【摘要】：目的探討基于中醫(yī)養(yǎng)生理論的辨證施護(hù)對(duì)高血壓患者血壓及中醫(yī)臨床癥候的影響,以期提高高血壓患者的生活質(zhì)量。方法選取2015年1—12月在浙江中醫(yī)藥大學(xué)附屬第二醫(yī)院心血管科和老年科住院的高血壓患者100例,采用隨機(jī)數(shù)字表法將患者分為試驗(yàn)組和對(duì)照組,各50例。對(duì)照組采用常規(guī)護(hù)理方法,試驗(yàn)組在常規(guī)護(hù)理方法基礎(chǔ)上采用基于中醫(yī)養(yǎng)生理論的辨證施護(hù),兩組均護(hù)理30 d。兩組患者在治療前及治療10、20、30 d后行24 h動(dòng)態(tài)血壓監(jiān)測(cè)(24 h平均收縮壓和24 h平均舒張壓)和中醫(yī)癥候分級(jí)量化評(píng)分,并進(jìn)行比較。結(jié)果護(hù)理方法與護(hù)理時(shí)間在24 h平均收縮壓、24 h平均舒張壓存在交互作用(P0.05);護(hù)理時(shí)間在24 h平均收縮壓、24 h平均舒張壓上主效應(yīng)顯著(P0.05);護(hù)理方法在24 h平均收縮壓、24 h平均舒張壓上主效應(yīng)顯著(P0.05)。治療20、30 d后試驗(yàn)組患者24 h平均收縮壓、24 h平均舒張壓低于對(duì)照組(P0.05)。護(hù)理方法與護(hù)理時(shí)間在中醫(yī)癥候分級(jí)量化評(píng)分上存在交互作用(P0.05);護(hù)理時(shí)間在中醫(yī)癥候分級(jí)量化評(píng)分上主效應(yīng)顯著(P0.05);護(hù)理方法在中醫(yī)癥候分級(jí)量化評(píng)分上主效應(yīng)顯著(P0.05)。治療10、20、30 d后試驗(yàn)組患者中醫(yī)癥候分級(jí)量化評(píng)分低于對(duì)照組(P0.01)。結(jié)論基于中醫(yī)養(yǎng)生理論的辨證施護(hù)可有效改善高血壓患者的血壓情況,緩解中醫(yī)臨床癥候,提高生活質(zhì)量。
[Abstract]:Objective to explore the effect of syndrome differentiation and nursing based on the theory of traditional Chinese medicine (TCM) on blood pressure and clinical symptoms of hypertension patients in order to improve the quality of life of patients with hypertension. Methods from January to December 2015, 100 patients with hypertension were selected from Cardiovascular Department and Geriatrics Department of the second affiliated Hospital of Zhejiang University of traditional Chinese Medicine. The patients were randomly divided into two groups: experimental group and control group (50 cases each). The control group was treated with routine nursing method, and the experimental group was treated with syndrome differentiation based on the theory of traditional Chinese medicine on the basis of routine nursing method. Both groups were given nursing for 30 days. 24 h ambulatory blood pressure monitoring (24 h mean systolic blood pressure and 24 h mean diastolic blood pressure) and quantitative score of TCM symptom grading were performed before and 30 days after treatment in the two groups. Results there was interaction between nursing method and nursing time in 24 h mean systolic blood pressure (24 h mean diastolic blood pressure) (P0.05), nursing time at 24 h mean systolic blood pressure (24 h), 24 h mean diastolic blood pressure (24 h) had significant main effect (P0.05). The main effect of nursing methods was significant at 24 h mean systolic blood pressure and 24 h mean diastolic blood pressure (P0.05). The mean systolic blood pressure (SBP) and diastolic blood pressure (DBP) in the experimental group were significantly lower than those in the control group (P0.05). There was interaction between nursing method and nursing time on the score of TCM symptom grading (P0.05), and the main effect of nursing time on TCM syndrome grading and quantification score was significant (P0.05). The main effect of nursing method on TCM symptom grading and quantification score was significant (P0.05). After 30 days of treatment, the scores of TCM syndromes in the experimental group were lower than those in the control group (P0.01). Conclusion Dialectic nursing based on the theory of traditional Chinese medicine can effectively improve the blood pressure of patients with hypertension, alleviate the clinical symptoms of traditional Chinese medicine, and improve the quality of life.
