【摘要】：目的:趙振昌教授是國(guó)家級(jí)名老中醫(yī),臨床經(jīng)驗(yàn)豐富,特別是治療腎臟疾病,效果顯著�；跀�(shù)據(jù)挖掘技術(shù)整理并總結(jié)趙老治療腎性血尿的辨證與用藥規(guī)律,旨在總結(jié)其辨證思維與用藥特色,傳承其學(xué)術(shù)思想及臨床經(jīng)驗(yàn)。方法:跟師出診,通過(guò)拍照、攝像等方式收集并整理出趙振昌教授于2014年2月—2016年2月期間門(mén)診病案原始資料398例,初步規(guī)范化病案信息后整理成word文檔,錄入數(shù)據(jù)挖掘系統(tǒng),系統(tǒng)自動(dòng)分析病案,歸納方法為疾病的證型分布、藥物頻次、藥對(duì)規(guī)律等,最后可總結(jié)并歸納其治療腎性血尿的辨證思維及用藥特色。結(jié)果:對(duì)納入的398例病案資料進(jìn)行數(shù)據(jù)分析,總結(jié)經(jīng)驗(yàn)如下:1、患者好發(fā)于青中年,且男性多于女性。2、秋、冬季患者居多,春、夏季患者較少。3、病因大致可分為素體虧虛,熱邪,瘀血,熱毒。4、病位主要在腎和膀胱,與脾、肝關(guān)系密切。5、證型排序依次為:脾腎雙虧證,下焦?jié)駸嶙C,外感熱邪證,腎虛火熱證,瘀血阻絡(luò)證,毒熱內(nèi)阻證。6、常用藥對(duì)10對(duì):熟地黃-生地黃,山萸肉-熟地黃,山萸肉-生地黃,小薊-地榆,仙鶴草-馬齒莧,杜仲-牛膝,地膚子-茜草,茯苓-澤瀉,牡丹皮-鱉甲,肉蓯蓉-大黃。7、常用自擬方:山萸肉、熟地黃、生地黃、小薊、側(cè)柏炭、藕節(jié)、馬齒莧、白茅根、仙鶴草、甘草。8、隨證加減藥物:涼血止血:地榆、地膚子、鱉甲、茜草;滋陰清熱:牡丹皮、鱉甲、白芍、墨旱蓮;利尿消腫:白術(shù)、茯苓、澤瀉、車(chē)前子。結(jié)論:趙振昌教授運(yùn)用中醫(yī)藥治療腎性血尿效果顯著,趙老認(rèn)為本病與素體虧虛、熱邪、瘀血、熱毒等因素關(guān)系密切,青中年患者居多,男患多于女患,秋冬季易復(fù)發(fā)本病,病位大致在腎、膀胱,疾病發(fā)展不盡相同,勿要見(jiàn)血既止血,要針對(duì)疾病的不同特點(diǎn)進(jìn)行診療。發(fā)掘名老中醫(yī)的辨證思維和用藥特色,對(duì)于傳承傳統(tǒng)中醫(yī)具有重要意義,也是我們年輕中醫(yī)人學(xué)好中醫(yī)的必要手段。
[Abstract]:Objective: professor Zhao Zhenchang is a national famous Chinese medicine with rich clinical experience, especially in the treatment of renal diseases. Based on the data mining technique, this paper summarizes Zhao Lao's syndrome differentiation and drug use rule in the treatment of renal hematuria, in order to sum up his syndrome differentiation thinking and medication characteristics, and inherit his academic thought and clinical experience. Methods: the original data of outpatient medical records of Professor Zhao Zhenchang from February 2014 to February 2016 were collected and sorted out by means of photography, camera and so on. The primary standardized medical records were compiled into word documents. The data mining system is used to analyze the medical records automatically. The methods are as follows: the distribution of syndromes, the frequency of drugs, the law of drug pairs, etc. Finally, the dialectical thinking and the characteristics of drugs used in the treatment of renal hematuria can be summed up. Results: the data of 398 cases of medical records were analyzed, and the experience was summarized as follows: 1. The patients tend to occur in youth and middle age, and the male is more than female. In autumn, the majority of the patients are in winter, spring and summer. The etiology can be roughly divided into deficiency of body and heat. Blood stasis, heat toxin. 4. The disease is mainly located in the kidney and bladder, closely related to spleen and liver. The order of syndrome type is as follows: spleen and kidney double deficiency syndrome, lower scorch dampness and heat syndrome, exogenous heat evil syndrome, kidney deficiency and fire heat syndrome, blood stasis blocking collaterals syndrome, the order of syndrome is: spleen and kidney deficiency, dampness and heat, heat deficiency of kidney, blood stasis and obstruction of collaterals. Toxic Heat Internal obstruction Syndrome .6, 10 pairs of common medicine: Rehmannia glutinosa, Corni meat-cooked Rehmannia, Corni meat-Rehmannia, small thistle, Portulaca oleracea, Eucommia Achyranthes, Fructus deliciae-alizarin, Poria Rehmanniae, Peony skin-Turtle beetle, Cistanche deserticola-rhubarb. 7, commonly used: Cornel, cooked Rehmannia, Rehmannia, small artichokes, Platycladus charcoal, lotus root, Portulaca oleracea, White Roots, Herba Herba, Glycyrrhizae. Ziyin Qingre: peony skin, Biejia, Radix Paeoniae Alba, Herba Mosurea; diuretic detumescence: Atractylodes macrocephala, Poria cocos, Alismatis, Plantago. Conclusion: professor Zhao Zhenchang used traditional Chinese medicine to treat renal hematuria. Zhao Lao thought that this disease was closely related to factors such as deficiency of element body, heat evil, blood stasis, heat poison and so on. Most of the young and middle-aged patients had more male than female, and it was easy to recur in autumn and winter. The disease is located in the kidney, bladder, disease development is not the same, not to see blood both hemostasis, should be based on the different characteristics of the disease diagnosis and treatment. It is of great significance to explore the dialectical thinking and medication characteristics of famous traditional Chinese medicine, which is also a necessary means for our young Chinese medicine practitioners to learn Chinese medicine well.
