【摘要】：目的:從黃祥武主任應(yīng)用當(dāng)歸四逆湯治療疑難雜癥的臨床經(jīng)驗(yàn)中探索其應(yīng)用適應(yīng)證。方法:選擇黃祥武主任以當(dāng)歸四逆湯為基礎(chǔ)方的有效醫(yī)案238例,對四診信息規(guī)范處理采用描述性分析。結(jié)果:涉及病種廣泛,如內(nèi)分泌科、婦科、皮膚科、神經(jīng)內(nèi)科、骨科、外科、心血管科等,病位主要在營、衛(wèi)及肝、脾、腎,厥陰等,病性多為陽虛、寒凝、血瘀等,主要四診信息包括舌淡+舌暗占80.3%,有瘀點(diǎn)瘀斑占29.0%,陽虛+寒凝+血瘀+營衛(wèi)不和占79.4%。總計(jì)涉及中藥128味,每例患者每次用25.7味藥,與溫陽、散寒、活血、通絡(luò)四類藥物配伍占85.1%。結(jié)論:當(dāng)歸四逆湯的應(yīng)用指征可概括為手足冰冷或偏涼,面色少華,舌質(zhì)淡暗,苔白,脈沉細(xì)緩,在臨床上要把握"陽虛寒凝血瘀"病機(jī)主要信息特征,隨證靈活加減。
[Abstract]:Objective: to explore the indications of Huang Xiangwu's application of Danggui Sini decoction in the treatment of difficult and complicated diseases. Methods: a total of 238 effective medical cases based on Danggui Sini decoction were selected, and descriptive analysis was used to deal with the information standard of four diagnoses. Results: the disease was involved in a wide range of diseases, such as Endocrinology, Gynecology, Dermatology, Neurology, Orthopaedics, surgery, Cardiovascular Department, etc. The disease was mainly located in camp, Wei, liver, spleen, kidney, Jueyin and so on. The disease was mostly due to deficiency of yang, cold coagulation, blood stasis, etc. The main information of the four diagnoses included tongue light tongue dark account for 80.3 points stasis spot accounted for 29.0 and Yang deficiency cold coagulation blood stasis Ying Wei discord accounted for 79.4%. A total of 128 kinds of Chinese medicine were involved. Each patient was treated with 25.7 flavor drugs, which was compatible with warm yang, cold dispersing, promoting blood circulation and collaterals, accounting for 85.1% of the total. Conclusion: the application indications of Danggui Sini decoction can be summed up as cold or cold hand and foot, Shaohua complexion, pale tongue, white fur and slow pulse sinking. It is necessary to grasp the main information characteristics of the pathogenesis of "Yang deficiency, cold coagulation and blood stasis" in clinic, and add and subtract flexibly with the syndrome.
