消痛散外敷治療癌性疼痛臨床療效研究

【摘要】：目的:通過對臨床病例的觀察,總結(jié)消痛散外敷治療癌性疼痛的效果,以及其對患者生活質(zhì)量的影響,用藥安全性問題,綜合分析中藥外敷法對癌性疼痛的治療價值。材料與方法:統(tǒng)計了2015年10月到2016年10月于大連市中醫(yī)醫(yī)院內(nèi)四病房住院治療,符合納入標(biāo)準(zhǔn)的患者,共計42例。采用隨機(jī)對照法進(jìn)行臨床研究,將42例病例隨機(jī)分配到對照組21例,治療組21例。治療組與對照組兩組同時使用中藥(口服湯藥、靜脈中藥)及止痛藥。治療組在此基礎(chǔ)上加用消痛散外敷,7天為一療程,觀察記錄治療前后的疼痛緩解程度,生活質(zhì)量評分、用藥安全性等相關(guān)數(shù)據(jù),對所獲得數(shù)據(jù)運(yùn)用統(tǒng)計學(xué)軟件進(jìn)行統(tǒng)計分析,得出統(tǒng)計結(jié)果。結(jié)果:本研究納入患者42例,剔除2例,實(shí)際入組40例,治療組與對照組各20例。治療組與對照組相比較,不同疼痛程度受試者治療后疼痛緩解率比較,治療組與對照組總有效率分別為85%、70%,經(jīng)統(tǒng)計學(xué)分析,說明兩組間差異有統(tǒng)計學(xué)意義。不同中醫(yī)證型受試者治療后疼痛緩解率有明顯差異。兩組受試者治療后生活質(zhì)量積分總有效率比較,治療組與對照組總有效率分別為90%、65%,經(jīng)統(tǒng)計學(xué)分析,說明兩組間差異有統(tǒng)計學(xué)意義。治療組與對照組治療前后血常規(guī)、肝功、腎功變化均不明顯,P0.05,說明差異無統(tǒng)計學(xué)意義。結(jié)論:1.消痛散對不同疼痛程度的癌性疼痛均有療效,輕度疼痛可單獨(dú)使用消痛散止痛。2.消痛散對氣滯血瘀型、痰濕蘊(yùn)結(jié)型的癌性疼痛療效顯著,對陽虛寒凝型癌性疼痛療效欠佳,考慮與該證型樣本量少有關(guān)。3.消痛散能提高患者生活質(zhì)量。4.消痛散不良反應(yīng)少,未見明顯毒副作用,安全性高。
[Abstract]:Objective: to summarize the effect of external application of Xiaotong Powder on the quality of life of patients with cancerous pain, the safety of drug use, and to analyze the value of external application of traditional Chinese medicine in the treatment of cancer pain through the observation of clinical cases. Materials and methods: from October 2015 to October 2016, 42 patients were admitted in four wards of Dalian traditional Chinese Medicine Hospital. 42 cases were randomly assigned to control group (21 cases) and treatment group (21 cases). The treatment group and the control group were treated with traditional Chinese medicine (oral decoction, intravenous Chinese medicine) and analgesic simultaneously. On this basis, the treatment group was treated with external application of Xiaotong Powder for a course of 7 days. The degree of pain relief, quality of life score, drug safety and other related data before and after treatment were observed and recorded, and the obtained data were statistically analyzed by statistical software. Get the statistical results. Results: 42 cases were included in the study, 2 cases were excluded, 40 cases were actually entered into the group, 20 cases in the treatment group and 20 cases in the control group. Compared with the control group, the total effective rate of the treatment group and the control group was 8570. The statistical analysis showed that the difference between the two groups was statistically significant. There were significant differences in pain relief rate between subjects of different TCM syndromes after treatment. The total effective rate of QOL in the two groups after treatment was 90 and 65, respectively. The statistical analysis showed that the difference between the two groups was statistically significant. The changes of blood routine, liver function and renal function before and after treatment were not obvious in the treatment group and the control group (P 0.05), indicating that there was no significant difference in blood routine test, liver function and renal function before and after treatment. Conclusion: 1. Xiaotong Powder has curative effect on cancer pain of different pain degree, mild pain can be used alone to relieve pain. 2. The curative effect of Xiaotong Powder on cancerous pain of qi stagnation and blood stasis type, phlegm dampness accumulation knot type was remarkable, and the curative effect on Yang deficiency cold coagulation type cancer pain was not good, which was related to the small sample size of this syndrome type. 3. Xiaotong Powder can improve the quality of life of patients. 4. Xiaotongsan has few adverse reactions, no obvious side effects and high safety.
