【摘要】：目的:探討高頻彩超對炎性乳癌的診斷價值。方法 :回顧性分析經(jīng)手術(shù)病理證實的16例炎性乳癌的臨床資料,對其臨床癥狀、超聲聲像圖特征及病理類型進(jìn)行總結(jié)分析。結(jié)果:16例炎性乳癌臨床表現(xiàn)為患側(cè)乳腺丹毒樣紅腫,觸診均可捫及不規(guī)則腫塊。1例誤診為急性乳腺炎,經(jīng)治療后無明顯改善,后經(jīng)穿刺病理學(xué)活檢診斷為炎性乳腺癌。15例經(jīng)高頻彩超提示診斷,聲像圖表現(xiàn)為:病變區(qū)域皮膚及皮下組織增厚水腫,淋巴管擴(kuò)張,呈"鵝卵石"樣改變,另可見不規(guī)則腫塊,伴或不伴鈣化,引流區(qū)淋巴結(jié)腫大。所有患者病理結(jié)果均伴有淋巴管癌栓。結(jié)論:對有急性乳腺炎表現(xiàn)的患者,結(jié)合高頻彩超的聲像圖表現(xiàn)綜合分析是提高炎性乳癌術(shù)前診斷的關(guān)鍵。
[Abstract]:Objective: to investigate the diagnostic value of high frequency color Doppler ultrasound in inflammatory breast cancer. Methods: the clinical data of 16 cases of inflammatory breast cancer confirmed by surgery and pathology were analyzed retrospectively and the clinical symptoms were analyzed. Results the clinical manifestations of 16 cases of inflammatory breast cancer were erysipelin redness, palpation and irregular mass were misdiagnosed as acute mastitis. After biopsy, 15 cases of inflammatory breast cancer were diagnosed by high frequency color ultrasound. The sonogram showed thickening and edema of skin and subcutaneous tissue, dilation of lymphatic vessels, cobblestone appearance, and irregular mass. With or without calcification, the lymph nodes in the drainage area were enlarged. All the patients were accompanied by lymphangiocarcinoma thrombus. Conclusion: for patients with acute mastitis, It is the key to improve the preoperative diagnosis of inflammatory breast cancer by analyzing the sonographic features of high frequency color Doppler sonography.
