【摘要】：目的:通過(guò)對(duì)觀察病例的尿潴留發(fā)生率、排尿情況、小腹癥狀等觀察,了解耳穴壓豆法預(yù)防肛腸病術(shù)后尿潴留的效果。以求進(jìn)一步了解其可行性、實(shí)用性。方法:1.選取所有病情為一般,采用腰腧穴進(jìn)行麻醉,并采用中醫(yī)傳統(tǒng)術(shù)式進(jìn)行手術(shù)的肛腸病(混合痔、肛裂、肛瘺、肛周膿腫單一病種)術(shù)后病人,入組病例均為來(lái)自我院2015年1月至2016年1月肛腸科療區(qū)的住院患者。將符合入組條件的66例患者按隨機(jī)法分為治療組和對(duì)照組兩組。每組33例。治療組采用耳穴壓豆法(常規(guī)護(hù)理基礎(chǔ)上進(jìn)行)進(jìn)行治療;對(duì)照組在常規(guī)護(hù)理基礎(chǔ)上不予處置。兩組其他條件相同(麻醉方式、手術(shù)方式、術(shù)前術(shù)后心理疏導(dǎo)),分別對(duì)術(shù)后2小時(shí)、術(shù)后4小時(shí)、術(shù)后6小時(shí)、術(shù)后8小時(shí)患者排尿情況進(jìn)行觀察,進(jìn)行統(tǒng)計(jì)分析,治療期間并進(jìn)行安全性檢查和療效性觀查。結(jié)果:術(shù)后首次排尿時(shí)間、排尿所需時(shí)間、排尿情況、小腹癥狀等對(duì)比,表明治療組效果優(yōu)于對(duì)照組;兩組尿潴留發(fā)生情況結(jié)果顯示:治療組術(shù)后尿潴留發(fā)生率為6.06%,對(duì)照組術(shù)后尿潴留的發(fā)生率為30.30%。經(jīng)卡方檢驗(yàn),表明P=0.0250.05,兩組存在統(tǒng)計(jì)學(xué)意義。結(jié)論:在預(yù)防肛腸病術(shù)后尿潴留方面,耳穴壓豆法可有效預(yù)防尿潴留的發(fā)生,減輕術(shù)后排尿相關(guān)的不適癥狀,減輕患者痛苦,療效安全可靠。
[Abstract]:Objective: to observe the incidence of urinary retention, micturition and the symptoms of lower abdomen in order to understand the effect of Auricular Point pressure on preventing postoperative urinary retention of anorectal disease. In order to further understand its feasibility and practicability. Method 1: 1. Patients with anorectal diseases (mixed hemorrhoids, anal fissure, anal fistula, perianal abscess) who were anesthetized with waist and acupoints and operated by traditional Chinese medicine were selected as general. All patients were hospitalized from January 2015 to January 2016. A total of 66 patients were randomly divided into two groups: treatment group and control group. 33 cases in each group. The treatment group was treated by auricular acupoint pressing bean (on the basis of routine nursing), while the control group was not treated on the basis of routine nursing. The other conditions of the two groups were the same (anaesthesia, operation, psychological guidance before and after operation). The patients' urination was observed at 2 hours after operation, 4 hours after operation, 6 hours after operation and 8 hours after operation, respectively. Safety examination and efficacy observation were also performed during treatment. Results: the comparison of the first time of urination, the condition of urination and the symptoms of abdomen showed that the effect of the treatment group was better than that of the control group. The results showed that the incidence of postoperative urinary retention was 6.06 in the treatment group and 30.30 in the control group. The chi-square test showed that the two groups had statistical significance. Conclusion: in the prevention of postoperative urinary retention of anorectal disease, auricular acupoint pressing method can effectively prevent the occurrence of urinary retention, relieve the symptoms associated with urination, alleviate the pain of patients, and the efficacy is safe and reliable.
