我國(guó)的財(cái)政分權(quán)對(duì)公共衛(wèi)生投入的影響研究

【摘要】:對(duì)于財(cái)政分權(quán)對(duì)公共衛(wèi)生投入的影響,前人進(jìn)行了大量的研究,本文在對(duì)前人研究的成果進(jìn)行梳理后,發(fā)現(xiàn)由于取樣、取樣的時(shí)間、公共衛(wèi)生投入角度的不同,得到的結(jié)果也不相同,這是促使本文在全國(guó)范圍內(nèi)研究2000年以來(lái)財(cái)政分權(quán)對(duì)公共衛(wèi)生投入影響的重要原因。 本文以地方政府支出自主度來(lái)衡量財(cái)政分權(quán)的,先著重分析了財(cái)政分權(quán)的現(xiàn)狀以及東中西部的情況,再?gòu)墓残l(wèi)生的投入量、公共衛(wèi)生的投入結(jié)構(gòu)、公共衛(wèi)生投入的努力程度、公共衛(wèi)生的投入效率四個(gè)方面對(duì)公共衛(wèi)生投入現(xiàn)狀進(jìn)行了詳細(xì)分析,最后本文在研究財(cái)政分權(quán)對(duì)公共衛(wèi)生的影響時(shí),打破了以往學(xué)者們只針對(duì)一項(xiàng)指標(biāo)研究的格局,,從三個(gè)角度來(lái)研究了財(cái)政分權(quán)對(duì)公共衛(wèi)生投入的影響,分別是財(cái)政分權(quán)對(duì)公共衛(wèi)生的總量水平(人均政府衛(wèi)生支出)的影響,財(cái)政分權(quán)對(duì)公共衛(wèi)生投入的努力程度的影響、財(cái)政分權(quán)對(duì)公共衛(wèi)生投入效率的影響,并對(duì)他們進(jìn)行了理論分析與實(shí)證研究。 通過(guò)實(shí)證發(fā)現(xiàn),我國(guó)的財(cái)政分權(quán)對(duì)公共衛(wèi)生的總量水平無(wú)論是東中西還是全國(guó),都是呈現(xiàn)正效應(yīng)的;財(cái)政分權(quán)對(duì)公共衛(wèi)生投入的努力程度的影響則要分情況,全國(guó)、東、中部是呈現(xiàn)正效應(yīng),而西部則呈現(xiàn)負(fù)效應(yīng);財(cái)政分權(quán)對(duì)公共衛(wèi)生投入的效率全部呈現(xiàn)負(fù)效應(yīng)。文章的最后針對(duì)出現(xiàn)負(fù)效應(yīng)結(jié)論的公共衛(wèi)生投入的努力程度、公共衛(wèi)生投入的效率,從合理劃分支出責(zé)任、建立科學(xué)的地方政府績(jī)效評(píng)價(jià)體系、完善轉(zhuǎn)移支付制度、健全全民偏好機(jī)制、調(diào)整、精簡(jiǎn)政府行政機(jī)構(gòu)以及超出財(cái)政分權(quán)體制范圍的其他建議的幾個(gè)方面,提出了相關(guān)的政策建議。
[Abstract]:As for the influence of fiscal decentralization on public health investment, the predecessors have carried out a large number of studies. After combing the results of previous studies, this paper finds that because of sampling, sampling time, public health investment angle is different. The results obtained are also different, which is an important reason for this paper to study the influence of fiscal decentralization on public health investment in the whole country since 2000. This paper measures fiscal decentralization by the degree of autonomy of local government expenditure, first analyzes the current situation of fiscal decentralization and the situation of east, west and west, and then from the input amount of public health, the structure of public health investment, the degree of effort of public health investment. Four aspects of public health investment efficiency are analyzed in detail. Finally, this paper breaks the pattern that scholars only focus on one index when studying the influence of fiscal decentralization on public health. This paper studies the influence of fiscal decentralization on public health input from three angles, namely, the influence of fiscal decentralization on the total level of public health (per capita government health expenditure), and the influence of fiscal decentralization on the efforts of public health investment. The influence of fiscal decentralization on the efficiency of public health investment is analyzed theoretically and empirically. It is found that the fiscal decentralization in China has positive effects on the total level of public health, whether eastern, western or national, and that the influence of fiscal decentralization on the efforts of public health investment is divided into two categories, the whole country, the east, and the whole country. The central part has positive effect, while the west has negative effect, and the fiscal decentralization has negative effect on the efficiency of public health investment. At the end of the article, aiming at the degree of public health investment and the efficiency of public health investment, the author establishes a scientific evaluation system of local government performance, and consummates the transfer payment system from the rational division of expenditure responsibility, the establishment of a scientific evaluation system of local government performance, and the improvement of the transfer payment system. This paper puts forward some policy suggestions on perfecting the popular preference mechanism, adjusting, streamlining the government administrative organs and other suggestions beyond the scope of the fiscal decentralization system.
