【摘要】：目的:本研究主要觀察補腎疏肝活血調(diào)周法治療腎虛肝郁型多囊卵巢綜合征患者的臨床療效,并對其治療效果做出科學(xué)評價,以豐富本病的臨床治療方法,為多囊卵巢綜合征的臨床治療提供參考。方法:選取符合本課題標準的PCOS患者32例,用補腎疏肝活血調(diào)周法治療三個月經(jīng)周期,分別于治療前和療程結(jié)束后觀察患者臨床癥狀(多毛、痤瘡、月經(jīng)不調(diào)、肥胖)、妊娠情況、中醫(yī)證候積分、性激素水平及B超(卵巢多囊樣改變)的變化情況,并根據(jù)臨床綜合療效判定標準評估補腎疏肝活血調(diào)周法治療腎虛肝郁型PCOS的總療效。結(jié)果:補腎疏肝活血調(diào)周法治療PCOS取得以下結(jié)果:1.痊愈7例,顯效10例,9例有效,6例無效,總有效率達到81.2%。2.治療后患者臨床癥狀得到明顯改善:月經(jīng)周期縮短,治療前后差異具有顯著性(P0.05),閉經(jīng)者11例,5例治愈,治愈率達45.5%;治療后患者痤瘡癥狀明顯改善,治療前后差異具有顯著性;多毛18例,無患者治愈,但多毛評分等級有不同程度的下降;治療后10例不孕患者有3例懷孕;治療后肥胖患者的BMI較治療前下降,差異有統(tǒng)計學(xué)意義。3.對中醫(yī)證候積分的影響:中醫(yī)證候積分治療后明顯降低,治療前后差異具有顯著性。4.對血清性激素水平的影響:治療后LH、LH/FSH、T的水平較治療前明顯下降,差異具有統(tǒng)計學(xué)意義;治療后FSH變化不明顯,無統(tǒng)計學(xué)意義。5.卵巢多囊樣人數(shù)的改變:治療后卵巢多囊樣改變患者的例數(shù)降低,治療前雙側(cè)卵巢多囊樣改變的患者有17例,單側(cè)卵巢多囊樣改變的患者有9例;治療后雙側(cè)卵巢多囊樣改變的患者降至11例,單側(cè)卵巢多囊樣改變的患者降至6例,治療前后差異具有顯著性。結(jié)論:補腎疏肝活血調(diào)周法治療腎虛肝郁型PCOS患者療效確切,可改善患者月經(jīng)不調(diào)、多毛、痤瘡、肥胖等癥狀,降低中醫(yī)證候評分,調(diào)節(jié)PCOS患者內(nèi)分泌紊亂的狀態(tài),改善患者卵巢多囊情況,同時有助于改善患者妊娠。研究證明,補腎疏肝活血調(diào)周法值得進一步推廣應(yīng)用。
[Abstract]:Objective: to observe the clinical effect of tonifying kidney, dispersing liver and activating blood circulation and regulating weeks in treating polycystic ovary syndrome with kidney deficiency and liver depression, and to make a scientific evaluation of its therapeutic effect, so as to enrich the clinical treatment methods of this disease. To provide a reference for the clinical treatment of polycystic ovary syndrome. Methods: 32 patients with PCOS who met the standard of this subject were treated with the method of tonifying the kidney, soothing the liver and activating blood circulation for three menstrual cycles. The clinical symptoms of the patients were observed before and after the course of treatment (hirsutism, acne, irregular menstruation). Obesity), pregnancy, TCM syndrome score, sex hormone level and B-ultrasound (ovarian polycystic changes), and according to the clinical comprehensive efficacy criteria to evaluate the treatment of kidney deficiency and liver stagnation of PCOS treatment of the total efficacy of the method of tonifying the kidney, dispersing the liver, activating blood circulation and regulating the week. Results: the treatment of PCOS by tonifying kidney, dispersing liver, activating blood circulation and regulating weeks has obtained the following results: 1: 1. 7 cases were cured, 9 cases were effective and 6 cases were ineffective, the total effective rate was 81.2%. After treatment, the clinical symptoms of the patients were obviously improved: the menstrual cycle was shortened, the difference before and after treatment was significant (P0.05), 11 cases of amenorrhea were cured in 5 cases, the cure rate was up to 45.5%, the symptoms of acne were obviously improved after treatment, and the difference was significant before and after treatment. There were 18 cases of hirsutism, none of them cured, but the score of hirsutism decreased to some extent; after treatment, 3 cases of infertile patients were pregnant; the BMI of obese patients was lower than that of before treatment, the difference was statistically significant. 3. The effect on TCM syndrome integral: TCM syndrome integral significantly decreased after treatment, the difference before and after treatment was significant. 4. Effect on serum sex hormone level: after treatment, the level of LH,LH/FSH,T decreased significantly compared with before treatment, the difference was statistically significant; after treatment, the change of FSH was not obvious, there was no statistical significance. 5. The number of patients with ovarian polycystic changes decreased after treatment, 17 patients with bilateral ovarian polycystic changes and 9 patients with unilateral ovarian polycystic changes before treatment. After treatment, the number of patients with bilateral ovarian polycystic changes decreased to 11 cases, and that of unilateral ovarian polycystic changes decreased to 6 cases. There was significant difference before and after treatment. Conclusion: the therapy of tonifying kidney, dispersing liver and activating blood circulation and regulating weeks is effective in treating PCOS patients with kidney deficiency and liver depression. It can improve the symptoms of irregular menstruation, hirsutism, acne and obesity, reduce the score of TCM syndromes and regulate the state of endocrine disorder in patients with PCOS. Improve the patient's ovarian polycystic condition, at the same time help to improve the patient's pregnancy. The study proved that the method of tonifying kidney, dispersing liver and activating blood circulation is worth further popularizing.
