【摘要】：目的觀察涼血解毒化瘀法對HBV-ACLF T細(xì)胞亞群的影響,探索中醫(yī)藥治療ACLF的可能免疫機(jī)制。方法將58例慢ACLF隨機(jī)分為實(shí)驗(yàn)組和對照組,在治療的0周、4周、8周分別檢測肝功能、T細(xì)胞亞群、NK、血清內(nèi)毒素水平進(jìn)行統(tǒng)計(jì)分析比較。結(jié)果 4周、8周肝功TBIL、DBIL、ALT和AST與0周比均有差異(P0.05);4周兩組間TBIL、DBIL、T細(xì)胞亞群及NK有差異(P0.05);8周兩組間ALT、AST、ALB、LPS有差異(P0.05),而T細(xì)胞亞群及NK無差異(P0.05)。結(jié)論涼血解毒化瘀法可以改善HBV-ACLF患者肝臟合成功能、降低LPS水平,短期內(nèi)改善、調(diào)節(jié)HBV-ACLF T細(xì)胞免疫紊亂狀態(tài)。

【正文快照】： 慢加急性肝衰竭(acute-on-chronic liver failure,ACLF)是在慢性肝臟疾病基礎(chǔ)上出現(xiàn)急性(通常在4周內(nèi))肝功能失代償?shù)谋憩F(xiàn),如黃疸進(jìn)行性加深(TB≥171umol/L或每日上升≥171umol/L),PTA≤40%或INR1.5,出現(xiàn)感性腦病等[1]。目前對于ACLF的發(fā)病機(jī)制并不清楚,PIRO假說認(rèn)為ACLF主要
