【摘要】：研究目的 通過(guò)研究制定一套科學(xué)、可行、適合我國(guó)縣級(jí)婦幼保健機(jī)構(gòu)績(jī)效評(píng)價(jià)的指標(biāo)體系,評(píng)價(jià)指標(biāo)體系體現(xiàn)婦幼保健機(jī)構(gòu)的功能定位和發(fā)展目標(biāo),兼顧對(duì)婦幼保健機(jī)構(gòu)的個(gè)體保健、醫(yī)療和公共衛(wèi)生服務(wù)的績(jī)效評(píng)價(jià)。 研究?jī)?nèi)容與方法：本研究分為二部分。 第一部分縣級(jí)婦幼保健機(jī)構(gòu)績(jī)效評(píng)價(jià)指標(biāo)體系的建立 (1)選擇適宜的評(píng)價(jià)理論,建立評(píng)價(jià)框架 首先查閱大量我國(guó)婦幼保健發(fā)展相關(guān)的方針、政策等規(guī)范性文件,以及有關(guān)婦幼保健機(jī)構(gòu)發(fā)展和評(píng)價(jià)的相關(guān)文獻(xiàn),同時(shí)復(fù)習(xí)有關(guān)績(jī)效評(píng)價(jià)的最新理論和方法,在此基礎(chǔ)上建立績(jī)效考核評(píng)價(jià)框架, (2)收集相關(guān)評(píng)價(jià)指標(biāo) 通過(guò)文獻(xiàn)回顧,初步選擇了6個(gè)一級(jí)指標(biāo),17個(gè)二級(jí)指標(biāo)和120個(gè)三級(jí)指標(biāo)。經(jīng)過(guò)1次現(xiàn)場(chǎng)座談,2輪專(zhuān)家討論和論證,形成了縣級(jí)婦幼保健機(jī)構(gòu)績(jī)效評(píng)價(jià)的一套備選指標(biāo)體系,該體系含有6個(gè)一級(jí)指標(biāo),15個(gè)二級(jí)指標(biāo)和84個(gè)三級(jí)指標(biāo)。 (3)指標(biāo)篩選 根據(jù)備選指標(biāo)體系設(shè)計(jì)Delphi專(zhuān)家咨詢(xún)調(diào)查表對(duì)評(píng)價(jià)框架的合理性和備選指標(biāo)的重要性、可行性進(jìn)行主觀(guān)評(píng)分,經(jīng)過(guò)兩輪專(zhuān)家咨詢(xún),結(jié)合專(zhuān)家權(quán)威程度系數(shù)對(duì)兩輪專(zhuān)家評(píng)分結(jié)果進(jìn)行統(tǒng)計(jì)分析,采用界值法進(jìn)行指標(biāo)的篩選。每一次專(zhuān)家咨詢(xún)后都召開(kāi)專(zhuān)家論證會(huì),在定量分析的基礎(chǔ)上,綜合回收問(wèn)卷中專(zhuān)家所提出的意見(jiàn)和專(zhuān)家論證會(huì)上專(zhuān)家意見(jiàn),增加、刪除或更改指標(biāo)的歸類(lèi)和名稱(chēng)。通過(guò)兩輪德?tīng)柗茖?zhuān)家咨詢(xún)和兩次專(zhuān)家論證最終形成了含有4個(gè)一級(jí)指標(biāo),10個(gè)二級(jí)指標(biāo)和42個(gè)三級(jí)指標(biāo)的縣級(jí)婦幼保健機(jī)構(gòu)績(jī)效評(píng)價(jià)指標(biāo)體系。 (4)確定指標(biāo)權(quán)重：對(duì)最終形成的指標(biāo)體系進(jìn)行第三次Delphi專(zhuān)家咨詢(xún),采用百分制的專(zhuān)家評(píng)分原則,請(qǐng)專(zhuān)家分別對(duì)每一級(jí)指標(biāo)中各個(gè)指標(biāo)所占的比重進(jìn)行評(píng)分,根據(jù)評(píng)分結(jié)果結(jié)合專(zhuān)家權(quán)威程度系數(shù)分別確定每一級(jí)中每個(gè)指標(biāo)的權(quán)重。 (5)確定指標(biāo)標(biāo)準(zhǔn)：針對(duì)前期研究產(chǎn)出《縣級(jí)婦幼保健機(jī)構(gòu)績(jī)效評(píng)價(jià)指標(biāo)體系》,通過(guò)查閱國(guó)家相關(guān)規(guī)定、標(biāo)準(zhǔn)和全國(guó)婦幼保健機(jī)構(gòu)監(jiān)測(cè)現(xiàn)狀,經(jīng)過(guò)專(zhuān)家研討初步確定每個(gè)指標(biāo)的取值范圍作為考核標(biāo)準(zhǔn),通過(guò)第三輪德?tīng)柗茖?zhuān)家咨詢(xún)中對(duì)每一個(gè)指標(biāo)的取值范圍進(jìn)行了咨詢(xún),并通過(guò)現(xiàn)場(chǎng)調(diào)研和專(zhuān)家論證最后確定每一個(gè)指標(biāo)的考核標(biāo)準(zhǔn)。 第二部分評(píng)價(jià)指標(biāo)體系的現(xiàn)場(chǎng)驗(yàn)證 利用以上研究產(chǎn)出的縣級(jí)婦幼保健機(jī)構(gòu)績(jī)效評(píng)價(jià)指標(biāo)體系設(shè)計(jì)調(diào)查問(wèn)卷,對(duì)河北省鹿泉市、平山縣和贊皇縣三個(gè)縣級(jí)婦幼保健機(jī)構(gòu)進(jìn)行調(diào)查,對(duì)相關(guān)人員進(jìn)行個(gè)人訪(fǎng)談,征求其對(duì)評(píng)價(jià)結(jié)果的意見(jiàn),對(duì)評(píng)價(jià)體系的全面性、代表性和可行性進(jìn)行論證。 研究結(jié)果 經(jīng)過(guò)研究,制定了《縣級(jí)婦幼保健機(jī)構(gòu)績(jī)效評(píng)價(jià)指標(biāo)體系》。指標(biāo)體系由一、二、三級(jí)指標(biāo)組成。4個(gè)一級(jí)指標(biāo),包括“群體保健管理”、“保健服務(wù)”、“臨床服務(wù)”和“綜合與支持”；一級(jí)指標(biāo)下設(shè)10個(gè)二級(jí)指標(biāo),包括“保健服務(wù)管理”、“信息管理”、“健康教育”、“服務(wù)種類(lèi)”、“服務(wù)質(zhì)量”、“服務(wù)質(zhì)量”、“服務(wù)效率”、“投入”、“效益”、“評(píng)價(jià)”；二級(jí)指標(biāo)分解成42個(gè)三級(jí)指標(biāo)。 研究結(jié)論 本研究基于平衡計(jì)分卡理論概念框架,結(jié)合縣級(jí)婦幼保健機(jī)構(gòu)的職能定位和發(fā)展目標(biāo),通過(guò)科學(xué)的研究方法建立了縣級(jí)婦幼保健機(jī)構(gòu)績(jī)效評(píng)價(jià)指標(biāo)體系。所建立的指標(biāo)體系層次分明,指標(biāo)的數(shù)量適中、涵蓋全面,各級(jí)指標(biāo)權(quán)重適宜,能夠合理評(píng)價(jià)縣級(jí)婦幼保健機(jī)構(gòu)的績(jī)效水平。 在實(shí)施過(guò)程中還需對(duì)考核標(biāo)準(zhǔn)進(jìn)一步細(xì)化,提高其可操作性,同時(shí),評(píng)價(jià)指標(biāo)要隨著婦幼保健機(jī)構(gòu)的發(fā)展而不斷調(diào)整。
[Abstract]:research objective
Through the study, a set of scientific, feasible and suitable index system for the performance evaluation of China's County Maternal and child health care institutions is made. The evaluation index system embodies the functional orientation and development goals of the maternal and child health care institutions, and gives consideration to the individual health care of the maternal and child health care institutions, and the performance evaluation of medical and public health services.
