【摘要】：目的：觀察推拿聯(lián)合中藥天麻鉤藤飲化裁治療頸性眩暈的臨床療效。方法：將符合納入標(biāo)準(zhǔn)的80例患者隨機分為推拿組及推拿、口服中藥組,兩組均為40例,推拿結(jié)合中藥組在推拿組的基礎(chǔ)上,服用天麻鉤藤飲化裁治療,比較兩組的臨床療效,并通過臨床癥狀的改善、功能評估表評分及TCD指標(biāo)等的變化評價其療效。結(jié)果：臨床療效：推拿結(jié)合中藥組總有效率明顯優(yōu)于單純推拿組。頸性眩暈癥狀及功能評估表評分：兩組在治療后評分均明顯高于治療前；推拿結(jié)合中藥組治療前后評分差值明顯高于推拿組。椎-基動脈血流動力學(xué)變化：兩組治療后雙側(cè)椎動脈(VA)及基底動脈(BA)收縮期流速峰值(Vs)均較治療前有明顯改善,推拿結(jié)合中藥組療效優(yōu)于單純推拿組。結(jié)論：1.推拿結(jié)合中藥天麻鉤藤飲化裁臨床療效顯著,能夠顯著改善患者臨床癥狀。2.推拿結(jié)合中藥天麻鉤藤飲化裁作用機制可能是通過調(diào)節(jié)小關(guān)節(jié)紊亂,促進局部軟組織炎癥的吸收,起到改善椎動脈供血。3.推拿結(jié)合中藥天麻鉤藤飲化裁優(yōu)于單純推拿手法治療,說明傳統(tǒng)手法配合中藥對某些疾病可以起到雙重治療作用。
[Abstract]:Objective: to observe the clinical effect of massage combined with Tianma Gouteng decoction in the treatment of cervical vertigo. Methods: 80 patients who met the inclusion criteria were randomly divided into massage group (n = 40) and massage group (n = 40). The patients in massage combined with traditional Chinese medicine group were treated with Tianma Gouteng decoction on the basis of massage group. The clinical efficacy of the two groups was compared, and the curative effect was evaluated by the improvement of clinical symptoms, the score of function evaluation form and TCD index. Results: the total effective rate of massage combined with traditional Chinese medicine group was significantly better than that of simple massage group. The scores of symptoms and function evaluation table of cervical vertigo: the scores of the two groups after treatment were significantly higher than those before treatment, and the difference of scores before and after treatment in the massage combined with traditional Chinese medicine group was significantly higher than that in the massage group. Hemodynamic changes of vertebral-basal artery: after treatment, the peak systolic velocity (Vs) of bilateral vertebral artery (VA) and basilar artery (BA) in both groups were significantly improved compared with those before treatment, and the curative effect of massage combined with traditional Chinese medicine group was better than that of simple massage group. Conclusion: 1. Massage combined with Tianma Gouteng decoction has remarkable clinical effect and can significantly improve the clinical symptoms of patients. 2. The mechanism of massage combined with Tianma Gouteng decoction may be to improve the blood supply of vertebral artery by regulating the disorder of facet joint and promoting the absorption of local soft tissue inflammation. 3. Massage combined with traditional Chinese medicine Gastrodia elata is superior to massage alone, indicating that traditional manipulation combined with traditional Chinese medicine can play a dual role in the treatment of some diseases.
