【摘要】：目的:觀察痤瘡合劑對(duì)尋常痤瘡(肺經(jīng)風(fēng)熱型)的療效,并通過(guò)檢測(cè)治療前后皮膚油脂含量的變化,探討藥物的作用機(jī)理。觀察可能出現(xiàn)的不良反應(yīng),評(píng)價(jià)其安全性,為本病的治療提供依據(jù)。方法:將尋常痤瘡(肺經(jīng)風(fēng)熱型)患者69例,隨機(jī)分為治療組和對(duì)照組。實(shí)驗(yàn)組35例,予痤瘡合劑內(nèi)服;對(duì)照組34例,予復(fù)方珍珠暗瘡膠囊內(nèi)服;兩組同時(shí)外用夫西地酸乳膏;1個(gè)療程共有4周。治療結(jié)束后統(tǒng)計(jì)兩組患者的臨床癥狀積分、復(fù)發(fā)情況、不良反應(yīng)和治療前后皮膚油脂含量的變化。結(jié)果:1.治療組35例,總有效率為94.29%,其中痊愈4例,顯效11例,有效18例,無(wú)效2例;對(duì)照組34例,總有效率為76.47%,其中痊愈1例,顯效4例,有效21例,無(wú)效8例。治療前后兩組皮損、中醫(yī)證候、綜合療效的積分有顯著性差異(P0.01),說(shuō)明在臨床療效方面,兩組均效果顯著。兩組皮損積分、綜合療效積分在療程結(jié)束后比較P0.05,有顯著性差異,說(shuō)明在改善患者皮損、綜合療效方面治療組優(yōu)于對(duì)照組;中醫(yī)證候積分比較P0.05,說(shuō)明痤瘡合劑在改善全身癥狀方面未顯出優(yōu)勢(shì)。2.治療組治療前平均油脂含量為163.44±42.41μg/cm2,對(duì)照組治療前平均油脂含量為161.08±36.74μg/cm2,兩組皮膚油脂含量在療程前后比較P0.01,有顯著性差異,說(shuō)明兩組在減少皮膚油脂含量方面都有顯著效果。兩組皮膚油脂含量在療程結(jié)束后比較P0.05,有顯著性差異,說(shuō)明痤瘡合劑在改善患者皮膚油脂含量方面優(yōu)于對(duì)照組。3.安全性結(jié)果:對(duì)照組治療期間出現(xiàn)輕度胃腸道反應(yīng)1例,停藥后癥狀消失。治療前后檢查患者生命體征、肝功能及腎功能檢查均未見(jiàn)異常。4.停藥隨訪4周,未發(fā)現(xiàn)復(fù)發(fā)病例。結(jié)論:痤瘡合劑治療肺經(jīng)風(fēng)熱型尋常痤瘡療效明確,不良反應(yīng)少,復(fù)發(fā)率低。
[Abstract]:Objective: to observe the curative effect of acne mixture on acne vulgaris (lung meridian wind-heat type), and to explore the mechanism of action by detecting the changes of skin oil content before and after treatment. To observe the possible adverse reactions and evaluate the safety of the disease to provide a basis for the treatment of the disease. Methods: 69 patients with acne vulgaris were randomly divided into treatment group and control group. 35 cases in the experimental group were given acne mixture intravenously, 34 cases in the control group were given the compound pearl dark ulcer capsule, and the two groups were treated externally with Fusidi yoghurt ointment. The course of treatment was 4 weeks. After the treatment, the clinical symptom scores, relapse, adverse reactions and the changes of skin fat content before and after treatment were counted. The result is 1: 1. In the treatment group, the total effective rate was 94.29, in which 4 cases were cured, 11 cases were effective, 18 cases were effective, and 2 cases were ineffective, while in the control group, the total effective rate was 76.477.The total effective rate was 76.477.In which 1 case was cured, 4 cases were markedly effective, 21 cases were effective, and 8 cases were ineffective. Before and after treatment, there were significant differences in the scores of skin lesions, TCM syndromes and comprehensive curative effects between the two groups (P0.01), which indicated that the two groups had a significant effect in terms of clinical efficacy. There was significant difference in the score of skin lesion between the two groups after the end of the course of treatment compared with P0.05, which indicated that the treatment group was superior to the control group in improving the skin lesions of the patients. The comparison of TCM syndromes score (P0.05) shows that acne mixture has no advantage in improving systemic symptoms. 2. The average oil content before treatment was 163.44 鹵42.41 渭 g / cm ~ 2 in the treatment group and 161.08 鹵36.74 渭 g / cm ~ 2 in the control group. The results showed that both groups had significant effects in reducing skin fat content. There was significant difference in the content of skin oil between the two groups after the end of the course of treatment, indicating that acne mixture is superior to the control group in improving the content of skin oil. Safety results: 1 case of mild gastrointestinal reaction occurred during treatment in the control group, and the symptoms disappeared after withdrawal. Before and after treatment, the vital signs, liver function and renal function were not abnormal. 4. 4. No recurrence cases were found after 4 weeks follow up. Conclusion: acne mixture has definite curative effect, less adverse reaction and low recurrence rate in the treatment of acne vulgaris.
