閻小萍教授運(yùn)用“清法”治療風(fēng)濕病臨床經(jīng)驗(yàn)淺析

【摘要】：目的"清法"為中醫(yī)治法的八法之一,是中醫(yī)臨床的主要治療方法,是指運(yùn)用寒涼性質(zhì)的方藥治療里熱證的方法。導(dǎo)師閻小萍教授從醫(yī)四十余載,在各種風(fēng)濕病的中西醫(yī)結(jié)合診治方面積累了豐富的臨床經(jīng)驗(yàn),對(duì)于風(fēng)濕病的病因病機(jī)、診斷治療等方面有著獨(dú)到見解,本文總結(jié)了閻小萍教授運(yùn)用"清法"辨治風(fēng)濕病的學(xué)術(shù)思想和臨床經(jīng)驗(yàn)。方法本論文主要分為兩個(gè)部分,第一部分主要通過文獻(xiàn)采集的方法,總結(jié)從古至今各醫(yī)家對(duì)于"清法"的認(rèn)識(shí)和臨床運(yùn)用。第二部分主要是闡釋閻小萍教授對(duì)中醫(yī)治法和中醫(yī)風(fēng)濕病的理解以及運(yùn)用"清法"辨治風(fēng)濕病的學(xué)術(shù)思想和臨床經(jīng)驗(yàn)。通過對(duì)導(dǎo)師病案的收集與分析,總結(jié)閻小萍教授臨床運(yùn)用"清法"治療風(fēng)濕病遣方用藥特點(diǎn)。結(jié)果中醫(yī)學(xué)是祖國(guó)傳統(tǒng)醫(yī)學(xué),中醫(yī)治法是中醫(yī)學(xué)基礎(chǔ)理論之一,"清法"為中醫(yī)治法八法之一,是中醫(yī)臨床的主要治療方法。古往今來,歷代醫(yī)家對(duì)"清法"均有自己的認(rèn)識(shí)和臨床運(yùn)用經(jīng)驗(yàn)。閻小萍教授在研讀中醫(yī)經(jīng)典古籍的基礎(chǔ)上,結(jié)合臨床經(jīng)驗(yàn),對(duì)中醫(yī)風(fēng)濕病有著深層次的理解,首先對(duì)中醫(yī)風(fēng)濕病的含義做了闡述,病因病機(jī)方面,閻小萍教授認(rèn)為正氣不足是中醫(yī)風(fēng)濕病發(fā)病的內(nèi)在條件,風(fēng)寒濕熱等邪氣是發(fā)病的外在因素,并注重風(fēng)濕病"動(dòng)變制化"規(guī)律,突出"從化"理論。閻小萍教授之"補(bǔ)腎強(qiáng)骨"、"活血通絡(luò)"、"調(diào)和營(yíng)衛(wèi)"、"調(diào)肝健脾"等辨治法則在風(fēng)濕病治療中取得了良好療效。同時(shí)創(chuàng)造性的提出綜合強(qiáng)化序貫治療,為中醫(yī)風(fēng)濕病治療提供新思路。閻小萍教授在臨床治療中突出"治未病"的學(xué)術(shù)思想,包括未病先防和既病防變兩個(gè)方面,著重強(qiáng)調(diào)預(yù)防疾病的發(fā)生和早發(fā)現(xiàn)早治療的重要性。閻小萍教授行醫(yī)四十余載,善用中醫(yī)治法,在運(yùn)用"清法"治療風(fēng)濕病方面有自己獨(dú)到見解。閻小萍教授在運(yùn)用"清法"辨治風(fēng)濕病時(shí)注重"寒熱為綱"的辨治論,突出清臟腑邪氣,重視臟腑功能在風(fēng)濕病中的重要作用,同時(shí)提倡"內(nèi)外兼治",重視外治法的臨床運(yùn)用。閻小萍教授在運(yùn)用"清法"治療具體疾病時(shí)又有不同的側(cè)重點(diǎn),如強(qiáng)直性脊柱炎(大僂)注重補(bǔ)腎強(qiáng)骨、養(yǎng)肝柔筋、干燥綜合征(燥痹)注重補(bǔ)腎清熱育陰、回紋型風(fēng)濕病(周痹)注重"急則治其標(biāo),緩則之氣本"的原則,急性期清熱祛風(fēng)、除濕通絡(luò)為主,緩解期補(bǔ)益肝腎,顧護(hù)脾胃等,,臨床運(yùn)用每獲良效。結(jié)論"清法"是中醫(yī)學(xué)重要治法之一,在風(fēng)濕病的治療中有著重要應(yīng)用。閻小萍教授善用中醫(yī)治法治療風(fēng)濕病,她認(rèn)為風(fēng)濕病多以虛為本,或?qū)俦咎摌?biāo)實(shí)之候,治療應(yīng)從整體出發(fā),個(gè)體化治療。閻小萍教授運(yùn)用"清法"辨治風(fēng)濕病注重"寒熱為綱",突出臟腑辨證,提倡"內(nèi)外兼治",其學(xué)術(shù)思想完善,臨床經(jīng)驗(yàn)豐富,臨床上得到很好的療效。
[Abstract]:Objective "Qing method" is one of the eight methods of TCM treatment, it is the main method of TCM clinical treatment, and refers to the method of treating heat syndrome by using cold medicine. Professor Yan Xiaoping, a tutor who has been in medicine for more than 40 years, has accumulated rich clinical experience in the diagnosis and treatment of various rheumatic diseases by integrating traditional Chinese and western medicine. He has unique views on the etiology, pathogenesis, diagnosis and treatment of rheumatism. This paper summarizes Professor Yan Xiaoping's academic thought and clinical experience in treating rheumatism with Qing method. Methods this paper is divided into two parts. The first part summarizes the understanding and clinical application of "Qing method" by literature collection. The second part is mainly to explain Professor Yan Xiaoping's understanding of TCM treatment and rheumatism, as well as the academic thought and clinical experience of treating rheumatism by using "Qing method". Based on the collection and analysis of tutor's medical records, Professor Yan Xiaoping's clinical application of "Qing method" in the treatment of rheumatic diseases was summarized. Results TCM is the traditional medicine of the motherland, TCM treatment is one of the basic theories of TCM, "Qing method" is one of the eight methods of TCM treatment, and it is the main treatment method of TCM clinic. Through the ages, physicians had their own understanding and clinical experience of