【摘要】：目的探討消栓通絡(luò)顆粒內(nèi)服聯(lián)合"靳三針"的針?biāo)幗Y(jié)合康復(fù)療法治療痙攣性癱瘓的臨床療效。方法將128例腦卒中后痙攣性癱瘓患者分為對照組和觀察組。在運(yùn)動康復(fù)的基礎(chǔ)上,對照組采用"靳三針",觀察組在對照組的基礎(chǔ)加用消栓通絡(luò)顆粒,每次12g,3次/天,開水沖服。療程均為4周。采用改良Ashworth評分量表(MAS)評價肌力情況,采用臨床痙攣指數(shù)(CSI)評價痙攣程度,采用功能獨(dú)立性評定量表(FIM)評定日常生活能力,采用四肢簡化Fugl-Meyer功能量表(FMA)評價肢體的功能活動;進(jìn)行神經(jīng)功能缺損評分(NDS)和腦卒中痙攣性癱瘓患者報告的臨床結(jié)局(PRO)評價。結(jié)果觀察組臨床療效總有效率為84.75%,高于對照組(67.24%),差異有統(tǒng)計學(xué)意義(χ~2=4.923,P0.05);治療后觀察組患者M(jìn)AS和CSI評分低于對照組(P0.01);治療后觀察組患者FIM和Fugl-Meyer(上、下肢)評分均高于對照組(P0.01);治療后觀察組NDS和PRO各維度評分均低于對照組(P0.01)。結(jié)論消栓通絡(luò)顆粒內(nèi)服聯(lián)合"靳三針"治療痙攣性偏癱能進(jìn)一步減輕患者的痙攣程度,改善神經(jīng)缺損功能,提高運(yùn)動活動能力和日常生活能力,提高患者自身感受和臨床療效。
[Abstract]:Objective to explore the clinical effect of Xiaoshuan Tongluo granule combined with Jin San Needle combined with rehabilitation therapy in the treatment of spastic paralysis. Methods 128 patients with spastic paralysis after stroke were divided into two groups: control group and observation group. On the basis of exercise rehabilitation, Jin three Needles were used in the control group. In the control group, Xiaoshuantongluo granules were added to the control group, each time for 12 g, 3 times a day, and the patients in the observation group were washed with boiling water. The course of treatment was 4 weeks. The muscle strength was evaluated by modified Ashworth scale (MAS), the degree of spasm was evaluated by clinical spasm index (CSI), and the ability of daily living (ADL) was assessed by functional independence scale (FIM). The functional activities of limbs were evaluated by using simplified Fugl-Meyer functional scale (FMA). Neurologic deficit score (NDS) and clinical outcome of stroke spasmodic paralysis reported by (PRO) were evaluated. Results the total effective rate of the observation group was 84.75%, which was significantly higher than that of the control group (67.24%) (蠂 ~ 2, 4.923, P0.05), and the scores of MAS and CSI in the observation group after treatment were lower than those in the control group (P0.01). After treatment, the scores of FIM and Fugl-Meyer (upper and lower limbs) in the observation group were higher than those in the control group (P0.01), and the scores of NDS and PRO in the observation group after treatment were lower than those in the control group (P0.01). Conclusion the treatment of spastic hemiplegia with Xiaoshuantongluo granule combined with Jin San Needle can further relieve the spasm degree of the patients, improve the nerve defect function, improve the ability of motor activity and daily living, and improve the patient's own feeling and clinical curative effect.
