【摘要】：目的歸納總結(jié)古籍文獻(xiàn)中運(yùn)用辛溫解表方藥治療咽喉疼痛的辨識(shí)及用藥經(jīng)驗(yàn)。方法以《中華醫(yī)典(光盤版)》為工具,檢索與辛溫解表方藥治療咽喉疼痛有關(guān)的古籍文獻(xiàn)原文,根據(jù)納排標(biāo)準(zhǔn)篩選出符合要求者并從中提取相關(guān)信息,經(jīng)過(guò)規(guī)范化處理后錄入WPS2013建立數(shù)據(jù)庫(kù),采用內(nèi)容分析法與描述性統(tǒng)計(jì)加以分類與歸納。結(jié)果辨識(shí)方面:采用辛溫解表方藥所治咽痛常具有以下特點(diǎn)。望診:咽喉局部色白或紫且不甚腫大,或咽喉局部色白且扁桃體明顯腫大,舌潤(rùn)。聞診:暴喑(急性聲音嘶啞)。問(wèn)診:有感受寒邪或服冷劑(對(duì)苦寒瀉下藥的統(tǒng)稱)會(huì)加重病情的病史,常兼見惡寒(伴或不伴發(fā)熱)、下利、咽喉異物感、咳嗽或口不渴等癥狀。切診:脈沉、遲/緩、弱/微、細(xì),或脈浮、弦/緊、數(shù)/疾。用藥方面:常用于治療陰證咽痛的辛溫解表方藥有桔梗湯、半夏散及湯、六味湯、荊防敗毒散及麻黃細(xì)辛附子湯等方劑,以及細(xì)辛、荊芥、蔥白、桂枝、生姜等藥物。結(jié)論咽痛辨證時(shí)應(yīng)關(guān)注患者發(fā)病的誘因、治療經(jīng)過(guò)、四診特點(diǎn),"惡寒、舌潤(rùn)、脈沉/細(xì)/弱"對(duì)辛溫解表方藥的使用有較強(qiáng)提示作用;桔梗湯、半夏散及湯等經(jīng)方為各家所喜用,細(xì)辛、蔥白等亦為利咽要藥。
[Abstract]:Objective to summarize the identification and experience of treating pharynx and larynx pain with Xin-wen prescription in ancient literature. Methods using the Chinese Medical Code (Optical disk Edition) as a tool, the original texts related to the treatment of pharynx and throat pain with Xin-Wen prescription were searched. According to the standard of Nanopai, those who met the requirements were screened out and the relevant information was extracted from them. After standardized processing, the database is set up by WPS2013, and classified and summarized by content analysis and descriptive statistics. Results Identification: the treatment of pharynx pain with Xin-wen prescription often has the following characteristics. Diagnosis: throat local color white or purple and not very swollen, or throat local color white and tonsil obvious swelling, tongue moist. Smelling: aphasia (acute hoarseness). Inquiry: feeling cold evil or taking cold medicine (general name for bitter cold diarrhea) will aggravate the history of illness, often also see bad cold (with or without fever), lower benefits, throat foreign body sensation, cough or mouth thirst and other symptoms. Diagnosis: pulse sinking, late / slow, weak / slight, thin, or floating, string / tight, number / disease. Medication: often used in the treatment of Yin-syndromes of sore throat Xin-Wen prescription drugs such as Platycodon decoction, Banxia Powder and decoction, Liu Wei Tang, Jing Fang Bai du San and ephedra Asarum aconite decoction, as well as Asarum, Schizonepeta, scallion white, cinnamon, ginger and other drugs. Conclusion when differentiating symptoms and signs of pharynx, we should pay attention to the inducement of the onset of the disease, the course of treatment, the characteristics of four diagnoses, such as "bad cold, moist tongue, deep pulse / fine / weak", which have a strong effect on the use of Xin-Wen antitabular medicine. Platycodon grandiflorum decoction, Banxia Powder and decoction are preferred by each family, Asarum, scallion, etc.