The research content and method: This study is divided into two parts.
The first part is the establishment of performance evaluation index system of maternal and child health care institutions at county level.
(1) select the appropriate evaluation theory and establish the evaluation framework
First, we refer to a large number of relevant guidelines, policies, and other relevant documents on the development and evaluation of maternal and child health care institutions, and review the latest theories and methods of performance evaluation. On this basis, a framework for performance assessment and evaluation is established.
(2) collect relevant evaluation indexes
Through the literature review, 6 first grade index, 17 grade two index and 120 three grade index are selected. After 1 on-site discussions and 2 rounds of expert discussion and demonstration, a set of alternative index system for performance evaluation of county-level maternal and child health care institutions is formed. The system contains 6 first grade indicators, 15 two level index and 84 three index.
(3) index screening
According to the alternative index system, the Delphi expert consultation questionnaire is designed for the importance of the rationality of the evaluation framework and the importance of the alternative index. The feasibility is evaluated by the subjective score. After two rounds of expert consultation, the results of the two rounds of experts are statistically analyzed with the experts' authority degree coefficient, and the boundary value method is used to select the index. On the basis of quantitative analysis, on the basis of quantitative analysis, the experts' opinions and experts' opinions in the comprehensive recovery questionnaire and expert opinions are added, and the classification and name of the index are added, deleted or changed. Through two rounds of Delphy expert consultation and two expert demonstration, there are 4 first grade indexes, 10 two grade indexes and 42 Three level indicators of maternal and child health care institutions performance evaluation index system.
(4) to determine the weight of the index: third Delphi expert consultation on the final index system and the score principle of percentage of experts, experts are asked to score the proportion of each index in each index, and the weight of each index in each level is determined according to the result of the score and the coefficient of expert authority.
(5) to determine the index standard: according to the previous research output "performance evaluation index system of County Maternal and child health care institution", through consulting the relevant state regulations, standards and national maternal and child health care institutions monitoring status, through expert discussion, the scope of each index is preliminarily determined as the assessment standard, and through the third round of Delphi expert consultation on each A range of indicators was consulted, and the assessment criteria of each index were finally determined through field research and expert demonstration.
The field verification of the second part evaluation index system
Using the questionnaire of the performance evaluation index system of the County Maternal and child health care institutions above, three county-level maternal and child health care institutions in Luquan, Pingshan County and Zanhuang County of Hebei province were investigated. Personal interviews were carried out to relevant personnel to seek their opinions on the evaluation results, and the overall, representative and feasibility of the evaluation system. Research results
After research, the index system of the performance evaluation of the County Maternal and child health care institution is formulated. The index system consists of one, two and three level indicators, which consists of.4 first grade indicators, including "group health care management", "health care service", "clinical service" and "comprehensive and support", and 10 level two indicators under the first level index, including "health service management", " Information management, health education, service type, service quality, service quality, service efficiency, input, benefit, evaluation, and two level indicators are broken down into 42 three levels.
research conclusion
Based on the theoretical concept framework of the Balanced Scorecard and the function orientation and development goal of the county-level maternal and child health care institutions, the performance evaluation index system of the County Maternal and child health care institutions is established through scientific research methods. The index system is clear, the number of indicators is moderate, and the weight of the indicators at all levels is suitable and the index weight is suitable. Reasonable evaluation of the performance level of maternal and child health care institutions at county level.
In the process of implementation, it is necessary to further refine the assessment criteria and improve its operability. At the same time, the evaluation indicators should be adjusted continuously along with the development of MCH institutions.
