【摘要】：目的研究加味苦參湯治療手部濕疹急性期的臨床療效,觀察加味苦參湯對于緩解手部濕疹急性期的皮損、瘙癢等方面的作用,探討其治療手部濕疹急性期的機理,為臨床治療濕疹開拓新思路、新方法。方法對手部濕疹急性期患者進行隨機分組,以加味苦參湯濕敷作為中藥治療組,并設(shè)立氫化可的松溶液濕敷作為西藥對照組,其中中藥治療組30例,西藥對照組30例。在研究過程中保存病人的相關(guān)資料(治療前、后的皮損評分、瘙癢程度評分、照片)治療1周為1療程,分別在治療1個療程和2個療程后觀察兩組患者治療前后相關(guān)指標,并進行統(tǒng)計學處理。結(jié)果(1)一般資料比較：經(jīng)分析中藥治療組與西藥對照組樣本在年齡、性別以及病程方面不具有統(tǒng)計學意義(P0.05),具有可比性。(2)皮疹積分比較：兩組患者治療一周和兩周后皮損皆有好轉(zhuǎn),其中中藥治療組治療一周后和治療兩周后療效皆優(yōu)于西藥對照組(P0.05),且有統(tǒng)計學意義。(3)瘙癢程度改善比較：兩組患者在經(jīng)過一周治療后瘙癢均有減輕,但其程度無明顯差異(P0.05),無統(tǒng)計學意義；治療兩周后,瘙癢有不同程度的減輕,且中藥治療組優(yōu)于西藥對照組(P0.05),差異有統(tǒng)計學意義。(4)兩組患者經(jīng)過2個療程的相應(yīng)治療,中藥治療組總有效率為96.7%,其中基本治愈4例,顯效25例,有效0例,無效1例；西藥對照組總有效率為80.0%,其中基本治愈1例,顯效14例,有效9例,無效6例,中藥治療組治療效果明顯優(yōu)于西藥對照組(P0.005),具有統(tǒng)計學意義。結(jié)論在手部濕疹急性期患者的臨床治療中使用加味苦參湯濕敷,能夠明顯改善患者的臨床癥狀,雖然其起效時間較激素濕敷長,但遠期治療效果優(yōu)于激素濕敷,可以作為手部濕疹急性期患者臨床治療的一種新的選擇方案,在臨床中推廣應(yīng)用。
[Abstract]:Objective to study the clinical effect of Jiawei Sophora decoction on acute hand eczema, observe the effect of Jiawei Sophora decoction on relieving skin damage and pruritus in acute stage of hand eczema, and explore its mechanism of treating hand eczema in acute stage. To develop new ideas and methods for clinical treatment of eczema. Methods the patients with hand eczema in acute stage were randomly divided into two groups: the treated group was treated with modified Kushen decoction and the control group was treated with hydrocortisone solution wet compress. 30 cases were treated with traditional Chinese medicine and 30 cases were treated with western medicine. During the course of the study, the relevant data of the patients (before and after the treatment, the score of pruritus, the photo) were kept for one course of treatment. The related indexes before and after treatment were observed in the two groups, respectively, after one course of treatment and two courses of treatment. Statistical analysis was carried out. Results (1) comparison of general data: the samples of the Chinese medicine treatment group and the western medicine control group were analyzed at the age, Sex and course of disease were not statistically significant (P0.05), comparable. (2) rash score comparison: two groups of patients after one week and two weeks of treatment of skin lesions improved, After one week treatment and two weeks treatment, the curative effect of the Chinese medicine treatment group was better than that of the western medicine control group (P0.05), and there was statistical significance. (3) the degree of pruritus was improved by comparison: after one week treatment, the pruritus of the two groups were all alleviated. But there was no significant difference (P0.05), there was no statistical significance; after two weeks of treatment, itching was alleviated to some extent, and the Chinese medicine treatment group was superior to the western medicine control group (P0.05). (4) the two groups of patients after two courses of corresponding treatment, the Chinese medicine treatment group is better than the western medicine control group (P0.05). (4) the two groups of patients after two courses of corresponding treatment, The total effective rate of the Chinese medicine treatment group was 96.7%, of which 4 cases were basically cured, 25 cases were markedly effective, 0 cases were effective, and 1 case was ineffective, while the total effective rate in the western medicine control group was 80.0%, in which 1 case was basically cured, 14 cases were markedly effective, 9 cases were effective, and 6 cases were ineffective. The therapeutic effect of traditional Chinese medicine group was significantly better than that of western medicine control group (P 0.005), with statistical significance. Conclusion the application of modified Kushen decoction in the treatment of hand eczema in acute stage can obviously improve the clinical symptoms of the patients. Although the effective time is longer than that of hormone wet compress, the long-term therapeutic effect is better than that of hormone wet compress. It can be used as a new choice for the treatment of hand eczema in acute stage.
