【摘要】：目的：通過觀察頻發(fā)室性期前收縮患者應(yīng)用早搏靈膠囊后對QT離散度及心率變異性的影響,來評估復(fù)方中藥治療本病的的療效和安全性。方法：本研究為自身對照研究,選取80例頻發(fā)室性期前收縮的患者,男35例,女45例,平均年齡在59.61±12..34歲。所有患者均來自黑龍江中醫(yī)藥大學附屬第一醫(yī)院心內(nèi)二科門診。主要終點是觀察患者應(yīng)用早搏靈膠囊治療前及治療后的QT離散度(QT dispersion,QTd)、校正QT離散度(Heartrate-corrected QT dispersion,QTcd)和心率變異性(Heart rate variabil ity,HRV)的變化,次要終點是觀察治療后室早數(shù)量變化及早搏靈膠囊的療效和安全性。所有患者均口服早搏靈膠囊4粒／次,3次／日。觀察患者服藥前及服藥4周后動態(tài)心電圖QTd、QTcd及HRV變化。數(shù)據(jù)采用SPSS22.0軟件分析結(jié)果。結(jié)果：(1)早搏靈膠囊能明顯降低室早數(shù)量(P0.05),24h平均心率在治療后無差異(P0.05)。(2)QT、QTc治療前后無明顯統(tǒng)計學意義(P0.05)、QTd及QTcd與治療前相比,差異有統(tǒng)計學意義(P0.01)。(3)治療前后的QT max和QTc max差異有統(tǒng)計學意義(P0.05)。QT min和QTc min治療前后差異無統(tǒng)計學意義(P0.05)。(4)HRV時域指標SDNN、SDANN、 SDNNindex、 pNN50、RMSSD及頻域指標LF、HF,與治療前相比,差異有統(tǒng)計學意義(P0.01)。LF/HF比值,用藥前后無差異(P0.05)。結(jié)論：1.早搏靈膠囊能縮短室早患者的0Td及QTcd。2.早搏靈膠囊能改善心臟自主神經(jīng)功能,提高心率變異性。
[Abstract]:Aim: to evaluate the efficacy and safety of compound Chinese medicine on QT dispersion and heart rate variability (HRV) in patients with frequent ventricular extrasystole by observing the effect of Zoboling capsule on VPV dispersion and heart rate variability (HRV) in patients with frequent ventricular extrasystole. Methods: 80 patients (35 males and 45 females) with frequent ventricular extrasystole were selected as a self-control study. The mean age was 59.61 鹵12.34 years old. All patients from the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine Heart 2 out-patient. The main endpoint was to observe the changes of QT dispersion (QT dispersion,QTd), corrected QT dispersion (Heartrate-corrected QT dispersion,QTcd) and heart rate variability (Heartrate variabil ity,HRV) before and after treatment with Zoboling capsule. The secondary end point is to observe the changes of ventricular premature quantity after treatment and the efficacy and safety of Bering capsule. All patients took Zoboling capsule 4 times, 3 times a day. The changes of QTd,QTcd and HRV were observed before and 4 weeks after treatment. The data were analyzed by SPSS22.0 software. Results: (1) Zaobaoling capsule could significantly reduce the number of ventricular premature (P0.05), and the 24-hour mean heart rate had no significant difference after treatment (P0.05). (2). There was no significant difference before and after QT,QTc treatment (P0.05). Compared with pre-treatment, QTd and QTcd, There was significant difference in QT max and QTc max before and after treatment (P0.01). (3). There was no significant difference between). QT min and QTc min before and after treatment (P0.05). (4) HRV time domain index SDNN,SDANN, (P0.05). (4). There was significant difference in SDNNindex, pNN50,RMSSD and frequency domain index LF,HF, between before and after treatment (P0.01). LF / HF ratio had no difference before and after treatment (P0.05). Conclusion: 1. 0Td and QTcd.2. of Zaoerling capsule in patients with short Ventricular premature Zaobaoling capsule can improve the function of cardiac autonomic nerve and increase heart rate variability.
