【摘要】：目的:探究二烏消痹湯治療寒濕痹阻型膝骨性關(guān)節(jié)炎的療效和優(yōu)勢(shì),并總結(jié)該治法的特點(diǎn)、優(yōu)點(diǎn),分析其理論依據(jù),為臨床中探索一種療效確切、安全性高的治療措施提供科學(xué)依據(jù),挖掘中醫(yī)藥治療KOA的價(jià)值。方法:收集20 16年3月至2017年1月黑龍江中醫(yī)藥大學(xué)附屬一院診斷為寒濕痹阻型膝骨性關(guān)節(jié)炎的門診患者62例,按就診序號(hào)使用隨機(jī)數(shù)字表法分為治療組、對(duì)照組各31例。治療組服用二烏消痹湯,療程30天;對(duì)照組服用金烏骨通膠囊,療程30天。通過觀察兩組的一般情況,兩組治療前后在VAS評(píng)分、Lequene-Mery膝關(guān)節(jié)骨性關(guān)節(jié)炎評(píng)分、中醫(yī)證候評(píng)分的變化,對(duì)兩組臨床數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析,同時(shí)對(duì)相關(guān)安全性指標(biāo)及不良反應(yīng)進(jìn)行觀察記錄。結(jié)果:1.兩組各項(xiàng)觀察指標(biāo)在治療前后組內(nèi)比較均具有顯著統(tǒng)計(jì)學(xué)差異(P0.01)。2.兩組治療后在VAS評(píng)分、休息痛、運(yùn)動(dòng)痛、腫脹、晨僵、行走能力及中醫(yī)癥候的膝關(guān)節(jié)疼痛、形寒肢重、腫脹、屈伸活動(dòng)的分值組間比較具有統(tǒng)計(jì)學(xué)意義(P0.05)。3.治療組和對(duì)照組在臨床療效總有效率分別為86.67%、74.19%,中醫(yī)證候療效總有效率分別為86.67%、77.42%。兩組在臨床療效判定結(jié)果及中醫(yī)癥候療效判定結(jié)果上比較均有顯著統(tǒng)計(jì)學(xué)意義(P0.01),治療組總療效優(yōu)于對(duì)照組。4.兩組各項(xiàng)常規(guī)檢查,均無(wú)異常變化情況;治療組出現(xiàn)輕度不良反應(yīng)2例,不良率為6.67%;對(duì)照組出現(xiàn)1例,不良率為3.23%;均自行緩解,未做特殊處置。結(jié)論:1.二烏消痹湯在止痛、消腫、改善晨僵、改善膝關(guān)節(jié)活動(dòng)及形寒肢重方面療效優(yōu)于金烏骨通膠囊。2.二烏消痹湯具有有效率高、療效顯著、不良率低的特點(diǎn),具有較高的安全性,是治療寒濕痹阻型KOA的有效方法,值得進(jìn)一步研究及推廣。
[Abstract]:Objective: to explore the curative effect and advantage of Erwuxiaobi decoction in the treatment of knee osteoarthritis with cold dampness and obstruction, summarize the characteristics and advantages of the treatment, analyze its theoretical basis, and explore a kind of effective effect in clinic. The safe treatment provides the scientific basis and excavates the value of traditional Chinese medicine in the treatment of KOA. Methods: from March 2016 to January 2017, 62 outpatients with cold and dampness arthralgia type osteoarthritis of knee were collected and divided into treatment group (n = 31) and control group (n = 31). The treatment group took Erwu Xiaobi decoction for 30 days while the control group took Jinwu Gutong capsule for 30 days. By observing the general situation of the two groups, the changes of VAS score, Lequene-Mery knee osteoarthritis score and TCM syndrome score before and after treatment, the clinical data of the two groups were statistically analyzed. At the same time, the safety indexes and adverse reactions were observed and recorded. The result is 1: 1. There were significant differences between the two groups before and after treatment (P0.01). After treatment, the scores of VAS score, rest pain, motor pain, swelling, morning stiffness, walking ability and TCM symptoms of knee joint pain, cold limb weight, swelling, flexion and extension activity in the two groups were statistically significant (P0.05). The total effective rates of the treatment group and the control group were 86.67 and 74.19 respectively, and the total effective rates of TCM syndrome were 86.67 and 77.42, respectively. There was significant difference between the two groups in clinical curative effect and TCM symptom (P0.01). The total curative effect of the treatment group was better than that of the control group. 4. There were 2 cases of mild adverse reactions in the treatment group and 1 case in the control group, with a rate of 3.23%. Conclusion: 1. Erwuxiaobi decoction is better than Jinwu Gutong capsule in relieving pain, detumescence, improving morning stiffness, improving knee joint movement and weight of cold limb. 2. Erwuxiaobi decoction has the characteristics of high effective rate, remarkable curative effect, low bad rate and high safety. It is an effective method for treating cold and dampness obstruction type KOA, which is worthy of further study and popularization.
