【摘要】:正案情簡(jiǎn)介■申請(qǐng)人于某系某酒店員工,2012年5月3日,于某在酒店收貨平臺(tái)上驗(yàn)貨時(shí),被酒店另一員工不慎撞傷了腰部。醫(yī)院診斷為:腰部外傷,腰椎間盤(pán)突出癥。2012年8月26日,于某向社會(huì)保險(xiǎn)行政部門(mén)提出工傷認(rèn)定申請(qǐng)。社會(huì)保險(xiǎn)行政部門(mén)受理后向某酒店發(fā)出《工傷認(rèn)定舉證通知書(shū)》,要求該酒店在5日內(nèi)提供于某所受傷害不是工傷的證據(jù)
