【摘要】：目的:觀察和研究自制溫陽灸條治療早期糖尿病腎病陽虛質(zhì)的臨床療效及其對(duì)早期糖尿病腎病陽虛質(zhì)患者血肌酐、血尿素氮、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血紅蛋白等臨床指標(biāo)的影響,同時(shí),觀察自制溫陽灸條對(duì)中醫(yī)體質(zhì)調(diào)節(jié)的影響,進(jìn)而探討自制溫陽灸條對(duì)早期糖尿病腎病陽虛質(zhì)治療的影響機(jī)制,為中醫(yī)外法治療早期糖尿病腎病陽虛質(zhì)提供理論依據(jù)。方法:選取70例早期糖尿病腎病陽虛質(zhì)患者,采用單盲法,隨機(jī)分為治療組35例,對(duì)照組35例。對(duì)照組采用一般性治療和常規(guī)對(duì)癥治療,再施以清艾條行溫和灸法,治療組在對(duì)照組治療基礎(chǔ)上換用自制溫陽灸條行溫和灸法。自制溫陽灸條藥物組成:將清艾條中艾絨與肉桂、黃芪、附子研成的細(xì)粉以2:1:1:1的比例混勻制成自制溫陽灸條,規(guī)格為直徑14mm、長度20mm。(中藥、清艾條均來源于云南中醫(yī)學(xué)院第一附屬醫(yī)院中藥房)灸雙側(cè)腎俞穴、命門穴、關(guān)元穴,雙側(cè)足三里穴,施灸時(shí)將艾條一端點(diǎn)燃,插入艾灸架中約距皮膚2-3cm左右,使灸架對(duì)準(zhǔn)應(yīng)灸腧穴。每穴每次15 min。每日1次,每星期6次,2星期為1個(gè)療程,共治療3個(gè)療程。治療3個(gè)療程后,觀察兩組治療前后血肌酐、血尿素氮、24小時(shí)尿蛋白定量、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血紅蛋白變化情況(安全性觀測、診斷學(xué)療效,中醫(yī)臨床癥候療效、中醫(yī)體質(zhì)調(diào)節(jié)情況)。結(jié)果:1.70例患者,10例被剔除,60例納入本研究,患者依從性大于85.71%,兩組患者在性別、年齡、病程、治療前中醫(yī)癥候評(píng)分及中醫(yī)體質(zhì)等臨床一般資料方面無顯著性差異。2.經(jīng)過3個(gè)療程治療后,治療組總有效率為83.33%;對(duì)照組總有效率為60.00%,兩組比較有顯著性差異(P㩳0.05)。3.兩組治療后,血肌酐、血尿素氮、24小時(shí)尿蛋白定量、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血紅蛋白與本組治療前比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05,P0.01);治療組在降低血肌酐、24小時(shí)尿蛋白定量、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血紅蛋白方面以及改善中醫(yī)臨床證候及陽虛質(zhì)程度方面優(yōu)于對(duì)照組(P0.05,P0.01)。結(jié)論:自制溫陽灸條對(duì)早期糖尿病腎病陽虛質(zhì)有較好的臨床療效,在提高整體療效,改善患者中醫(yī)臨床癥狀,降低血肌酐、24小時(shí)尿蛋白定量、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血紅蛋白方面,治療組效果優(yōu)于對(duì)照組。
