【摘要】：目的太陰病是《傷寒論》六經(jīng)辨證之中的重要部分,古往今來,許多醫(yī)家對太陰病問題從臟腑經(jīng)絡(luò)、氣化、現(xiàn)代研究等多角度做了大量相關(guān)討論,對理論和臨床作出了重要貢獻(xiàn)。在這些討論中,大多數(shù)的內(nèi)容是單純圍繞太陰病的理論來源、病機(jī)認(rèn)識、分型證治展開的,是對太陰病理論進(jìn)行縱向深入的認(rèn)識。筆者在理論學(xué)習(xí)與跟師臨證的過程中意識到太陰病涉及病種廣泛,又極易在六經(jīng)病中夾雜為病,而僅從其本證研究,可能尚顯不足,故擬從太陰病與六經(jīng)病的關(guān)系入手,對太陰病與六經(jīng)的關(guān)系及其是如何在六經(jīng)病的發(fā)生與傳變中起作用的,進(jìn)行更深入、完善的討論,幫助認(rèn)識太陰病在六經(jīng)中的意義。方法傷寒六經(jīng)是以陰陽五行、臟腑經(jīng)絡(luò)氣血學(xué)說為指導(dǎo),用陰陽分類法對人體功能活動所做的分類概括,是認(rèn)識疾病的需要。首先對單純太陰病的研究結(jié)果進(jìn)行縱向挖掘,學(xué)習(xí)各醫(yī)家對六經(jīng)病相關(guān)問題的分別闡述,以太陰病為認(rèn)識的重心,分析其與太陽、陽明、少陽、少陰、厥陰病的之間的聯(lián)系,明確其在六經(jīng)病生理、發(fā)病、證治中的特點(diǎn),以前輩醫(yī)家的臨床病案加以說明,并歸納、總結(jié)出研究太陰病與六經(jīng)病關(guān)系的意義。結(jié)果通過對太陰病與其他五經(jīng)病的生理、病理關(guān)系的討論,分析出太陰病與其他五經(jīng)病的證治聯(lián)系,在三陽病出現(xiàn)虛損的過程中會首先引起太陰的變化,三陰病中均兼太陰病。結(jié)論1、三陽病病理演變的選擇與太陰所處的狀態(tài)有極其密切的關(guān)系：在太陽病中,太陰之氣是太陽營衛(wèi)之氣的重要支撐,對太陽病的發(fā)病過程及預(yù)后有重要影響；陽明病中病邪入里,陽氣奮起抗邪,正邪斗爭劇烈,由太陰與陽明之氣相互配合,保證營衛(wèi)氣血在中焦脾胃的升降運(yùn)動中相互轉(zhuǎn)化、進(jìn)而敷布全身；少陽病中正氣已顯疲憊,需通過太陰外為疏解、內(nèi)降陽明調(diào)整整體狀態(tài)；2、三陰病中人體正氣已不足,呈現(xiàn)正不勝邪,邪氣內(nèi)入,太陰受邪首當(dāng)其沖,反應(yīng)人體虛損的初期階段；正氣繼續(xù)衰減,將發(fā)展至此心腎機(jī)能衰憊,呈現(xiàn)人體根本元?dú)獠蛔愕纳訇幉‰A段：若在太陰基礎(chǔ)上陽氣虛損進(jìn)一步加重,厥陰氣化失調(diào),肝失疏泄,少火轉(zhuǎn)出不利無以溫煦太陰反生邪熱：故太陰病是三陰病的開始,三陰病中必含太陰病,太陰病為諸虛之本,在治療上三陰病辨治應(yīng)以太陰為基礎(chǔ)。
[Abstract]:Objective Taiyin disease is an important part of the syndrome differentiation of the six meridians. From ancient to modern times, many doctors have done a lot of relevant discussions on the problem of Taiyin disease from the aspects of the viscera and meridian, gasification, modern research, etc. Has made the important contribution to the theory and the clinical. In these discussions, most of the content is simply around the theoretical source of Taiyin disease, the pathogenesis of the disease, the classification of the spread of syndrome and treatment, is to the theory of Taiyin disease longitudinal in-depth understanding. In the course of theoretical study and clinical diagnosis, the author realized that Taiyin disease is involved in a wide range of diseases, and is easy to be mixed in six meridians diseases. However, the study of Taiyin disease and six meridian diseases may not be sufficient only from the study of this syndrome, so I intend to start with the relationship between Taiyin disease and six meridian diseases. The relationship between Taiyin disease and six meridians and how to play a role in the occurrence and transmission of six meridians disease are discussed in order to help understand the significance of Taiyin disease in the six meridians. Methods based on the theory of yin and yang five elements and zang-fu meridian qi and blood the classification and generalization of human functional activities by the method of yin and yang is the need to recognize diseases. First of all, the research results of pure Taiyin disease were excavated longitudinally, and the doctors were studying the related problems of Six-meridian disease, and taking Taiyin disease as the focus of understanding, analyzed its relationship with the Sun, Yangming, Shaoyang, Shaoyin, Jueyin disease, and analyzed the relationship between it and Sun, Yangming, Shaoyang, Shaoyin and Jueyin. The characteristics of its physiology, pathogenesis, syndrome and treatment in six meridian diseases were clarified, and the clinical records of the previous physicians were explained, and the significance of studying the relationship between Taiyin disease and six meridian diseases was summarized. Results by discussing the physiological and pathological relationship between Taiyin disease and other five meridian diseases, the relationship between Taiyin disease and other five meridian diseases was analyzed. Conclusion 1. The choice of pathological evolution of Sanyang disease is closely related to the state of Taiyin: in the sun disease, Taiyin Qi is the important support of the Sun Ying-Wei Qi, which has an important influence on the pathogenesis and prognosis of the Sun disease; Yang Ming disease in the disease into the evil, Yang Qi rose up to resist evil, fierce struggle, from Taiyin and Yang Ming Qi to cooperate with each other, to ensure that Ying Wei Qi and blood in the rising and falling movement of the spleen and stomach, and then spread the whole body; Shaoyang disease in the Zhengqi has been tired, need to pass through the Taiyin for the dredging, internal Yangming adjustment of the overall state; (2) the deficiency of human body's vital qi in the disease of three yin, showing the inexorable evil and evil, the first to bear the brunt of the evil in Taiyin, and the initial stage of reflecting the deficiency of the human body; The decaying of the positive qi will develop to the point where the heart and kidney function is exhausted, presenting the stage of Shaoyin disease with the deficiency of the basic vitality of the human body: if the deficiency of yang qi is further aggravated on the basis of Taiyin, the Gasification of Jueyin is disordered, and the liver is lost and dissipated, So Taiyin disease is the beginning of Sanyin disease, which must contain Taiyin disease, Taiyin disease is the foundation of deficiency, in the treatment of Sanyin disease, it should be based on Taiyin.
