【摘要】：目的了解長沙市望城區(qū)契約式家庭醫(yī)生制度的實(shí)施現(xiàn)狀及影響因素。方法選取長沙市望城區(qū)6個(gè)實(shí)施家庭健康契約式服務(wù)的鎮(zhèn)衛(wèi)生院185例就診患者和63名醫(yī)務(wù)人員,就契約式家庭醫(yī)生服務(wù)的發(fā)展現(xiàn)狀和影響因素進(jìn)行問卷調(diào)查。結(jié)果望城區(qū)居民對鎮(zhèn)衛(wèi)生院的總滿意率為49.44%;對不同機(jī)構(gòu)滿意度差異情況進(jìn)行分析,結(jié)果顯示:在就醫(yī)環(huán)境、服務(wù)態(tài)度和醫(yī)療設(shè)備設(shè)施上,6個(gè)鎮(zhèn)衛(wèi)生院滿意度比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);Logistic回歸分析結(jié)果顯示,候診時(shí)間長短、診療價(jià)格、服務(wù)態(tài)度、病情解釋、技術(shù)水平、診療效果是影響居民滿意度的主要因素(P0.05);發(fā)展前景,工作量和時(shí)間的分配,公平性,工作風(fēng)險(xiǎn)是影響醫(yī)務(wù)人員滿意度的因素(P0.05)。結(jié)論望城區(qū)契約式家庭醫(yī)生制度的實(shí)施過程中存在較多的困難和影響因素,可以從家庭醫(yī)生崗位職責(zé)、家庭醫(yī)生的績效評(píng)價(jià)、醫(yī)療保險(xiǎn)報(bào)銷比例的分配等方面進(jìn)行完善。
[Abstract]:Objective to understand the present situation and influencing factors of contract family doctor system in Wangcheng District of Changsha City. Methods 185 patients and 63 medical staff were selected from 6 town health centers in Wangcheng District of Changsha City to investigate the development status and influencing factors of contract family doctor service. Results the total satisfaction rate of the residents in Wangcheng District was 49.44, and the difference of satisfaction between different institutions was analyzed. In terms of service attitude and medical equipment and facilities, there were significant differences in satisfaction of six town hospitals by logistic regression analysis. The results showed that the waiting time, the price of diagnosis and treatment, the attitude of service, the explanation of illness, the level of technology, etc. The effect of diagnosis and treatment is the main factor affecting the satisfaction degree of residents (P0.05), and the development prospect, the distribution of workload and time, the fairness and the work risk are the factors that influence the satisfaction of the medical staff. Conclusion there are many difficulties and influencing factors in the implementation of contract family doctor system in Wangcheng area, which can be improved from the aspects of family doctor's duty, family doctor's performance evaluation, distribution of medical insurance reimbursement ratio, etc.
