小柴胡顆粒合逍遙丸治療橋本氏病的臨床療效觀察

【摘要】：目的:通過與口服西藥的對照組進行比較研究,從臨床癥狀的改善和實驗室化驗指標(biāo)的變化兩方面來觀察和評價小柴胡顆粒與逍遙丸聯(lián)用對橋本氏病患者的治療療效,同時對于所選西藥和治療組所選藥物的理論依據(jù)的來源、現(xiàn)代藥理學(xué)研究以及目前在臨床中的應(yīng)用進行闡明、論述和分析,為在橋本氏病的治療過程中中成藥所應(yīng)該發(fā)揮的作用提供證明。方法:選取60例臨床初診為橋本氏病的患者,依據(jù)臨床試驗的隨機對照原則,隨機分為對照組和治療組,兩組各30例。對照組:口服硒酵母膠囊100ug/次,2次/日;治療組:口服小柴胡顆粒,每次10g,每日兩次;逍遙丸,每次3g,每日兩次,兩種中成藥在我院門診配藥。兩組均以8周為一個療程,觀察一個療程,兩組患者在治療前后都要檢測TPO-Ab(甲狀腺過氧化物酶抗體)和甲功三項包括:TSH(促甲狀腺素)、FT3(游離三碘甲狀腺原氨酸)、FT4(游離甲狀腺素)進行檢測,均采用化學(xué)發(fā)光法,此外,還需比較兩組實驗對象在治療前和治療后的中醫(yī)證候積分。運用SPSS.21.0統(tǒng)計軟件分析所得實驗數(shù)據(jù)。在研究期間有藥物過敏等不良反應(yīng)發(fā)生時退出研究。結(jié)果:兩組患者在治療前的年齡、性別等一般情況未見統(tǒng)計學(xué)差異,組間可比(P0.05),治療前的血清過氧化物酶抗體(TPO-Ab)以及中醫(yī)證候積分也無統(tǒng)計學(xué)差異,具有可比性(P0.05)。經(jīng)過8周的治療,得出:(1)在各自小組內(nèi)部比較治療前后的TPO-Ab濃度可見統(tǒng)計學(xué)差異(P0.05),在兩組之間比較未見差異(P0.05)。(2)在組內(nèi)對比中醫(yī)證候積分,可見治療后兩組積分都有明顯下降,比較數(shù)據(jù)具有統(tǒng)計學(xué)意義(P0.01);兩組之間相比明顯可見治療組改善情況較好,對比數(shù)據(jù)有統(tǒng)計學(xué)意義(P0.01)。(3)中醫(yī)證候療效評定,治療組和對照組的總有效率分別為86.7%和50%,治療組療效優(yōu)于對照組,經(jīng)秩和檢驗具有統(tǒng)計學(xué)意義(P0.01)。結(jié)論:中成藥小柴胡顆粒和逍遙丸聯(lián)用可使橋本氏病患者的TPO-Ab濃度下降,同時研究觀察到,硒酵母膠囊在降低TPO-Ab濃度方面也有明顯的療效,但是在改善臨床癥狀方面,治療組明顯優(yōu)于對照組。
[Abstract]:OBJECTIVE:To observe and evaluate the therapeutic effect of Xiaochaihu Granules combined with Xiaoyao Pills on Hashimoto's disease by comparing with the control group of oral western medicine in terms of the improvement of clinical symptoms and the changes of Laboratory Laboratory Laboratory test indexes. Methods: 60 patients with Hashimoto's disease were randomly divided into control group and treatment group according to the randomized control principle of clinical trial, and each group had 30 cases. The control group: oral selenium yeast capsule 100ug / time, twice a day; the treatment group: oral small Bupleurum granules, 10 g each time, twice a day; Xiaoyao pills, 3 g each time, twice a day, two kinds of Chinese patent medicine in our hospital outpatient dispensing. The body and thyroid function of the three items include: TSH (thyrotropin), FT3 (free triiodothyronine), FT4 (free thyroxine) were detected by chemiluminescence method, in addition, the two groups of experimental subjects before and after treatment of TCM syndrome integrals need to be compared. Result: There was no significant difference in age, sex and other general conditions between the two groups before treatment (P 0.05). There was no significant difference in serum peroxidase antibody (TPO-Ab) and TCM syndrome score between the two groups before treatment (P 0.05). The TPO-Ab concentration in each group before and after treatment showed statistical difference (P 0.05), and there was no difference between the two groups (P 0.05). (2) Compared with the scores of TCM syndromes in the group, the scores of the two groups were significantly decreased after treatment, and the comparison data was statistically significant (P 0.01); The improvement of the treatment group was significantly better than that of the two groups (P 0.01). The total effective rate of the treatment group and the control group were 86.7% and 50%, respectively. The curative effect of the treatment group was better than that of the control group. The rank sum test showed statistical significance (P 0.01). Conclusion: The combination of Xiaochaihu Granule and Xiaoyao Pill can reduce the concentration of TPO-Ab in Hashimoto's disease patients. Selenium yeast capsule was also effective in reducing the concentration of TPO-Ab, but the treatment group was superior to the control group in improving clinical symptoms.
