基于關(guān)聯(lián)規(guī)則挖掘健脾類中藥對骨關(guān)節(jié)炎患者免疫炎癥指標(biāo)的影響

【摘要】:目的應(yīng)用關(guān)聯(lián)規(guī)則和統(tǒng)計(jì)學(xué)方法分析導(dǎo)師劉健教授治療骨關(guān)節(jié)炎所用健脾類中藥對患者體內(nèi)免疫炎癥及代謝指標(biāo)的影響,為優(yōu)化處方提供依據(jù)。方法整理安徽中醫(yī)藥大學(xué)第一附屬醫(yī)院風(fēng)濕免疫科2012年6月至2015年10月骨關(guān)節(jié)炎住院患者的病歷資料。采用SPSS 22.0和SPSS Clementine 12.0軟件Aprior模塊分析健脾類中藥與炎癥免疫及代謝指標(biāo)變化的關(guān)系。結(jié)果共有1414例骨關(guān)節(jié)炎患者。對其中藥處方進(jìn)行分析,以健脾類中藥頻率最高,前5位分別為茯苓、甘草、陳皮、山藥、薏苡仁。設(shè)定最小置信度為75%,最小支持度為20%。經(jīng)Aprior模塊分析得出薏苡仁與茯苓關(guān)聯(lián)于超氧化物歧化酶(SOD)指標(biāo)上升(置信度88.06%,支持度32.05%),關(guān)聯(lián)于超敏C反應(yīng)蛋白(hs-CRP)指標(biāo)下降(置信度84.03%,支持度23.29%),健脾藥薏苡仁與活血藥雞血藤組合關(guān)聯(lián)于ESR下降(置信度84.40%,支持度20.16%)。健脾藥薏苡仁與祛風(fēng)濕藥宧薟草組合與補(bǔ)體C3、C4關(guān)聯(lián)度最高;健脾藥茯苓、甘草與川牛膝組合與IgA、IgG、IgM關(guān)聯(lián)度最高。有薏苡仁組治療后SOD明顯升高,ESR、hs-CRP、IgG、補(bǔ)體C3、補(bǔ)體C4下降(P0.05或P0.01);與無薏苡仁組治療后比較,有薏苡仁組在降低ESR、hs-CRP、補(bǔ)體C3方面優(yōu)于無薏苡仁組,且能上提SOD水平(P0.05或P0.01)。與氨基葡萄糖組治療后比較,有薏苡仁組在降低ESR、hs-CRP以及提高SOD水平上明顯優(yōu)于氨基葡萄糖組,且差異有統(tǒng)計(jì)學(xué)意義(P0.05或P0.01)。結(jié)論健脾類中藥薏苡仁、茯苓和甘草與免疫炎癥及代謝指標(biāo)關(guān)聯(lián)度較高,其中薏苡仁在降低ESR、hs-CRP以及提高SOD水平上起重要作用,其作用機(jī)制有待于進(jìn)一步研究。
[Abstract]:Objective to analyze the effect of Chinese herbal medicine for strengthening spleen in patients with osteoarthritis treated by Professor Liu Jian by using association rules and statistical methods, so as to provide the basis for optimizing the prescription. Methods the medical records of patients with osteoarthritis from June 2012 to October 2015 in the Department of Rheumatology and Immunology of the first affiliated Hospital of Anhui University of traditional Chinese Medicine were analyzed. SPSS 22.0 and SPSS Clementine 12.0 software Aprior module were used to analyze the relationship between traditional Chinese medicine for invigorating spleen and the changes of inflammatory immunity and metabolic indexes. Results there were 1414 cases of osteoarthritis. The Chinese medicine prescription was analyzed, the frequency of Chinese medicine for strengthening spleen was the highest, the top five were Poria cocos, liquorice, old peel, yam and Coix seed respectively. Set minimum confidence to 75 and minimum support to 20. The results of Aprior module analysis showed that Coix seed and Poria cocos were associated with the increase of superoxide dismutase (SOD) index (confidence degree 88.06, support 32.05%), and the decrease of hs-CRP index (confidence degree 84.03%). The degree of support was 23.29%, and the combination of Coix seed and blood activating drug was associated with the decrease of ESR (confidence 84.40, support 20.16%). The combination of Coix seed and Herba Sesbeckiae had the highest correlation degree with complement C _ 3N _ 4, and the combination of poria cocos, liquorice and Achyranthes bidentata with IgA,IgG,IgM had the highest correlation degree. In Coix seed group, SOD increased significantly, ESR,hs-CRP,IgG, complement C3 and complement C4 decreased (P0.05 or P0.01). Compared with the group without Coix seed after treatment, the Coix seed group was superior to the Coix seed group in reducing the complement C3 of ESR,hs-CRP, and could raise the level of SOD (P0.05 or P0.01). Compared with the glucosamine group, the Coix seed group was superior to the glucosamine group in decreasing ESR,hs-CRP and increasing the level of SOD, and the difference was statistically significant (P0.05 or P0.01). Conclusion Coix seed, Poria cocos and Glycyrrhiza uralensis have high correlation with immune inflammation and metabolism. Coix seed plays an important role in decreasing ESR,hs-CRP and increasing SOD level, and its mechanism needs further study.
