【摘要】：研究目的：本研究旨在以慢性腹瀉虛寒證患者為研究對象,探討導(dǎo)師運用中醫(yī)經(jīng)驗方配合足浴治療慢性腹瀉(虛寒證)的經(jīng)驗及其臨床療效,為進一步開發(fā)本病的新型治療方法提供條件。研究方法：將符合診斷標準的60例慢性腹瀉虛寒證患者納入,隨機分為兩組,治療組和對照組各30例。治療組予以溫中悅腸方湯劑,1劑/日,水煎服,早晚分服；配合中藥足浴,1次/日。對照組予以蒙脫石散(思密達)口服,3g/次,3次/日。兩組均在治療4周后評價療效,治療結(jié)束后4周進行隨訪。研究結(jié)果：(1)綜合療效：治療組30例中,臨床有效5例,顯效19例,痊愈4例,無效2例,愈顯率為76.67%,總有效率為93.33%：對照組30例中,臨床有效10例,顯效10例,痊愈2例,無效8例,愈顯率為40.00%,總有效率為73.33%；治療組總有效率明顯優(yōu)于對照組(93.33%vs73.33%,P0.05)。(2)臨床主要癥狀：組內(nèi)比較：兩組主要癥狀治療后均有改善, (P0.05)：組間比較：治療后大便泄瀉和畏寒肢冷癥狀改善均比對照組明顯(P0.05)。(3)臨床次要癥狀：組內(nèi)比較：治療組各次要癥狀治療后與治療前相比均有改善(P0.05)；對照組脘腹痞滿、腹脹腹痛癥狀改善明顯(P0.05),而食欲不振、腰膝冷痛癥狀改善不明顯(P0.05)。組間比較：治療組對于食欲不振、腰膝冷痛癥狀改善優(yōu)于對照組(P0.05),兩組對于脘腹痞滿、腹脹腹痛癥狀改善相當(P0.05)。(4)安全性評估：兩組患者在治療過程中1均未發(fā)生不良反應(yīng),治療前后各安全性檢驗指標均未發(fā)現(xiàn)異常。(5)遠期隨訪：治療結(jié)束后4周內(nèi)復(fù)發(fā)率治療組低于對照組(7.14%vs40.91%,P0.05)。研究結(jié)論：溫中悅腸方配合中藥足浴治療虛寒型慢性腹瀉具有較好療效,安全性較高,值得臨床推廣應(yīng)用。
[Abstract]:Objective: the purpose of this study was to investigate the experience and clinical effect of Chinese medicine experience prescription combined with foot bath in treating chronic diarrhea (deficiency cold syndrome) with chronic diarrhea and cold syndrome. To provide conditions for further development of the new treatment of this disease. Methods: 60 patients with chronic diarrhea and cold syndrome were randomly divided into two groups: treatment group (n = 30) and control group (n = 30). The treatment group was given 1 dose / day of Wenzhong Yueshang decoction, 1 dose of water decoction, divided in the morning and evening, and 1 time / day with traditional Chinese medicine foot bath. In the control group, montmorillonite powder (smecta) was given orally for 3 g / d. Both groups were evaluated after 4 weeks of treatment and followed up 4 weeks after treatment. Results: (1) Comprehensive curative effect: in the treatment group, there were 5 cases of clinical effectiveness, 19 cases of remarkable effect, 4 cases of cure, 2 cases of ineffectiveness, 76.67% of the total effective rate and 93.33% of the total effective rate. In the control group of 30 cases, 10 cases were clinically effective, 10 cases were remarkable effect, 2 cases were cured. The effective rate was 40.00and the total effective rate was 73.33; The total effective rate in the treatment group was significantly better than that in the control group (93.33 vs 73.33 vs P0.05). (2): the main symptoms in the two groups were improved after treatment. (P0.05): the comparison between the two groups: the improvement of stool diarrhea and cold limb symptoms after treatment was better than that of the control group. Significant (P0.05). (3) clinical secondary symptoms: intragroup comparison: treatment group after treatment of secondary symptoms were improved compared with before treatment (P0.05); In the control group, the symptoms of abdominal distension and abdominal pain were improved significantly (P0.05), while the symptoms of anorexia and cold pain of waist and knee were not improved significantly (P0.05). Comparison between groups: the treatment group for loss of appetite, waist and knee cold pain symptoms improved better than the control group (P0.05), the two groups for epigastric fullness, abdominal distension and abdominal pain symptoms improved equivalent (P0.05). (4) safety evaluation: the two groups of patients in the course of treatment 1 did not have adverse reactions, (5) Long-term follow-up: the recurrence rate within 4 weeks after treatment in the treatment group was lower than that in the control group (7.14 vs 40.91). (5) Long-term follow-up: the recurrence rate in the treatment group was lower than that in the control group (7.14 vs 40.91% P 0.05). Conclusion: Wenzhong Yueshang recipe combined with traditional Chinese medicine foot bath has better curative effect and higher safety in treating chronic diarrhea of deficiency cold type, and it is worth popularizing in clinic.
