【摘要】：目的:中風(fēng)后肢體痙攣具有發(fā)病率高、致殘率高、治療難度大等特點(diǎn),本課題即基于此,欲通過(guò)多針淺刺法與傳統(tǒng)針刺法對(duì)中風(fēng)后肢體痙攣的患者進(jìn)行治療前后的肌張力比較,觀察兩種方法的療效,旨在探求出治療本病的更有效的療法。方法:收集符合條件的中風(fēng)后肢體痙攣患者60例,利用Excel軟件將符標(biāo)準(zhǔn)的60個(gè)名額序號(hào)隨機(jī)分配至多針淺刺治療組與常規(guī)針刺治療組,每組各30例。兩組均為每天治療一次,針刺得氣后留針30分鐘。治療1療程20天后,通過(guò)改良的Ashwortn痙攣評(píng)定肌張力,采用簡(jiǎn)化的Fugl-Meyer運(yùn)動(dòng)量表評(píng)估肢體的運(yùn)動(dòng)功能,對(duì)比兩組治療前后的治療效果。結(jié)果:1.肌張力療效方面:經(jīng)過(guò)兩個(gè)療程的治療后,兩組患者的肢體肌張力的療效分別為:治療組上肢的有效率為83%,對(duì)照組為60%,兩組有顯著的差異性(P0.01)。治療組下肢的有效率是86%,對(duì)照組是70%,兩組具有統(tǒng)計(jì)學(xué)意義(P0.05),說(shuō)明兩種療法都可降低肌張力,治療組優(yōu)于對(duì)照組。2.肢體運(yùn)動(dòng)功能方面:兩組對(duì)痙攣的肢體FMA評(píng)分值均有提高,治療后兩組比較P0.01,說(shuō)明差異具有顯著性,且治療組優(yōu)于對(duì)照組。治療組對(duì)患者上下肢的FMA改善作用較明顯(P0.01),兩組上下肢的FMA改善有顯著性差異(P0.01)。結(jié)論:兩種療法均可降低患肢過(guò)高的肌張力,可以提高患肢FMA分?jǐn)?shù),改善痙攣患者上下肢運(yùn)動(dòng)功能,多針淺刺法治療中風(fēng)后肢體痙攣比常規(guī)針刺法療效更好。
[Abstract]:Objective: limb spasm after apoplexy has the characteristics of high incidence, high rate of disability and difficulty in treatment. Based on this, this paper intends to compare the muscle tension before and after treatment with multi-needle shallow acupuncture method and traditional acupuncture method for patients with post-stroke limb spasm, and it is necessary to compare the muscle tension before and after treatment with multiple shallow acupuncture method and traditional acupuncture method. Observe the efficacy of the two methods in order to explore a more effective treatment for the disease. Methods: 60 patients with limb spasm after apoplexy were collected and randomly assigned to the light acupuncture treatment group and the routine acupuncture treatment group by using Excel software. Each group had 30 cases in each group. The patients were divided into two groups: one group (n = 30) and the other group (n = 30). Both groups were treated once a day. After acupuncture, the needle was retained for 30 minutes. After 20 days of treatment, the muscle tension was assessed by modified Ashwortn spasm, and the motor function of limbs was evaluated by simplified Fugl-Meyer exercise scale. The therapeutic effects were compared between the two groups before and after treatment. Results: 1. After two courses of treatment, the effective rate of upper limb tension was 83% in the treatment group and 60% in the control group. There was significant difference between the two groups (P0.01). The effective rate of the treatment group was 86%, that of the control group was 70%, the two groups had statistical significance (P0.05), indicating that the two kinds of therapy can reduce muscle tension, the treatment group is better than the control group. 2. In the aspect of limb motor function, the scores of limb FMA in the two groups were improved. After treatment, the two groups were compared with P0.01, indicating that the difference was significant, and the treatment group was superior to the control group. The FMA of upper and lower extremities in the treatment group was significantly improved (P0.01), and there was a significant difference between the two groups in improving the FMA of the upper and lower extremities (P0.01). Conclusion: both methods can reduce the hypertensity of the affected limbs, improve the FMA score of the affected limbs and improve the motor function of the upper and lower limbs of the patients with spasticity. The therapeutic effect of multi-acupuncture shallow acupuncture is better than that of the conventional acupuncture in the treatment of limb spasm after apoplexy.
