【摘要】：目的基于多維指標(biāo)構(gòu)建治療質(zhì)量綜合評(píng)價(jià)模型,探討模型估計(jì)治療質(zhì)量的有效性;綜合評(píng)價(jià)醫(yī)院、醫(yī)生乳腺癌的治療質(zhì)量。方法構(gòu)建兩參數(shù)logistic潛變量模型作為測(cè)量模型,將測(cè)量模型中的潛變量作為因變量整合進(jìn)多水平結(jié)構(gòu)模型,設(shè)定虛擬水平代表多維潛變量,構(gòu)建多水平結(jié)構(gòu)模型。應(yīng)用MCMC估計(jì)模型參數(shù),擬合三水平二維模型分析實(shí)例數(shù)據(jù)。結(jié)果模擬研究結(jié)果提示,評(píng)價(jià)指標(biāo)數(shù)及醫(yī)生數(shù)均為20,模型估計(jì)治療質(zhì)量的有效性最好;應(yīng)用該模型分析可以獲得乳腺癌診斷、手術(shù)操作質(zhì)量較優(yōu)的醫(yī)生及醫(yī)院。結(jié)論多水平多維潛變量模型可以分析具有層次結(jié)構(gòu)和多維性的治療質(zhì)量評(píng)價(jià)數(shù)據(jù),能夠做到全面、合理的進(jìn)行疾病治療質(zhì)量綜合評(píng)價(jià);治療質(zhì)量綜合評(píng)價(jià)使醫(yī)院的等級(jí)劃分、醫(yī)生的績(jī)效考核與治療質(zhì)量的銜接成為可能。
[Abstract]:Objective to establish a comprehensive evaluation model based on the treatment of quality indexes, to explore the effective treatment model to estimate the quality; comprehensive evaluation of the hospital, the doctor for the treatment of breast cancer. The quality of construction method of the two parameter logistic model as a latent variable measurement model, the measurement model of latent variable as the dependent variable into the multilevel structure model, set the virtual level representative multidimensional latent variables, constructing multi level structure model. The model parameters are estimated by MCMC, three level fitting two-dimensional model instance data. Results the simulation results indicate that the evaluation index number and the number of doctors was 20, effective treatment model to estimate the quality of the best; the application of the model analysis can get the diagnosis of breast cancer, doctors and hospital surgery the operation quality is excellent. Conclusion multilevel multidimensional latent variable model can analyze the treatment quality evaluation data with hierarchical structure and multi dimension, can A comprehensive and reasonable comprehensive evaluation of the quality of disease treatment is needed. The comprehensive evaluation of treatment quality enables the classification of hospitals and the connection between doctors' performance evaluation and treatment quality.
