血清腫瘤壞死因子相關(guān)凋亡誘導配體與C-反應蛋白對兒童急性病毒感染的診斷價值

【摘要】:目的探討急性細菌和病毒感染患兒血清腫瘤壞死因子相關(guān)凋亡誘導配體(TRAIL)的水平變化及臨床價值。方法選取2013年1月-2016年4月醫(yī)院住院的急性病毒感染患兒51例、急性細菌感染患兒52例為研究對象,對照組選取同期醫(yī)院體檢未發(fā)現(xiàn)感染疾病者51例,采用酶聯(lián)免疫吸附法檢測血漿TRAIL水平,西門子BNⅡ特定蛋白分析儀測定血清C-反應蛋白(CRP)。結(jié)果不同組別中TRAIL和CRP水平明顯不同(P0.05),病毒感染組TRAIL水平明顯高于細菌感染組和正常對照組(P0.05),病毒感染組CRP水平明顯低于細菌感染組,但高于正常對照組(P0.05);受試者工作特征曲線分析,TRAIL和CRP在區(qū)分急性病毒感染患者和正常對照者的曲線下面積分別為0.759和0.744,以147.35pg/mL和5.415g/L為診斷界值點時,靈敏度分別為76.0%、70.0%,特異度分別為76.5%、88.2%;TRAIL與CRP聯(lián)合檢測,靈敏度和特異度分別提高到78.0%和92.2%。結(jié)論高水平血清TRAIL可能對患兒發(fā)生急性病毒感染有一定的預測價值,TRAIL和CRP聯(lián)合檢測提高急性病毒感染診斷效能。
[Abstract]:Objective to investigate the changes and clinical value of serum tumor necrosis factor-related apoptosis-inducing ligand trail in children with acute bacterial and viral infection. Methods 51 hospitalized children with acute viral infection from January 2013 to April 2016 were selected. 52 children with acute bacterial infection were studied. 51 patients with no infection were selected as control group. Plasma TRAIL levels were detected by enzyme linked immunosorbent assay (Elisa). Results the levels of TRAIL and CRP were significantly different in different groups. The TRAIL level in viral infection group was significantly higher than that in bacterial infection group and normal control group (P 0.05). The CRP water in viral infection group was significantly higher than that in bacterial infection group and normal control group. The level of ping was significantly lower than that of bacterial infection group. The area under the curve of trail and CRP were 0.759 and 0.744, 147.35pg / mL and 5.415g / L, respectively. The sensitivity was 70.0 and the specificity was 76.588. 2%, respectively. Trail was detected with CRP, and the sensitivity was 76. 0%, and the specificity was 76. 5% and 88. 2%, respectively. Conclusion the high level of serum TRAIL may be a predictor of acute viral infection in children. The combined detection of trail and CRP can improve the diagnostic efficacy of acute viral infection.
