針刺治療血瘀型卵巢巧克力囊腫的臨床觀察

【摘要】：目的:對(duì)符合標(biāo)準(zhǔn)的血瘀型卵巢巧克力囊腫患者進(jìn)行針刺治療,觀察針刺治療對(duì)血瘀型卵巢巧克力囊腫患者的中醫(yī)癥候積分、CA125水平、囊腫最大徑線及生存質(zhì)量評(píng)分改善的情況。方法:將收集于黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院婦科門診符合血瘀型診斷標(biāo)準(zhǔn)的60例卵巢巧克力囊腫患者給予針刺治療,根據(jù)針刺部位不同分成治療組和對(duì)照組各30例。治療組針刺關(guān)元、中極、地機(jī)、氣海、子宮、天樞、次毼、血海(雙側(cè))、三陰交(雙側(cè))、足三里(雙側(cè))、太沖(雙側(cè))。對(duì)照組針刺非經(jīng)絡(luò)非穴位的位置,每日針刺1次,每次30分鐘,1個(gè)月為一個(gè)療程,連續(xù)治療3個(gè)療程后觀察兩組患者在治療前后的中醫(yī)癥候積分、CA125水平、囊腫最大徑線及生存質(zhì)量方面的變化情況,綜合評(píng)估針刺療法治療血瘀型卵巢巧克力囊腫的臨床療效。結(jié)果:1.治療后兩組的臨床綜合療效評(píng)定結(jié)果顯示治療組痊愈1例,顯效14例,有效10例,無(wú)效5例,總有效率高達(dá)83.33%,對(duì)照組痊愈0例,顯效4例,有效8例,無(wú)效17例,總有效率為41.37%。治療組優(yōu)于對(duì)照組,療效差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。2.治療后兩組的中醫(yī)癥候積分相比差異顯著,具有統(tǒng)計(jì)學(xué)意義(P0.05)。3.治療后治療組與對(duì)照組相比CA125水平明顯下降,差異明顯,具有統(tǒng)計(jì)學(xué)意義。4.治療后治療組與對(duì)照組相比巧囊最大徑線明顯縮小,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。5.治療后兩組的生存質(zhì)量評(píng)分差異顯著,具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:針刺療法對(duì)血瘀型卵巢巧克力囊腫的臨床治療效果確切,可以提高臨床綜合療效,顯著改善中醫(yī)癥候積分,降低CA125數(shù)值,縮小病灶包塊,提高患者的生存質(zhì)量,提升包括控制疼痛的各項(xiàng)能力。
[Abstract]:Objective: to observe the TCM symptom integral and CA125 level of the patients with blood stasis type ovarian chocolate cyst treated by acupuncture, and to observe the effect of acupuncture treatment on the syndrome score and the level of TCM of the patients with blood stasis type ovarian chocolate cyst. The maximum diameter of the cyst and the improvement of the quality of life score. Methods: 60 patients with ovarian chocolate cyst were treated with acupuncture and divided into two groups: treatment group (n = 30) and control group (n = 30), who were collected from the Gynecology Clinic of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. The treatment group acupuncture Guan Yuan, middle pole, earth machine, qi sea, uterus, Tianshu, secondary, blood sea (both sides), three Yin Jiao (both sides), Zusanli (both sides), Taichong (both sides). The position of non-meridian and non-acupoint acupuncture in the control group was observed once a day for 30 minutes, 1 month as a course of treatment. After 3 consecutive courses of treatment, the TCM symptom score and CA125 level of the two groups were observed before and after treatment. To evaluate the clinical efficacy of acupuncture therapy in the treatment of blood stasis ovarian chocolate cyst. Results: 1. After treatment, the clinical comprehensive curative effect evaluation of the two groups showed that the treatment group was cured in 1 case, remarkable effect in 14 cases, effective in 10 cases, ineffective in 5 cases, the total effective rate was 83.33%, the control group was cured in 0 cases, remarkable effect in 4 cases, effective in 8 cases, ineffective in 17 cases, the total effective rate was 83.33% in the control group. The total effective rate was 41.37%. The therapeutic effect of the treatment group was better than that of the control group, the difference was statistically significant (P0.05). 2. After treatment, the scores of TCM symptoms between the two groups were significantly different, with statistical significance (P0.05). 3. After treatment, the level of CA125 in the treatment group was significantly lower than that in the control group (P < 0.05). After treatment, the maximum diameter of the capsule in the treatment group was significantly smaller than that in the control group (P0.05). After treatment, there was significant difference in the quality of life score between the two groups (P0.05). Conclusion: acupuncture therapy is effective in the treatment of blood stasis ovarian chocolate cyst. It can improve the clinical comprehensive curative effect, significantly improve the TCM syndrome score, decrease the CA125 value, reduce the mass of the lesion, and improve the quality of life of the patients, and improve the quality of life of the patients.
