【摘要】：目的本課題旨在觀察祛風牽正推拿法配合康復訓練治療腦癱患兒斜視的臨床療效,為腦癱患兒斜視的治療提供一種新的治療方法。方法將60例腦癱且伴有斜視癥狀的患兒,隨機分為研究組和對照組各30例。兩組均進行常規(guī)的康復治療,研究組在康復治療的基礎上配合祛風牽正推拿法的治療。兩組分別于治療前和治療4個月后行眼球運動、斜視度、代償頭位的檢查,觀察治療4個月后兩組患兒斜視改善的臨床療效。結果(1)治療后兩組患兒斜視的改善情況,研究組總有效率為90%,對照組總有效率為76.7%,兩組療效對比,差異具有統(tǒng)計學意義,研究組療效優(yōu)于對照組(P0.05)。(2)研究組、對照組眼球運動障礙級別治療前、后自身比較,級別均降低,差異具有統(tǒng)計學意義(P0.05);治療后研究組與對照組眼球運動障礙級別組間比較,有統(tǒng)計學差異(P0.05),研究組優(yōu)于對照組。(3)研究組、對照組斜視度治療前、后自身比較均得到改善,差異具有統(tǒng)計學意義(P0.05);治療后研究組與對照組斜視度組間比較,差異具有統(tǒng)計學意義(P0.05),研究組優(yōu)于對照組。(4)研究組、對照組代償頭位治療前、后自身比較均得到改善,差異具有統(tǒng)計學意義(P0.05),治療后研究組與對照組代償頭位比較,差異具有統(tǒng)計學意義(P0.05),研究組優(yōu)于對照組。結論(1)祛風牽正推拿法配合康復治療、康復治療兩種方法對于腦癱患兒斜視的治療均有效;(2)祛風牽正推拿法配合康復治療對腦癱患兒斜視治療的療效優(yōu)于單純康復治療,在改善患兒的斜視方面能發(fā)揮其獨特優(yōu)勢,為小兒腦癱斜視的治療提供了新的治療方法;(3)治療的臨床效果與患兒的年齡關系密切,臨床提示盡早的發(fā)現(xiàn)、干預及治療,更有利于腦癱患兒斜視的康復。
[Abstract]:Objective to observe the clinical efficacy of wind-dispelling traction massage combined with rehabilitation training in the treatment of strabismus in children with cerebral palsy, and to provide a new method for the treatment of strabismus in children with cerebral palsy. Methods 60 children with cerebral palsy with strabismus were randomly divided into study group (n = 30) and control group (n = 30). Both groups were treated with routine rehabilitation therapy, and the study group combined with the therapy of dispelling wind traction and massage on the basis of rehabilitation treatment. Eyeball movement, strabismus and compensatory head position were examined before and 4 months after treatment in the two groups, and the clinical effects of strabismus improvement in the two groups were observed after 4 months of treatment. Results (1) after treatment, the total effective rate was 90% in the study group and 76.7% in the control group. The difference was statistically significant between the two groups in the improvement of strabismus, the total effective rate was 90% in the study group and 76.7% in the control group. The curative effect of the study group was better than that of the control group (P0.05). (2). Before and after the treatment, the level of eyeball movement disorder in the control group was lower than that of the control group (P0.05). After treatment, there was statistical difference between the study group and the control group (P0.05), the study group was superior to the control group. (3) the study group, the control group before and after the treatment of strabismus, the self-comparison was improved. The difference was statistically significant (P0.05); After treatment, the difference of strabismus between the study group and the control group was statistically significant (P0.05), and the study group was superior to the control group. (4) the study group and the control group improved before and after the treatment of compensatory head position. The difference was statistically significant (P0.05). After treatment, the study group and the control group compensated head position, the difference was statistically significant (P0.05), the study group was superior to the control group. Conclusion (1) both methods are effective in the treatment of strabismus in children with cerebral palsy. (2) the treatment of strabismus in children with cerebral palsy by dispelling wind, pulling and massage combined with rehabilitation is better than that of rehabilitation alone. It can play its unique advantage in improving children's strabismus, and provides a new treatment method for children with cerebral palsy strabismus; (3) the clinical effect of the treatment is closely related to the age of the children. It is suggested that early discovery, intervention and treatment are more beneficial to the rehabilitation of strabismus in children with cerebral palsy.
