【摘要】：目的:本課題通過研究水液代謝理論,觀察水液代謝和原發(fā)性干燥綜合征之間的關系,探討本病的病名、病因病機及治療方法,為臨床治療氣陰兩虛型干燥綜合征提供思路和方法。方法:主要通過兩個部分進行論述,第一部分,通過檢索國內外文獻,對干燥綜合征的病名、病因病機進行探討,基于水液代謝理論擬定生津潤燥方;第二部分,研究50例符合氣陰兩虛型干燥綜合征患者,隨機分為兩組,每組25例,其中治療組為中藥生津潤燥方聯(lián)合硫酸羥氯喹片、白芍總苷膠囊治療,對照組為硫酸羥氯喹片和白芍總苷膠囊治療,療程為12周,分別記錄患者治療前與治療后的ESSDAI評分、中醫(yī)證候積分、實驗室指標變化等,結合治療數(shù)據(jù)進行療效評價。結果:通過對研究數(shù)據(jù)的總結,治療組總有效率為72%,對照組總有效率為60%,并且治療組對中醫(yī)諸癥均有顯著改善,對眼干、口干、納呆、手足心熱、乏力、關節(jié)痛的改善,治療前后有明顯差異(P0.05),而對皮膚干、大便干、關節(jié)腫脹的改善,兩組比較治療前后無明顯差異(P0.05);兩組ESR、IgG、RF均有明顯改善,有統(tǒng)計學意義(P0.05);治療組僅發(fā)現(xiàn)一例SSB改善,而兩組ANA定量、SSA變化皆不明顯。結論:本次臨床研究證實,通過對水液代謝理論的研究,在此基礎上自擬方劑生津潤燥方,采用清燥生津、益氣養(yǎng)陰之法,可有效改善患者眼干、口干癥狀,同時改善其氣陰兩虛的相關癥狀,降低炎性指標,改善的生活質量,研究結果提示生津潤燥方有抗炎和調節(jié)免疫的作用,具有一定的臨床意義。
[Abstract]:Objective: to investigate the relationship between water and liquid metabolism and primary Sjogren's syndrome by studying the theory of water and liquid metabolism, and to explore the name, etiology, pathogenesis and treatment of the disease. To provide ideas and methods for clinical treatment of Sjogren syndrome with deficiency of Qi and Yin. Methods: mainly through two parts to discuss, the first part, through the retrieval domestic and foreign literature, carries on the discussion to the Sjogren syndrome disease name, the etiology and the pathogenesis, based on the water liquid metabolism theory formulates the Shengjin Runzao recipe; In the second part, 50 cases of Sjogren syndrome with deficiency of qi and yin were randomly divided into two groups, 25 cases in each group. The treatment group was treated with Shengjin Runzao prescription combined with hydroxychloroquine sulfate tablet and total paeony glucoside capsule. The control group was treated with hydroxychloroquine sulfate and total glucoside of paeony for 12 weeks. The ESSDAI scores, TCM syndromes and laboratory indexes were recorded before and after treatment, respectively. Results: the total effective rate of the treatment group was 72.The total effective rate of the control group was 60, and the treatment group had significant improvement on the symptoms of traditional Chinese medicine, including dry eyes, dry mouth, dry eyes, hot palms and feet, and fatigue. The improvement of arthralgia was significantly different before and after treatment (P0.05), but there was no significant difference between the two groups before and after treatment (P0.05). There was significant improvement in ESR,IgG,RF in both groups (P0.05); in the treatment group, only one case of SSB was improved, while the changes of ANA quantitative and SSA were not obvious in both groups. Conclusion: this clinical study confirmed that through the study of the theory of water and liquid metabolism, the prescription of Shengjin Runzao, a self-made prescription, can effectively improve the symptoms of dry eyes and dry mouth by using the method of clearing dryness and nourishing qi and nourishing yin. At the same time, it can improve the symptoms of deficiency of Qi and Yin, reduce the inflammatory index, improve the quality of life. The research results suggest that Shengjin Runzao prescription has the function of anti-inflammation and regulating immunity, which has certain clinical significance.