Qing Law. Professor Yan Xiaoping has a deep understanding of rheumatism in traditional Chinese medicine on the basis of studying classical ancient books of traditional Chinese medicine and combining with clinical experience. First of all, he expounds the meaning of rheumatism in traditional Chinese medicine, the etiology and pathogenesis of rheumatism. Professor Yan Xiaoping thinks that the deficiency of proper qi is the internal condition of rheumatism in TCM, and the evil factors such as wind, cold, dampness and heat are the external factors of the disease, and pay attention to the law of "dynamic transformation" of rheumatism, and stress the theory of "consonance". Professor Yan Xiaoping's differentiation and treatment such as "tonifying kidney and strengthening bone", "activating blood circulation and dredging collaterals", "reconciling Ying Wei", "regulating liver and strengthening spleen" and so on, have obtained good curative effect in the treatment of rheumatism. At the same time, we creatively put forward comprehensive intensive sequential therapy, which provides a new idea for the treatment of rheumatism in traditional Chinese medicine. Professor Yan Xiaoping has emphasized the academic thought of "preventing disease before treatment" in clinical treatment, including prevention of disease before illness and prevention of both disease and change, with emphasis on the importance of preventing the occurrence of disease and early discovery and treatment. Professor Yan Xiaoping has practiced medicine for more than 40 years, and has his own unique views on the treatment of rheumatism by using "Qing method". Professor Yan Xiaoping pays attention to the theory of "cold and heat as the outline" when using "Qing method" to distinguish and treat rheumatism, highlights the evil spirit of clearing the viscera, attaches importance to the important role of the function of zang-fu organs in rheumatism, and advocates "both internal and external treatment" and attaches importance to the clinical application of the external treatment method. Professor Yan Xiaoping has different emphases in the treatment of specific diseases, such as ankylosing spondylitis (big rickets) pay attention to tonifying kidney and strengthening bone, nourishing liver and soft tendons, and Sjogren syndrome (dryness) pay attention to tonifying kidney and clearing away heat and nourishing yin. Return striated type rheumatism (Zhou Bi) pay attention to "urgent is treated its standard, slow is Qi this" principle, acute phase clears heat dispels wind, dehumidify and clear collaterals is main, remission period invigorates liver and kidney, care to protect spleen and stomach and so on, clinical use obtains each good effect. Conclusion Qing method is one of the most important methods in traditional Chinese medicine and has important application in the treatment of rheumatism. Professor Yan Xiaoping makes good use of traditional Chinese medicine to treat rheumatism. She thinks that rheumatism is mostly based on deficiency, or is a sign of deficiency, treatment should be from the whole, individualized treatment. Professor Yan Xiaoping uses "Qing method" to distinguish and treat rheumatism by focusing on "cold and heat as the outline", highlighting the syndrome differentiation of zang-fu organs and advocating "internal and external treatment". His academic thought is perfect, clinical experience is rich, and the clinical effect is very good.
