【摘要】：目的：通過對(duì)治療前后患者的精液分析及陰囊超聲來證實(shí)補(bǔ)腎活血法結(jié)合睪丸冷敷貼治療左側(cè)精索血管曲張型不育癥的臨床療效。方法：采用隨機(jī)、對(duì)照法,分別從成都中醫(yī)藥大學(xué)附屬醫(yī)院及第二附屬醫(yī)院收取72例腎虛血瘀型左側(cè)精索靜脈曲張型不育癥男性患者,分為兩組,每組36例,行基礎(chǔ)治療(服用補(bǔ)腎活血法治則方劑)的同時(shí),對(duì)照組行手術(shù)治療(固定手術(shù)醫(yī)生,腹腔鏡下左側(cè)精索靜脈高位結(jié)扎治療)；實(shí)驗(yàn)組給予實(shí)驗(yàn)方法(睪丸冷敷貼外敷陰囊)進(jìn)行1個(gè)療程(3個(gè)月)的治療,治療前、結(jié)束時(shí)及結(jié)束后隨訪3個(gè)月,觀察比較各組患者精液常規(guī)分析與陰囊超聲及中醫(yī)證候評(píng)分。實(shí)驗(yàn)數(shù)據(jù)采集后,運(yùn)用SPSS22.0進(jìn)行分析。結(jié)果：實(shí)驗(yàn)組顯效率38.89%、有效率27.78%、無效率33.33%,對(duì)照組顯效率27.78%、有效率25.00%、無效率47.22%,其P0.05,表明差異具統(tǒng)計(jì)學(xué)意義。在中醫(yī)證候療效方面,實(shí)驗(yàn)組顯效率58.33%、有效率25.00%、無效率16.77%,對(duì)照組顯效率27.78%、有效率36.11%、無效率36.11%。兩組分級(jí)療效相比較,P0.05,其差異具有統(tǒng)計(jì)學(xué)意義。兩組在分級(jí)證候療效、總有效率上的差異性具統(tǒng)計(jì)學(xué)意義。實(shí)驗(yàn)組在精液常規(guī)分析方面療效優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)；治療后兩組在精索靜脈最大內(nèi)徑改善例數(shù)之間的差異不具統(tǒng)計(jì)學(xué)意義。結(jié)論：補(bǔ)腎活血法聯(lián)合睪丸冷敷貼對(duì)腎虛血瘀型左側(cè)精索靜脈曲張型不育癥的對(duì)精子質(zhì)量有顯著提高作用,可明顯改善患者的臨床自覺癥狀,其總體治療效安全有效,但對(duì)精索靜脈的最大內(nèi)徑的改善無明顯作用效果。
[Abstract]:Objective: to confirm the clinical effect of tonifying kidney and activating blood method combined with cold compress of testis on left spermatic varicose type infertility by semen analysis and scrotal ultrasound before and after treatment. Methods: 72 infertile male patients with varicocele type of kidney deficiency and blood stasis type were collected from affiliated Hospital and second affiliated Hospital of Chengdu University of traditional Chinese Medicine at random and were divided into two groups: 36 cases in each group. At the same time, the control group was treated with surgery (fixed surgeon, laparoscopic high ligation of the left spermatic vein), while taking the prescription of tonifying kidney and activating blood circulation, the patients in the control group were treated with high ligation of the left spermatic vein. The experimental group was given a course of treatment (3 months), followed up for 3 months before, at the end and after the treatment. To observe and compare the semen routine analysis, scrotal ultrasound and TCM syndrome score. The experimental data were collected and analyzed by SPSS22.0. Results: the effective rate was 38.89%, the effective rate was 27.78%, the non-effective rate was 33.33%, the control group was 27.78%, the effective rate was 25.00 and the non-effective rate was 47.22 (P0.05), the difference was statistically significant. As for the curative effect of TCM syndrome, the effective rate of the experimental group was 58.33, the effective rate was 25.00, the inefficiency was 16.77m, the effective rate of the control group was 27.78, the effective rate was 36.11, and the non-effective rate was 36.11. The difference between the two groups was statistically significant (P 0.05). The difference of the curative effect and total effective rate between the two groups was statistically significant. The effect of routine semen analysis in the experimental group was better than that in the control group, the difference was statistically significant (P0.05); after treatment, there was no significant difference between the two groups in the improvement of the maximum internal diameter of spermatic vein. Conclusion: the method of tonifying kidney and activating blood circulation combined with cold compress of testis can significantly improve the quality of spermatozoa in infertility of left varicocele type of kidney deficiency and blood stasis type, can obviously improve the clinical conscious symptoms of the patients, and its overall curative effect is safe and effective. However, there was no significant effect on the improvement of the maximum diameter of spermatic vein.
