【摘要】：目的:觀察復(fù)方參鹿顆粒治療腎陽(yáng)虛型再生障礙性貧血(AA)的臨床療效和安全性,初步探討其作用機(jī)制。方法:將符合納入標(biāo)準(zhǔn)的60例患者隨機(jī)分為治療組和對(duì)照組,每組30例,療程均為6個(gè)月,隨訪1年以上。觀察兩組臨床療效、中醫(yī)證候療效、外周血象、T淋巴細(xì)胞亞群的變化及不良反應(yīng)。結(jié)果:治療組臨床總有效率高于對(duì)照組,但差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。治療組中醫(yī)證候療效優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。與治療前比較,治療組療后紅細(xì)胞、血紅蛋白、血小板計(jì)數(shù)明顯升高(P0.01),但與對(duì)照組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。與治療前比較,治療組療后CD3~+、CD4~-、CD8~+/CD3~+、CD19-細(xì)胞比例較療前明顯降低(P0.05),但與對(duì)照組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。治療組療后CD4~+、CD25~+、Fox P3~+/CD4~+、CD25~+細(xì)胞比例明顯上升,(P0.05),且與對(duì)照組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:復(fù)方參鹿顆粒治療腎陽(yáng)虛型AA療效確切、安全,其作用機(jī)制可能與減少CD3~+、CD4~-、CD8~+細(xì)胞數(shù)量,增加CD4~+、CD25~+調(diào)節(jié)性T細(xì)胞數(shù)量、提高其Foxp3的表達(dá)有關(guān)。
[Abstract]:Objective: to observe the clinical efficacy and safety of compound Shenlu granule in the treatment of (AA) with kidney-yang deficiency type aplastic anemia. Methods: 60 patients who met the inclusion criteria were randomly divided into treatment group (n = 30) and control group (n = 30). The course of treatment was 6 months and the follow-up was more than one year. The clinical efficacy, TCM syndromes, peripheral blood, T lymphocyte subsets and adverse reactions were observed. Results: the total effective rate in the treatment group was higher than that in the control group, but the difference was not statistically significant (P0.05). The therapeutic effect of TCM syndrome in the treatment group was better than that in the control group, and the difference was statistically significant (P0.05). Compared with before treatment, the red blood cell, hemoglobin, platelet count in the treatment group increased significantly (P0.01), but there was no significant difference between the treatment group and the control group (P0.05). Compared with before treatment, the proportion of CD3~, CD4~-,CD8~ / CD3~, CD19- cells in the treatment group was significantly lower than that before treatment (P0.05), but there was no significant difference between the treatment group and the control group (P0.05). The proportion of CD4~, CD25~, Fox P3 ~ / CD4~ and CD25~ cells in the treatment group increased significantly after treatment (P0.05), and the difference was statistically significant compared with the control group (P0.05). Conclusion: compound Shenlu granule is effective and safe in the treatment of AA with deficiency of kidney-yang, and its mechanism may be related to decreasing the number of CD3~ and CD4~-,CD8~ cells, increasing the number of CD4~ and CD25~ regulatory T cells and increasing the expression of Foxp3.
