【摘要】：目的:探討滋陰潛陽辨證治療頑固性高血壓合并頸動脈粥樣硬化患者NO、CGRP以及生存質量的影響。方法:選取醫(yī)院2013年8月—2016年1月收治的頑固性高血壓合并頸動脈粥樣硬化患者84例,按隨機數字表法分組,分別42例,對照組予以降壓、穩(wěn)定斑塊治療;研究組在降壓、穩(wěn)定斑塊基礎上予以滋陰潛陽辨證治療,共治療2個月為1療程,采血測定炎癥因子以及血管內皮功能,超聲觀察頸動脈粥樣硬化斑塊改變,記錄SF-36生存質量,同時對比臨床療效。結果:與治療前比較,兩組治療后FMD升高,斑塊Crouse積分、IMT降低,治療后生理功能、總體健康、活力、社會功能、精神健康評分升高,治療后IL-6、CRP、vWF含量降低,治療后ET降低,NO、CGRP含量升高(P0.05);與對照組比較,研究組治療后FMD較高,斑塊Crouse積分、IMT較低,治療后生理功能、總體健康、活力、社會功能、精神健康評分較高,治療后IL-6、CRP、vWF含量較低,治療后ET較低,NO、CGRP含量較高(P0.05);對照組有效率71.43%與研究組有效率90.48%數據對比,研究組高于對照組(P0.05)。結論:滋陰潛陽辨證治療頑固性高血壓合并頸動脈粥樣硬化療效確切,穩(wěn)定斑塊,改善內皮功能及炎癥指標,提高生存質量。
[Abstract]:Objective: to explore the effect of differentiation of nourishing yin and burying yang on NO-CGRP and quality of life in patients with intractable hypertension complicated with carotid atherosclerosis. Methods: from August 2013 to January 2016, 84 patients with intractable hypertension complicated with carotid atherosclerosis were selected and divided into two groups: 42 patients in the control group were treated with hypotension and plaque stabilization. The stable plaque was treated with syndrome differentiation of nourishing yin and burying yang for 2 months. The inflammatory factors and vascular endothelial function were measured by blood collection, the changes of carotid atherosclerotic plaque were observed by ultrasound, and the quality of life of SF-36 was recorded. At the same time, the clinical efficacy was compared. Results: compared with before treatment, FMD increased, plaque Crouse score decreased, physiological function, general health, vitality, social function and mental health scores increased after treatment, and the content of IL-6 / CRP VWF decreased after treatment. Compared with the control group, the content of FMD was higher, the Crouse score of plaque was lower, the physiological function, general health, vitality, social function, mental health score were higher in the study group than in the control group (P0.05), and the content of IL-6 CRPvWF was lower in the study group than in the control group. After treatment, the content of CGRP in et was higher than that in control group (P0.05), the effective rate of control group was 71.43% and the effective rate of study group was 90.48%, which was higher than that of control group (P0.05). Conclusion: it is effective to treat refractory hypertension with carotid atherosclerosis by syndrome differentiation of nourishing yin and burying yang, to stabilize plaque, to improve endothelial function and inflammation index, and to improve quality of life.
