如增強(qiáng)T1WI抑脂序列,不過后者需要注射MR對(duì)比劑,增加了患者經(jīng)濟(jì)負(fù)擔(dān)和副作用風(fēng)險(xiǎn)。
[Abstract]:Objective: to explore the effect of contrast magnetic resonance (PDWI) lipid-suppression sequence and enhanced T1WI sequence on the diagnosis of anal fistula in order to improve the therapeutic effect of anal fistula. Methods 68 cases of anal fistula confirmed by operation and pathology were examined by MRI before operation. According to the results of operation, we compared and analyzed the display of transected PDWI and enhanced T1WI on the internal orifice of anal fistula. Results: the diagnostic accuracy of anal fistula was 94.11% on PDWI and 95.58% on enhanced T1WI. The difference was not statistically significant (P 0.05). But enhanced T1WI fat suppression sequence was significantly higher than that of PDWI in evaluating the articulation of anal fistula (P 0.001). Conclusion the weight PDWI sequence can accurately locate the internal orifice of anal fistula, which is of great value to the choice of operation method and improve the therapeutic effect of anal fistula. However, the clarity of the internal mouth of anal fistula is not as clear as that of enhanced T1WI sequence, but the latter needs to be injected with Mr contrast agent. Increased patient financial burden and side effects risk.
