【摘要】：目的:根據(jù)傅汝林教授運(yùn)用水蛭治療疑難雜癥的經(jīng)驗(yàn),我們采用對(duì)比研究方法,評(píng)價(jià)水蛭配合西藥治療慢性腎臟病的臨床療效與安全性。方法:將54例符合CKD CKD3~4期診斷標(biāo)準(zhǔn)的患者用隨機(jī)數(shù)字表法隨機(jī)分為治療組和對(duì)照組各27例,兩組患者均采用慢性腎臟病基礎(chǔ)治療。治療組:在基礎(chǔ)治療上加服水蛭粉。對(duì)照組:僅予基礎(chǔ)治療。共觀察4周,比較中醫(yī)證候積分,血肌酐、24h尿蛋白定量、血常規(guī)、肝功能、凝血功能、大便常規(guī)+潛血等指標(biāo)的變化。結(jié)果:治療組與對(duì)照組在年齡、性別、原發(fā)病及相關(guān)指標(biāo)方面無(wú)統(tǒng)計(jì)學(xué)差異。治療組脫落1例,對(duì)照組自行退出1例。中醫(yī)證候療效比較,兩組治療前無(wú)統(tǒng)計(jì)學(xué)差異,治療后治療組總有效率為73.1%,對(duì)照組總有效率為26.9%,兩組治療后療效比較有顯著性差異(P0.01)。兩組治療前血肌酐比較無(wú)統(tǒng)計(jì)學(xué)差異,治療后治療組平均血肌酐水平較對(duì)照組平均血肌酐水平下降,經(jīng)比較有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組治療前、后24h尿蛋白定量比較無(wú)統(tǒng)計(jì)學(xué)差異。兩組患者治療后血常規(guī)、肝功能、凝血功能、大便常規(guī)+潛血檢查均未見(jiàn)異常。結(jié)論:水蛭粉對(duì)慢性腎臟病可明顯改善臨床癥狀及腎功能,大量(9g/d)吞服療效確切,且無(wú)明顯毒副反應(yīng)。
[Abstract]:Aim: to evaluate the clinical efficacy and safety of Hirudo combined with western medicine in the treatment of chronic kidney disease according to Professor Fu Rulin's experience in the treatment of difficult and complicated diseases. Methods: 54 patients with CKD CKD3~4 stage were randomly divided into two groups: treatment group (n = 27) and control group (n = 27). Treatment group: add hirudin powder to basic treatment. Control group: basic treatment only. The changes of TCM syndrome score, serum creatinine, 24-hour urine protein quantity, blood routine, liver function, coagulation function and stool routine occult blood were observed for 4 weeks. Results: there was no significant difference in age, sex, primary disease and related indexes between the treatment group and the control group. There were 1 case in treatment group and 1 case in control group. After treatment, the total effective rate was 73.1% in the treatment group and 26.9% in the control group. There was significant difference in the curative effect between the two groups after treatment (P0.01). There was no significant difference in serum creatinine between the two groups before treatment. After treatment, the mean serum creatinine level in the treatment group was lower than that in the control group, and there was statistically significant difference between the two groups (P0.05). There was no statistical difference between the two groups before treatment and 24 hours after treatment. After treatment, blood routine, liver function, coagulation function and stool routine occult blood examination were not abnormal in both groups. Conclusion: hirudin powder can significantly improve the clinical symptoms and renal function in patients with chronic renal disease. A large number of (9g/d) swallowing is effective and has no obvious toxic and side effects.
