【摘要】：目的:本課題為清熱化濕、健脾和胃法治療Hp相關(guān)性慢性非萎縮性胃炎的臨床研究。選用中醫(yī)經(jīng)典方劑清中湯加味作為清熱化濕、健脾和胃法的代表方劑進(jìn)行研究。通過(guò)對(duì)清中湯加味治療Hp相關(guān)性慢性非萎縮性胃炎的臨床研究,明確其對(duì)本病的臨床有效性、臨床安全性,驗(yàn)證中醫(yī)藥對(duì)本病的治療優(yōu)勢(shì)。方法:所有入組患者均來(lái)自于長(zhǎng)春中醫(yī)藥大學(xué)附屬醫(yī)院肝脾胃病科門(mén)診,且均屬于Hp相關(guān)性慢性非萎縮性胃炎患者,采用隨機(jī)對(duì)照方法,將入組病例分成兩組,即治療組與對(duì)照組,每組患者30例。其中治療組在應(yīng)用標(biāo)準(zhǔn)三聯(lián)療法(雷貝拉唑鈉腸溶片10mg、阿莫西林1000mg、克拉霉素500mg均餐后日2次口服)的基礎(chǔ)上給予清中湯加味治療。對(duì)照組僅采用標(biāo)準(zhǔn)三聯(lián)療法治療。各組均以2周為1個(gè)療程,共治療1個(gè)療程。治療過(guò)程中密切觀察并準(zhǔn)確記錄兩組患者的癥狀以及體征的變化。1個(gè)療程治療結(jié)束4周后,復(fù)查并記錄Hp的變化,比較治療前及治療后的Hp根除情況,并進(jìn)行統(tǒng)計(jì)學(xué)分析。治療前及治療后各進(jìn)行安全性指標(biāo)觀測(cè)1次。結(jié)果:治療組30例,對(duì)照組30例,共有60例患者按照治療方案完成了本研究。在總療效方面,治療組與對(duì)照組的總有效率分別為93.33%、80.00%,經(jīng)統(tǒng)計(jì)學(xué)分析,P=0.0040.01,說(shuō)明治療組的總療效明顯優(yōu)于對(duì)照組。兩組在根除Hp上,治療組有28例患者轉(zhuǎn)為陰性,2例未轉(zhuǎn)陰,有效率為93.33%,對(duì)照組有22例轉(zhuǎn)為陰性,8例未轉(zhuǎn)陰,有效率為73.33%,經(jīng)統(tǒng)計(jì)學(xué)分析,P=0.038(0.05),說(shuō)明治療組在Hp的根除方面明顯優(yōu)于對(duì)照組。中醫(yī)各證候積分方面,兩組在改善脘腹痞滿、胃脘疼痛、燒心或灼熱感、食少納呆、口干苦及身重困倦上均有明顯差異(P0.05)。在改善小便短黃方面,兩組具有顯著性差異(P0.01)。而在噯氣方面,治療組與對(duì)照組則無(wú)明顯差異(P0.05)。兩組患者在整個(gè)實(shí)驗(yàn)過(guò)程中,均未有不良反應(yīng)的發(fā)生。結(jié)論:清熱化濕、健脾和胃法在治療Hp相關(guān)性慢性非萎縮性胃炎方面具有顯著效果,能夠明顯改善患者癥狀,有利于Hp的根除。
[Abstract]:Objective: to study the treatment of chronic non-atrophic gastritis associated with Hp by clearing heat and removing dampness, invigorating spleen and stomach. The traditional Chinese medicine prescription Qingzhong decoction was chosen as the representative prescription of clearing heat and removing dampness, invigorating spleen and stomach. Through the clinical study on the treatment of chronic non-atrophic gastritis associated with Hp with Qingzhong decoction, the clinical efficacy and clinical safety of this disease were determined, and the advantages of traditional Chinese medicine in the treatment of this disease were verified. Methods: all the patients were from the outpatient department of liver, spleen and stomach, affiliated hospital of Changchun University of traditional Chinese Medicine, and belonged to the patients with chronic non-atrophic gastritis associated with Hp. The patients were divided into two groups by randomized control method. Treatment group and control group, 30 patients in each group. The treatment group was treated with Qingzhong decoction on the basis of standard triple therapy (10 mg of Rabeprazole sodium enteric-coated tablets, 1000 mg of amoxicillin and twice a day of clarithromycin 500mg). The control group was treated with standard triple therapy only. All groups were treated with 2 weeks as a course of treatment. The changes of symptoms and signs of the two groups were observed and accurately recorded in the course of treatment. After a course of treatment for 4 weeks, the changes of Hp were rechecked and recorded, and the eradication of Hp before and after treatment was compared and analyzed statistically. The safety indexes were observed once before and after treatment. Results: the treatment group (30 cases) and the control group (30 cases) completed the study according to the treatment protocol. The total effective rate of the treatment group and the control group was 93.3380.000.The total effective rate of the treatment group was significantly better than that of the control group by statistical analysis. In the eradication of Hp in both groups, 28 patients in the treatment group turned negative, 2 cases did not turn negative, and the effective rate was 93.33. In the control group, 22 cases turned negative and 8 cases did not turn negative. The effective rate was 73.33 (P 0.038), which indicated that the treatment group was superior to the control group in the eradication of Hp. TCM syndromes integral, the two groups in the improvement of epigastric fullness, epigastric pain, heartburn or burning feeling, food less, dry mouth and heavy sleepiness were significantly different (P0.05). There was a significant difference between the two groups in improving short yellow urine (P0.01). In terms of belching, there was no significant difference between the treatment group and the control group (P0.05). There were no adverse reactions in both groups during the whole experiment. Conclusion: the methods of clearing heat and removing dampness, invigorating spleen and stomach have remarkable effect in treating chronic non-atrophic gastritis associated with Hp, and can obviously improve the symptoms of patients and help to eradicate Hp.
