活血止痛膏治療膝痹病瘀血痹阻證的臨床研究

【摘要】：目的：目前探究對于膝骨性關節(jié)炎的治療方法是重要的一項臨床課題。本課題通過對活血止痛膏治療膝骨性關節(jié)炎的臨床療效的觀察以及其對于人體膝關節(jié)液中相關因子指標的影響,提供了中醫(yī)藥更安全有效的應用到膝骨性關節(jié)炎的治療依據。方法：本研究選擇44例膝骨性關節(jié)炎患者,入院后取所有確診患者膝關節(jié)液,用于測定TNF-α、IL-6含量,運用ELISA法來檢查TNF-α和IL-6含量,選用上海江萊生物科技有限公司試劑盒。操作均按操作試劑盒說明書來進行。同時予以VAS評分及麥克馬斯特大學骨關節(jié)炎指數(WOMAC)評分。然后膝關節(jié)外敷活血止痛膏,每次4小時,每天1次,治療2周后和治療4周后,用同樣方法復查TNF-α、IL-6及復測VAS評分及WOMAC評分。通過觀察VAS評分、WOMAC評分和血清TNF-α、IL-6數值的變化,從臨床療效上探究活血止痛膏對于膝骨性關節(jié)炎的效果。結果：1.安全性方面：一位患者因皮膚過敏退出本次臨床試驗觀察,其余患者均未出現不良反應。2.視覺疼痛評分(VAS)方面：經外敷活血止痛膏,42位患者疼痛癥狀較治療前有改善,用藥2周、用藥4周與治療前比較均存在差異(P0.05)有統(tǒng)計學意義。3.骨關節(jié)炎指數(WOMAC)方面：經外敷活血止痛膏,42位患者WOMAC指數較治療前有降低,用藥2周、用藥4周與治療前比較均存在差異(P0.05)有統(tǒng)計學意義。4.生化指標方面：經外敷活血止痛膏,42位患者血清中TNF-α及IL-6數值較治療前有所減低,用藥2周、用藥4周與治療前比較均存在差異(P0.05)有統(tǒng)計學意義。結論：活血血-J_止痛膏對于減輕膝骨性關節(jié)炎患者疼痛等臨床癥狀是有效的,其可以使膝關節(jié)功能改善,降低血清TNF-a及IL-6含量,不良反應較低,副作用較小,對于治療膝骨性關節(jié)炎是安全可行的。
[Abstract]:Objective: to explore the treatment of knee osteoarthritis is an important clinical topic. By observing the clinical curative effect of Huoxue Zhitong ointment in treating knee osteoarthritis and its influence on the related factors in human knee joint fluid, this subject provides a more safe and effective basis for the treatment of knee osteoarthritis with traditional Chinese medicine. Methods: in this study, 44 patients with knee osteoarthritis were selected. The knee joint fluid was taken from all the patients after admission, and the contents of TNF- 偽 and IL-6 were measured by ELISA method. The kit of Shanghai Jianglai Biotechnology Co., Ltd was used to detect the content of TNF- 偽 and IL-6. The operation is carried out according to the instructions of the kit. At the same time, the VAS score and the (WOMAC) score of the McMaster University osteoarthritis index were given. Then the knee joint was applied with Huoxue Zhitong ointment, 4 hours each time, once a day, 2 weeks after treatment and 4 weeks after treatment, TNF- 偽 IL-6, VAS score and WOMAC score were re-examined with the same method. The effect of Huoxue Zhitong ointment on knee osteoarthritis was investigated by observing the changes of VAS score and serum TNF- 偽 and IL-6 in order to find out the clinical efficacy of Huoxue Zhitong ointment. The result is 1: 1. Safety: one patient withdrew from the clinical trial because of skin allergy, and none of the other patients had adverse effects. 2. 2. Visual pain score (VAS): pain symptoms of 42 patients were improved after external application of Huoxue Zhitong ointment. There were significant differences in pain symptoms between 2 weeks and 4 weeks after treatment (P0.05). Osteoarthritis index (WOMAC): the WOMAC index of 42 patients treated with external application of Huoxue Zhitong ointment was lower than that before treatment. There were significant differences in WOMAC index between 2 weeks and 4 weeks after treatment (P0.05). Biochemical indexes: the values of TNF- 偽 and IL-6 in serum of 42 patients with Huoxue Zhitong ointment were lower than those before treatment. There were significant differences in the values of TNF- 偽 and IL-6 between 2 weeks and 4 weeks after treatment (P0.05). Conclusion: Huoxue Xue-J _ Zhitong ointment is effective in relieving pain and other clinical symptoms in patients with knee osteoarthritis. It can improve knee joint function, reduce serum TNF-a and IL-6 levels, lower adverse reactions and less side effects. It is safe and feasible for the treatment of knee osteoarthritis.
