【摘要】：目的:觀察助卵1號(hào)湯聯(lián)合外治法治療排卵障礙性不孕癥腎虛肝郁證月經(jīng)表現(xiàn)、全身癥狀表現(xiàn)、基礎(chǔ)體溫、卵泡、內(nèi)膜發(fā)育及妊娠情況,并探討其作用機(jī)理。方法:收集排卵障礙性不孕癥腎虛肝郁證患者60例,治療組30例,予助卵1號(hào)湯聯(lián)合推拿、拔罐、艾灸治療;對(duì)照組30例,予氯米芬及人絨毛膜促性腺激素治療。3個(gè)月經(jīng)周期為一個(gè)療程,連續(xù)治療1-2個(gè)療程,記錄兩組患者治療前后臨床癥狀、月經(jīng)情況、基礎(chǔ)體溫、卵泡、內(nèi)膜發(fā)育及妊娠情況。結(jié)果:治療組總有效率為86.67%,排卵率為67.32%,妊娠率56.67%,對(duì)照組總有效率為73.33%,排卵率為69.27%,妊娠率30.00%,兩組患者治療后療效比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),排卵率比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),妊娠率比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),治療組在總有效率、妊娠率方面優(yōu)于對(duì)照組。治療后兩組患者中醫(yī)證候總積分,兩組比較有顯著統(tǒng)計(jì)學(xué)意義(P0.01),兩組在治療前后的比較也有統(tǒng)計(jì)學(xué)意義(P0.01),治療組優(yōu)于對(duì)照組。治療后兩組患者月經(jīng)癥狀總積分,治療組和對(duì)照組相比有顯著統(tǒng)計(jì)學(xué)意義(P0.05),兩組在治療前后的比較有顯著統(tǒng)計(jì)學(xué)意義(P0.01),治療組優(yōu)于對(duì)照組。兩組治療后子宮內(nèi)膜厚度比較差異有顯著統(tǒng)計(jì)學(xué)意義(P0.01),治療組優(yōu)于對(duì)照組。兩組治療后最大優(yōu)勢(shì)卵泡直徑比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),兩組在治療前后的比較有顯著統(tǒng)計(jì)學(xué)意義(P0.01)。兩組治療后基礎(chǔ)體溫比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:助卵1號(hào)湯聯(lián)合推拿、拔罐、艾灸是治療排卵障礙性不孕癥腎虛肝郁證的有效方法。治療組和對(duì)照組均能通過(guò)增加排卵率和妊振率及改善臨床癥狀和月經(jīng)情況以達(dá)治療目的,在增加患者妊振率、增加子宮內(nèi)膜厚度、改善臨床癥狀及總療效方面的比較來(lái)看,治療組的綜合療效均優(yōu)于對(duì)照組。助卵1號(hào)湯聯(lián)合推拿、拔罐、艾灸在治療過(guò)程中未發(fā)現(xiàn)毒副作用及不良反應(yīng),安全可靠,值得臨床推廣應(yīng)用。
[Abstract]:Objective: to observe the menstrual manifestations, systemic symptoms, basal body temperature, follicle, intimal development and pregnancy of ovulation disorder infertility treated by ZY-1 decoction combined with external treatment, and to explore its mechanism. Methods: 60 patients with kidney deficiency and liver depression were collected and 30 patients in treatment group were treated with ZY-1 decoction combined with massage, cupping and moxibustion. The control group (30 cases) was treated with clomiphene and human chorionic gonadotropin (HCG). Endometrial development and pregnancy. Results: the total effective rate, ovulation rate and pregnancy rate of the treatment group were 86.67 and 67.32, respectively. The total effective rate of the control group was 73.33, the ovulation rate was 69.27, and the pregnancy rate was 30.00. Two groups of patients after treatment were statistically significant difference in curative effect (P0.05), ovulation rate difference was not statistically significant (P0.05), pregnancy rate difference was statistically significant (P0.05), treatment group in the total effective rate, The pregnancy rate was better than that of the control group. After treatment, the total score of TCM syndromes in the two groups was statistically significant (P0.01), the comparison between the two groups before and after treatment was also statistically significant (P0.01), the treatment group was better than the control group. After treatment, the total score of menstrual symptoms in the two groups, treatment group and control group compared with significant statistical significance (P0.05), the two groups before and after treatment has significant statistical significance (P0.01), the treatment group is better than the control group. There was significant difference in endometrial thickness between the two groups after treatment (P0.01), the treatment group was superior to the control group. There was no significant difference in the diameter of the largest dominant follicle between the two groups after treatment (P0.05), and there was significant difference between the two groups before and after treatment (P0.01). There was no significant difference in basic body temperature between the two groups after treatment (P0.05). Conclusion: ZY-1 decoction combined with massage, cupping and moxibustion is an effective method to treat the syndrome of kidney deficiency and liver depression. Both the treatment group and the control group could increase ovulation rate and pregnancy induced vibration rate, and improve the clinical symptoms and menstrual conditions in order to achieve the purpose of the treatment, in increasing the pregnancy induced vibration rate, increasing the endometrial thickness, improving the clinical symptoms and overall curative effect. The comprehensive curative effect of the treatment group was better than that of the control group. No toxic side effects and adverse reactions were found in the treatment of ZY-1 decoction combined with massage, cupping and moxibustion. It is safe and reliable and worthy of clinical application.
