【摘要】：目的:觀察滋腎增水湯聯(lián)合針刺治療月經(jīng)過少的臨床療效并初步探討其機理。方法:根據(jù)入選及排除標(biāo)準(zhǔn)隨機選取2015年1月~2015年12月就診于廣西中醫(yī)藥大學(xué)第一附屬醫(yī)院婦科門診或住院部的腎陰虛型月經(jīng)過少患者60例。按隨機數(shù)字表法分治療組及對照組兩組,各組分別納入30名。治療組患者予口服中藥方并聯(lián)合針刺治療,對照組口服西藥補佳樂聯(lián)合黃體酮序貫治療,3個月經(jīng)周期后觀察兩組患者月經(jīng)量的恢復(fù)、中醫(yī)癥候的改善情況及對子宮內(nèi)膜的影響,通過統(tǒng)計學(xué)方法處理。結(jié)果:治療前通過對兩組患者臨床癥狀評分、月經(jīng)量、子宮內(nèi)膜厚度比較,兩組差異無統(tǒng)計學(xué)意義(P0.05),具有可比性。經(jīng)治療3個月經(jīng)周期后,兩組患者各項觀察指標(biāo)均較療前有所改善。其中治療組月經(jīng)情況特別是月經(jīng)色、質(zhì)、量的改善以及伴隨中醫(yī)癥候的改善明顯優(yōu)于對照組,療效具有統(tǒng)計學(xué)差異(P0.05)。兩組患者治療3個月經(jīng)周期后月經(jīng)晚期(月經(jīng)周期第21~24天)子宮內(nèi)膜厚度均增加,但組間比較無統(tǒng)計學(xué)差異(P0.05)。本研究治療組30例患者的月經(jīng)量療效好轉(zhuǎn)2例,顯效9例,治愈17例,總有效率為93.32%,對照組為好轉(zhuǎn)7例,顯效5例,治愈10例,總有效率為73.32%,治療組優(yōu)于對照組。治療組與對照組的中醫(yī)癥候總療效均有改善,治療組中醫(yī)臨床癥狀總有效率為90%,對照組為66.66%,并且治療組中醫(yī)臨床各癥狀改善率明顯優(yōu)于對照組,差異具有統(tǒng)計學(xué)差異(P0.05)。治療期間,兩組均未發(fā)生無不良反應(yīng)及并發(fā)癥。結(jié)論:滋腎增水湯聯(lián)合針刺治療腎陰虛型月經(jīng)過少臨床療效確切,可明顯增加患者的月經(jīng)量并且改善伴隨中醫(yī)臨床癥候,療效優(yōu)于雌孕激素序貫治療,且用藥安全,無明顯不良反應(yīng)。
[Abstract]:Objective: to observe the clinical effect of Zishen Zengshui decoction combined with acupuncture on menorrhagia and explore its mechanism. Methods: according to the criteria of inclusion and exclusion, 60 patients with menorrhagia due to deficiency of kidney yin were randomly selected from January 2015 to December 2015 in gynecological outpatient or inpatient department of the first affiliated Hospital of Guangxi University of traditional Chinese Medicine. According to the random digital table method, the treatment group and the control group were divided into two groups. The patients in the treatment group were treated with traditional Chinese medicine combined with acupuncture, while the patients in the control group were treated with western medicine Bujialue combined with progesterone. The menstrual volume of the two groups was observed after three menstrual cycles. The improvement of TCM symptoms and its influence on endometrium are dealt with by statistical method. Results: there was no significant difference between the two groups in clinical symptom score, menstrual volume and endometrial thickness before treatment (P0.05). After 3 menstrual cycles, the observation indexes of the two groups were improved. The treatment group especially menstrual color, quality, quantity improvement and accompanied with the improvement of TCM symptoms were significantly better than the control group, the curative effect was statistically different (P0.05). The endometrial thickness of the two groups increased after 3 menstrual cycles (21 ~ 24 days of menstrual cycle), but there was no significant difference between the two groups (P0.05). In the study group, there were 2 cases of menorrhagia improvement, 9 cases of remarkable effect, 17 cases of cure and 93.32% of total effective rate, while 7 cases of improvement, 5 cases of remarkable effect, 10 cases of cure, and 73.32% of total effective rate in the control group. The treatment group was superior to the control group. The total curative effect of TCM symptoms in the treatment group and the control group were improved. The total effective rate of TCM clinical symptoms in the treatment group was 90 and that in the control group was 66.66.The improvement rate of the TCM symptoms in the treatment group was obviously better than that in the control group. The difference was statistically significant (P0.05). During the treatment, no adverse reactions and complications occurred in both groups. Conclusion: Zishen Zengshui decoction combined with acupuncture is effective in the treatment of excessive menstruation of kidney yin deficiency type. It can obviously increase the menstrual volume of the patients and improve the clinical symptoms of TCM. The curative effect is superior to the sequential treatment of estrogen and progesterone, and the medication is safe. There was no obvious adverse reaction.
