【摘要】：目的:本研究采用止帶方加味治療濕熱下注型細(xì)菌性陰道病(bacterial vaginosis,BV),通過觀察治療后臨床癥狀及陰道分泌物實(shí)驗(yàn)室指標(biāo)變化,從而評(píng)價(jià)止帶方加味治療濕熱下注型細(xì)菌性陰道病的臨床療效。方法:將臨床診斷為濕熱下注型BV的66例患者,隨機(jī)分為治療組及對(duì)照組各33例。在研究過程中對(duì)照組有1例患者因未按時(shí)口服藥物而脫落。治療組予以口服止帶方加味及甲硝唑片,對(duì)照組予以口服甲硝唑片。治療1周,停藥1周后復(fù)查患者陰道分泌物,重新評(píng)估患者中醫(yī)癥狀及婦科檢查局部體征,痊愈及顯效者隨訪2個(gè)月經(jīng)周期。結(jié)果:1.兩組綜合療效比較,治療組痊愈率高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05),兩組總療效相比差異無統(tǒng)計(jì)學(xué)意義(P0.05)。2.治療組可明顯改善患者中醫(yī)癥狀,與治療前比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),與對(duì)照組相比優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3.治療組可明顯改善患者陰道分泌物"清潔度"、"線索細(xì)胞"、"胺味試驗(yàn)"、"PH值",與治療前比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療組在改善陰道分泌物"清潔度"方面優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組在改善其他三項(xiàng)方面比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。4.治療組可明顯改善患者婦科檢查局部體征,與治療前比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),與對(duì)照組比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。5.在隨訪過程中發(fā)現(xiàn),治療組在恢復(fù)陰道乳酸桿菌及降低復(fù)發(fā)率方面優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。6.治療組治療前后安全性指標(biāo)均正常,無明顯不良反應(yīng)出現(xiàn)。結(jié)論:1.止帶方加味聯(lián)合甲硝唑片可改善患者臨床癥狀體征,改善患者陰道分泌物實(shí)驗(yàn)室檢查各項(xiàng)指標(biāo)。2.止帶方加味聯(lián)合甲硝唑片,在改善患者中醫(yī)癥候、改善陰道分泌物清潔度、恢復(fù)陰道乳酸桿菌及降低復(fù)發(fā)率方面優(yōu)于甲硝唑組。3.止帶方加味聯(lián)合甲硝唑片治療濕熱下注型BV療效確切,無明顯不良反應(yīng)。
[Abstract]:Objective: to observe the changes of clinical symptoms and laboratory indexes of vaginal secretions after treatment of bacterial vaginosis (bacterial vaginosis,BV) with Zhibanfang decoction. So as to evaluate the clinical efficacy of Zhibanfang Jiawei in the treatment of bacterial vaginosis caused by dampness and heat. Methods: 66 patients with BV were randomly divided into treatment group (n = 33) and control group (n = 33). In the course of the study, one patient in the control group dropped off due to late oral administration. The treatment group was treated with Zhibai prescription and metronidazole tablets, and the control group with metronidazole tablets. After 1 week of treatment and 1 week of withdrawal, the vaginal secretions of the patients were reexamined, the symptoms of TCM and local signs of gynecological examination were re-evaluated, and the patients were followed up for 2 menstrual cycles. The result is 1: 1. Compared with the two groups, the cure rate of the treatment group was higher than that of the control group, the difference was statistically significant (P0.05), the total curative effect of the two groups had no statistical significance compared with the control group (P0.05). Treatment group can significantly improve the symptoms of traditional Chinese medicine patients, compared with the treatment before the difference was statistically significant (P0.05), compared with the control group was better than the control group, the difference was statistically significant (P0.05). The treatment group can obviously improve the vaginal secretion of patients "cleanliness", "clue cells", "amines test", "PH value", compared with the treatment before the difference was statistically significant (P0.05). The treatment group in improving vaginal discharge "cleanliness" than the control group, the difference was statistically significant (P0.05). There was no significant difference between the two groups in improving the other three items (P0.05). The treatment group can obviously improve the local signs of gynecological examination of patients, compared with the treatment before the difference was statistically significant (P0.05), compared with the control group, the difference was not statistically significant (P0.05). During the follow-up, the treatment group was superior to the control group in recovering Lactobacillus vaginalis and reducing the recurrence rate, the difference was statistically significant (P0.05). The safety indexes of the treatment group were normal before and after treatment, and no obvious adverse reactions occurred. Conclusion: 1. Zhibanfang plus metronidazole tablets can improve the clinical symptoms and signs of patients, improve the vaginal discharge laboratory examination indicators. 2. Zhibanfang plus metronidazole tablet is superior to metronidazole group in improving TCM symptoms, improving vaginal discharge cleanliness, restoring lactobacillus vaginalis and reducing recurrence rate. Zhibanfang plus metronidazole tablet was effective in the treatment of BV with dampness and heat.
